Cocaine use during pregnancy. Research findings and clinical implications.

Cocaine's popularity has escalated drastically within the past decade, and pregnant women from all socioeconomic, racial, and cultural groups are among the burgeoning number of users. The pharmacologic properties of this highly addictive, mind-altering drug and its profound effects on the pregnant woman and fetus are presented. Recent clinical research findings on cocaine use during pregnancy are reviewed, implications for nursing care are examined, and specific nursing interventions are identified.

Massive cardiac involvement by malignant lymphoma.

Cardiac infiltration by lymphoma is a rare finding during life. However, metastatic deposits are frequently found at postmortem in patients with malignant lymphoma. In the past, this may have been due to difficulties in imaging the heart. Today, 2-dimensional echocardiography is a widely used, safe, noninvasive test which allows excellent spatial imaging of cardiac structures. We report a case of extensive primary myocardial and pericardial involvement by malignant lymphoma in which echocardiography was used to document the diagnosis and follow the response to chemotherapy.

Contraceptive practices in HIV seropositive females in Ireland.

A questionnaire was administered to a cohort of 130 HIV seropositive women attending the Department of Genitourinary Medicine, St James's Hospital, Dublin, in order to establish their current contraceptive practices. Forty (42%) of the sexually active women did not use reliable contraception; 55 (57%) used a method reliable in fertility control only, and 21 (22%) used a method reliable in terms of both partner and congenital transmission. The intravenous drug users were less likely to use reliable contraception, 9 (39%), in comparison to the heterosexual group 14 (70%), P < 0.05. The heterosexual group were more likely to use condoms consistently, 7 (35%) in comparison to one (4%) of intravenous drug users, P < 0.02. Nineteen children, 3 borne to heterosexual mothers and 16 borne to intravenous drug-using mothers were HIV seropositive after 18 months.

Power and authority: their meaning for group practice administrators.

The word power is filled with meaning. It can't be spoken without creating some type of reaction. Power is a key issue in our lives. Authors Mikel Lynch and Seth Allcorn, Ph.D., look at power and authority through a scenario involving a physician and an administrator to show how it can be used effectively in a group practice setting.

Pain as the fifth vital sign.

Patients who do not report pain and healthcare providers who fail to assess for pain are major barriers to the relief of pain. Using pain as the fifth vital sign and being knowledgeable about pain assessment and management can help nurses and other healthcare providers overcome many of the barriers to successful pain control. A successful pain control plan includes establishing the pain diagnosis, treating the cause of the pain when possible, optimizing analgesic use, implementing nonpharmacological interventions to maximize physical and psychological comfort and function, and referring the patient for invasive pain management options when indicated.

Special children, special needs: the ectodermal dysplasias.

Ectodermal dysplasias are a group of genetic disorders that arise from abnormalities of the ectoderm. Children with the disorder exhibit a wide range of clinical features that frequently affect the skin, teeth, hair, and sweat glands. With early detection and careful management, these children can lead normal, productive lives.

Flowsheet documentation of chemotherapy administration and patient teaching.

The use of a flowsheet to document chemotherapy administration is an established practice in many oncology settings. Traditionally, these flowsheets have focused on dates of drug administration, drug doses, and laboratory values. In an attempt to simplify documentation of nursing practice related to chemotherapy administration, sections on drug administration, patient teaching, and symptom management were added to the traditional chemotherapy flowsheet in our setting. Development of a standard of care for knowledge deficit related to chemotherapy and standardized patient-teaching materials allowed use of the flowsheet to document patient teaching. This standard also provided consistency and continuity in patient teaching. Additionally, the format of the flowsheet and the charted information has simplified data retrieval for quality-improvement monitoring of some aspects of care.

Henoch-Schonlein purpura: a case study.

Henoch-Schonlein Purpura is best recognized by a characteristic petechial/purpuric skin rash. Clinical features arise from a widespread vasculitis, affecting many systems. Nursing interventions focus on family education and supportive measures to alleviate symptoms and prevent complications.

"Céad Míle Fáilte"--an assessment of the screening of 178 Bosnian refugees to Ireland.

The number of refugees in Western Europe has risen dramatically. Such an increase poses unique challenges for all health personnel. This paper describes how the Republic of Ireland, a country with little experience of refugees, initially coped with 178 Bosnian refugees. An open-access general practitioner service was combined with a general screening protocol. Patients with significant problems tended to visit GP's, for appropriate treatment, soon after arrival. Of a total of 92 problems diagnosed at screening, general medical (14%), psychiatric (14%) and gynaecological (11%) were the most frequently encountered. Of the 30 problems referred, 80% were referred by a GP. Deficiencies in immunisation and drug history are highlighted. We make recommendations to improve the quality of future initial refugee assessment procedures.
