[Immunological aspects of pulse therapy in systemic lupus erythematosus].

Twenty SLE patients were examined against a background of pulse-therapy with methylprednisolone. The analysis showed that there was a decrease in the CIC concentration, antibodies to native DNA and an increase in the level of C3c and C4 components of the complement against a background of pulse-therapy. A more rapid time course of the CIC level was noted shortly after pulse-therapy as compared to changes in other immunological indices. A dynamic study of immunological indices in SLE against a background of pulse-therapy was appropriate for a clinical assessment and a study of the mechanisms responsible for the therapeutic efficacy of this method.

[Combined use of impact doses of 6-methylprednisolone and cyclophosphamide in patients with systemic lupus erythematosus].

The priming doses of 6-methylprednisolone (6-MP) and cyclophosphamide (pulse therapy) were used for the treatment of 23 patients with SLE associated with primary involvement of the kidneys and pronounced immunologic disorders. The effect of combined therapy was evaluated on the extrarenal manifestations of SLE, proteinuria, hematuria, glomerular filtration, creatinine, antibodies to native DNA (anti-nDNA), complement, cryoprecipitins, antinuclear factor, and circulating immune complexes (CIC). The treatment efficacy was evaluated on day 4 since the beginning of the treatment and on the patients' discharge from hospital. The data obtained point to a significant increase of glomerular filtration and complement level, a reduction in CIC and anti-nDNA and cryoprecipitins within the first day after discontinuation of the treatment. The diminution of proteinuria and improvement of the urinary sediment were seen in over 50% of the patients. Variation of the laboratory findings correlated well with the improvement of the disease clinical picture. The combination of the priming doses of 6-MP and cyclophosphamide holds promise in the treatment of patients with associated SLE and grave renal involvement, marked immunologic disorders and generalized autoimmune vasculitis.

[Prognosis and treatment of patients with systemic lupus erythematosus in different age groups].

The paper is concerned with observation over 470 patients with systemic lupus erythematosus (SLE) admitted to the Institute of Rheumatology, Academy of Medical Sciences of the USSR, since December 1958 to December 1983. Women (453) with a disease onset at 10 to 55 years (355 had associated SLE and nephritis, 155 presented with SLE without nephritis) accounted for the absolute majority of cases. Comparison of the disease clinical picture in 2 groups of patients, who had fallen ill at 14 to 24 years (278 subjects) and at 40 to 55 years (25 subjects) revealed a graver clinical picture in the group of patients, who had fallen ill at a younger and (nephritis in 82% against 56% in the group of older patients) and a considerably less survival as compared to the group of older patients despite more intensive care including pulse-therapy with methyl prednisolone.
