Erythrocyte sedimentation rate in women with different manifestations of joint disease.

A comprehensive population study of women aged 44-46 was carried out in Göteborg, Sweden in 1974-75. The mean erythrocyte sedimentation rate (ESR) and the rate of high ESR values (defined as ESR greater than or equal to 30 mm) were higher in women with manifestations of joint diseases than in other women, the differences being statistically significant for women with swollen or deformed finger joints and symptoms from the wrists. Women with manifestations of active joint disease at the time of the examination had even higher ESR values, the differences being statistically significant also for women with symptoms from the finger joints. High ESR values were more common in women with a positive serological test for rheumatoid factor and joint manifestations indicating arthritis and osteoarthrosis. The "arthritis" and "osteoarthrosis" groups each included about one-third of the subjects with rheumatoid factor. It is concluded that although ESR values are moderately increased in subjects with different manifestations of joint disease, the vast majority of such subjects have ESR values less than 30 mm.

The C-wave of the rabbit electroretinogram during dark-adaptation and the steady-state.

C-waves of the rabbit ERF under urethane anesthesia were elicited by a Grass photostimulator during dark adaptation and under steady-state conditions. After strong pre-adaptation with light, the amplitude and time-to-peak of the c-wave increased in parallel with the a- and b-waves, reaching a maximum of 90-130 min. In the dark-adapted steady-state, repetitive stimuli at intervals ranging from 15 sec to 5 min elicited a stable c-wave response except for a small dip in amplitude that occurred 2-6 min after initiating the shortest interval (15 sec) flashes. Although the changes in light and dark used in these experiments should have elicited shifts in the standing potential, no fluctuations of the c-wave amplitude were evident with a time course corresponding to a light response of the standing potential.

Ovarian teratoma with peritoneal gliomatosis and elevated serum alpha-fetoprotein.

An adolescent girl, had a grade 1 immature teratoma combined with a rare manifestation of glial peritoneal implants of grade 0. A unilateral salpingo-oophorectomy was performed. Alpha-fetoprotein levels, which were elevated before the operation, returned to normal. The patient has been symptom-free for more than three years after the surgical treatment. Repeated normal assays for alpha-fetoproteins can serve as a guide to a projected favorable course. The World Health Organization grading of the teratoma is listed and pertinent medical literature is briefly discussed.

The effect of timolol vs placebo on angina pectoris.

The effect of timolol vs placebo on the frequency of anginal episodes, nitroglycerin consumption and exercise performance was investigated in a double-blind, randomized, crossover study in 23 patients with angina pectoris. The optimal dose of timolol (10-30 mg twice daily) for each patient was titrated by exercise studies. Compared with placebo, timolol decreased the weekly number of anginal attacks and the weekly number of nitroglycerin tablets consumed, reduced the resting heart rate, systolic and diastolic blood pressure, and product of systolic blood pressure times heart rate, decreased the heart rate, systolic and diastolic blood pressure, and product of systolic blood pressure times heart rate at the onset of angina pectoris or marked fatigue, prolonged exercise duration, and diminished electrocardiographic evidence of myocardial ischemia. Timolol is an excellent antianginal agent when prescribed twice daily, with the optimal dose titrated by exercise studies.

Cross-sectional echocardiographic observations on the mechanism of preservation of the opening snap in calcific mitral stenosis.

Cross-sectional echocardiography (CSE) was performed on 19 patients with mitral stenosis (MS). Nine patients (group 1) had little or no mitral valve (MV) calcification; ten patients (group 2) had heavy MV calcification. An opening snap was present in each of the patients in group 1. Of the group 2 patients, five (group 2A) had an opening snap and five (group 2B) did not. CSE in the long axis revealed in group 1 patients a mobile MV with sharp anterior arching of the anterior leaflet in early diastole; in group 2A, dense echoes suggestive of calcification were noted near the tip of the MV, while the body of the leaflet was relatively thin and it arched anteriorly sharply in early diastole; and in group 2B, dense echoes were noted at the tip and body of the MV with reduction in mobility of the valve, and the sharp anterior arching of the anterior leaflet in early diastole was absent. Preservation of the OS in calcific MS depends on the distribution of calcification. Heavy calcification confined to the tip of the MV allows the anterior leaflet to arch anteriorly in early diastole, allowing preservation of the OS, whereas heavy calcification of the tip and body of the MV reduces the pliability of the valve with consequent disappearance of the OS.

Regional distribution of the MB isoenzyme of creatine kinase in the human heart.

Human myocardial tissue obtained at autopsy from ten patients was examined for content of the MB isoenzyme of creatine kinase (CK-MB). We wished to determine whether this isoenzyme is distributed homogeneously throughout the heart. In eight cases, there was no history or pathological evidence of heart disease. Two had a history of previous myocardial infarction; in these, tissue was obtained from sites distant from the scar. Difference was found between the CK-MB content of the right atrium and the left atrium, and between the right ventricle and left ventricle. In all cases, the CK-MB content of the right side of the heart significantly exceeded that of the left side of the heart. Statistically significant differences were also found between the CK-MB content of the anterior interventricular septum and that of the posterior septum. These topographical variations in CK-MB content may be related to differences in the density of contractile elements in various parts of the heart and, moreover, are not taken into account in the enzymatic estimation of infarct size.

Congenital adrenal hyperplasia diagnosed in a middle-aged woman.

Short stature, severe hirsutism, an enlarged clitoris and lack of vaginal introitus were manifest in a 55-year-old female karyotype, male phenotype patient brough up as a female. The hormone profile suggested congenital adrenal hyperplasia due to 21-hydroxylase adrenal enzyme deficiency. Follicle-stimulating hormone and luteinizing hormone levels were 122.5 and 100.3 mIU/ml, respectively, within the normal range for a postmenopausal woman. Computerized axial tomography revealed a large cystic tumor connected to the right adrenal gland. Both the right adrenal gland and the cyst were removed. The adrenal cortex showed diffuse hyperplasia and contained a well-defined nodule. The case was interesting because we had the opportunity of examining a patient of advanced age with untreated congenital adrenal hyperplasia and because of the very rare combination of adrenogenital syndrome with nodular hyperplasia and a cystic tumor, which was most probably a degenerated adenoma.

Similarities between the c-wave and slow PIII in the rabbit eye.

The c-wave of the electroretinogram was recorded from the eyes of Dutch rabbits and compared with the slow PIII response isolated by intravenous infection of sodium iodate and intravitreal injection of sodium aspartate. Although opposite in polarity, the waveforms of the two responses were remarkably similar over a wide range of stimulus intensities and durations. Plots of time-to-peak vs. log stimulus energy show that both responses follow the Bunsen-Roscoe law. Curves plotting the locus of all points for which the Bunsen-Roscoe law held were approximately parallel for the two responses, and the critical durations for each stimulus intensity were equivalent. The slow PIII peaked earlier than the c-wave, consistent with published observations that the time-to-peak of light-induced K+ changes is shorter near the photoreceptor inner segments than near the pigment epithelium. These data support the hypothesis that there is a common generator for the c-wave and slow PIII.

Acebutolol in supraventricular arrhythmias.

The effect of intravenous acebutolol on supraventricular arrhythmias was evaluated in 20 patients, 5 with chronic obstructive pulmonary disease. A rapid ventricular rate in atrial fibrillation was slowed greater than 15% in all of 10 patients by acebutolol and in none of 5 patients by saline. A rapid ventricular rate in atrial flutter was slowed greater than 15% in all of 6 patients by acebutolol and in none of 3 patients by saline. Frequent premature atrial beats were abolished or reduced by greater than 75% in each of 2 patients by acebutolol and not in a patient by saline. Acebutolol converted multifocal atrial tachycardia to sinus rhythm in a patient. Digitalis-induced nonparoxysmal atrioventricular junctional tachycardia was not affected by saline but was abolished by acebutolol in a patient. Acebutolol was well tolerated in each of 5 patients with chronic obstructive pulmonary disease. Acebutolol is useful in the treatment of supraventricular arrhythmias.

Treatment of premature ventricular complexes with acebutolol.

The effect of intravenous acebutolol versus saline solution on frequent premature ventricular complexes was evaluated in a double-blind, randomized study in 20 patients, including 3 with chronic obstructive pulmonary disease. Frequent premature ventricular complexes were abolished or reduced by 75% or more in none of 12 patients given saline solution but in 18 of 20 patients (90%) given acebutolol (P less than 0.001). This therapeutic effect of acebutolol persisted for at least 2.5 hours in 17 of 20 patients (85%), for at least 3.5 hours in 14 (70%) and for at least 4 hours in 8 (40%). Acebutolol was well tolerated by the three patients with chronic obstructive pulmonary disease. These data indicate that intravenous acebutolol is useful in the treatment of premature ventricular complexes.

Effect of prazosin vs placebo on chronic left ventricular heart failure.

The effect of the vasodilator prazosin vs placebo on exercise duration until marked dyspnea, and on left ventricular function measured by echocardiography, was evaluated in a double-blind, randomized study in 24 patients with chronic left ventricular failure despite digitalis and diuretic therapy. Compared with the double-blind placebo, prazosin reduced resting systolic and diastolic blood pressure and systolic blood pressure times heart rate, improved clinical symptoms, decreased cardiothoracic ratio measured by chest roentgenography, decreased left ventricular and left atrial dimensions, improved ejection fraction and Vcf measured by echocardiography, and improved treadmill exercise duration. All 12 patients taking prazosin had greater than or equal to 20% improved treadmill exercise duration; none of 12 receiving placebo improved. In six of 12 patients taking prazosin, roentgenographic evidence of pulmonary venous congestion disappeared compared with none of the patients on placebo. These data suggest that prazosin may be effective in treating chronic left ventricular failure.
