Diagnosis and treatment of cystic disease of the thyroid by aspiration.

One hundred forty-one cases of euthyroid solitary "cold" and "uniform" nodules, each of which contained at least 3 ml of fluid obtained on aspiration, are presented. Sixty-three patients had complete disappearance of swelling after one aspiration. Seventy-eight patients were selected for surgery on the basis of cytologic findings of malignancy in the aspirated fluid (one patient), on the basis of recurrent cyst formation after one aspiration (17 patients; in these cases the incidence of malignancy was similar to that in solid, cold nodules), and on the basis of palpable tissue after aspiration that indicated a partially cystic lesion (60 patients). Surgicopathologic findings revealed six malignant lesions; all of these nodules were more than 3 cm in diameter. In percentage, 6.6% of the partially cystic and 2.5% of the purely cystic nodules were malignant. The color of the aspirated fluid did not add to the discriminant value of the aspirate. The identification of malignant conditions in thyroid cysts was handicapped by the yield of acellular material in three of five cases of malignant disease and in more than 50% of benign lesions. However, the study suggests that aspiration biopsy of cystic thyroid nodules when used in association with the above clinical criteria is a useful technique of selecting patients for operation and, by itself, is a safe and reliable means of treating the disorder.

Carcinoma of the thyroid: differential behaviour in solitary and multinodular tumours.

One hundred and sixty-three cases of thyroid carcinoma were treated between 1979 and 1983. The cancers were classified as solitary nodules or carcinomas in multinodular goitres according to their clinical presentation. Analysis revealed a significantly higher incidence of T0/T1 in the solitary nodular carcinoma group (P less than 0.001) and a lower incidence of metastatic disease (P less than 0.05) compared with multinodular carcinoma. Survival for the T0/T1 subgroup of solitary nodular carcinoma was significantly better compared with the same subgroup of multinodular carcinoma (P less than 0.05).

The effect of trauma on serum C3 activation and its correlation with injury severity score in man.

The sera of 12 patients with mechanical trauma were studied to determine C3 levels and activation. The injury severity score (ISS) was then related to serum C3 levels and activation. It was found that in the immediate postinjury period, serum C3 activation occurred in cases where ISS was greater than or equal to 12. The mean ISS of patients with complement activation was 25.2. In comparison, in patients with nonactivation of complement, the mean ISS was 9.5 (p less than 0.05). Serum C3 levels were inversely related to ISS. The mean serum C3 level of patients with ISS greater than or equal to 12 was 73.3 mg% and mean serum C3 level with ISS less than 12 was 109.4 mg%. This difference was again statistically significant (p less than 0.05). There is indication that complement depletion occurs in the immediate postinjury period in moderate to severe injury (ISS greater than or equal to 12). This finding could explain, in part, the immunosuppressive effect of trauma and can be used as a marker to predict possible septic complications.

Copper intravas device (IVD) and male contraception.

An alternative method of male contraception is crucial at this stage of increasing population in the developing world. A non-obstructive, non-hormonal, reversible method of male contraception would find acceptance in oriental communities. Our experience with copper IVD in small animals has been encouraging. We present here an extension of the use of the copper IVD in the rhesus monkey. Treated male rhesus ejaculate showed fifteen-to thirty-five-fold increase in copper levels up to 10 months after insertion of Cu IVD . Semen analysis showed an increase of dead sperm (23.0% - 52.3%; control 4.5% - 14.0%) and decrease in percentage of motile sperm (10-30%; control 70-80%). Fertility studies using treated male rhesus of proven fertility from the breeding colony mated with cycling females of proven fertility showed highly significant reduction of fertility up to 13 months (P less than 0.001). Reduction in fertility of the treated rhesus was also significant up to a period of 23 months (P less than 0.01). A reversal of contraceptive effect and increase in sperm count and motility is observed after 13 months post-treatment. Studies are required to achieve an increase in the effective life span of the IVD .

Leiomyosarcoma of the colon: a case report and review of literature.

Leiomyosarcoma of the colon, excluding that of the rectum, is extremely rare. We report a case of leiomyosarcoma of the ascending colon with a brief review of literature. The clinical manifestations in our case were typical: abdominal pain, diarrhea, anorexia, and a palpable tumor that was firm, smooth, and mobile inspite of its bulk. Toxic features, cachexia and weight loss were less prominent. Morphologically the tumor was of dumbbell type with intracolic and extracolic components. The microscopic picture, consisting of spindle-shaped smooth-muscle cells with pleomorphism and one to three mitotic figures per high-power field, was characteristic. In the absence of local spread, a hemicolectomy was considered curative, but our patient developed extensive metastasis in 25 months. The correlation between mitotic activity of the tumor and prognosis in our case was consistent with that reported in literature.

Further studies of an intravasal copper device in rats.

A 1.5 cm long copper wire caused complete inhibition of fertility in male rats when placed properly in the vasal lumen. The contraceptive effectiveness of the device was directly proportional to the surface area of the device and was non-obstructive. The release rate of copper in vivo from a 1.5 cm long wire was 11.6 micrograms/day during the first month and 6.8 micrograms/day over the first 3 months. The contraceptive action of the wire starts about 20 days after insertion. Fertility was regained in 67% of the rats by 90 days after removal of the device. Similar devices of stainless steel and silver did not affect fertility.
