Incidence of long-term sick-listing in an urban area of Sweden and its relationship with demographic data of the population.

By checking the card indexes of seven out of twelve Social Insurance Offices covering 66% of the total wage-earning population in the city of Göteborg the patients recorded for 90 days of continuous sick-listing were classified into four diagnostic categories according to the doctor's certificate: "non-specific pain" and "specific pain" of the musculoskeletal system, "other pain" and "non-pain" diagnoses. The overall yearly incidence of 90 days' sick-listing averaged 5.4%. A significant correlation was found between the incidence of 90 days' sick-listing due to "non-pain" and musculoskeletal pain diagnoses and the proportion of demographic characteristics of the areas. The hypothesis of presuming the highest association between non-specific pain diagnoses and demographic factors was rejected.

Exploring partner communication and patterns of sexual networking: qualitative research to improve management of sexually transmitted diseases.

This ethnographic research among rural South African men and women seeking care for STDs explores constraints to communication among partners and defines predominant patterns of sexual networking. A series of fifteen open-ended interviews explored topics related to sexual networking and partner communication. Patterns of sexual networking showed clear gender variation, with men more likely to discuss multiple partners. Both men and women believed they should tell one partner about their STD, most often a steady partner. Strong distinctions were made between regular and casual partners, with an emphasis on the role of trust in a steady relationship. Men and women expressed anxiety about telling their partners of their illness, but women more often expressed fear whereas men felt embarrassed. Multi-partnered sexuality is common and is widely accepted for men. While the importance of regular partnerships is clear for both men and women, sexual relationships appear to be highly unstable. Communication among partners could be facilitated by stronger health promotion messages, such as the importance of telling a partner about an STD. Prevailing patterns of communication and sexual networking have profound implications for the STD epidemic.

A randomized prospective study of vocational outcome in rehabilitation of patients with non-specific musculoskeletal pain: a multidisciplinary approach to patients identified after 90 days of sick-leave.

This study was designed to evaluate the effectiveness of a multidisciplinary rehabilitation programme offered to a general population with 90 days of sick-leave due to non-specific musculoskeletal pain. The results concerning return to work and re-sick-listing during a follow-up period of five years were evaluated for Swedes and immigrants separately. Compared to a control group, the rehabilitation offer resulted in improved work stability after work return among the Swedes. The immigrants, as a group, did not benefit from the programme compared to the controls in primary care.

Local terminology for febrile illnesses in Bagamoyo District, Tanzania and its impact on the design of a community-based malaria control programme.

This paper reviews results of several ethnographic studies that have examined the issue of local terminology for malaria in Africa, then presents findings from an on-going study in Bagamoyo District, Tanzania. The study used a mixture of qualitative and quantitative interview methods to examine local perceptions of malaria and malaria treatment practices. Although the local term homa ya malaria or malaria fever appeared on the surface to correspond closely with the biomedical term malaria, significant and often subtle differences were found between the two terms. Of perhaps greatest importance, common consequences of malaria in endemic areas such as cerebral malaria in young children, severe anaemia and malaria in pregnancy were not connected with homa ya malaria by many people. A set of guidelines are described that were used to determine how best to promote acceptance and use of insecticide-impregnated mosquito nets, given these results. It is demonstrated that the position of the term used to denote malaria in the local taxonomy of febrile illnesses has important implications for the design of health education interventions.

Cyfra 21-1. A new potential tumor marker for squamous cell carcinoma of head and neck.

OBJECTIVE: Evaluation of Cyfra 21-1 (cytokeratin fraction 21-1) in squamous cell carcinoma of the head and neck. DESIGN: Prospective study. PATIENTS: Serum Cyfra 21-1 concentration was measured in 250 samples from patients with squamous cell carcinoma of head and neck, patients with benign tumors of head and neck, healthy control subjects, and patients in remission from squamous cell carcinoma of head and neck. RESULTS: Cyfra 21-1 concentration was elevated in 60% of the new patients with squamous cell carcinoma but only in 8% of patients with benign tumors and 3.5% of the healthy controls. At a cutoff of 1.3 ng/mL, the sensitivity of the test was 60%, the specificity was 94%, positive predictive value was 75%, and negative predictive value was 89%. The marker levels tended to follow the clinical course of the disease and were useful for therapy monitoring. Cyfra 21-1 levels were in good correlation with the tumor stage expressed by the local (T) and the lymphatic spread (N) and were inversely correlated with histologic grade, eg, higher in poorly differentiated carcinoma than in well-differentiated squamous cell carcinoma. CONCLUSION: Cyfra 21-1 evaluation in head and neck squamous cell carcinoma is worthwhile for performance of an ample study that will prove and establish its routine use.

Choriocarcinoma of the bladder. Report of a case of primary tumor or late metastasis of a molar pregnancy.

Choriocarcinoma of the bladder was diagnosed in a 54-year-old woman presenting with macroscopic hematuria 17 years following evacuation of a molar pregnancy. The patient was treated by cystoscopic transurethral tumor resection followed by three courses of triple-agent chemotherapy and total abdominal hysterectomy with bilateral salpingo-oophorectomy. Six months later a gradual rise in beta-human chorionic gonadotropin levels led to the diagnosis of recurrent bladder tumor at the site of the previous tumor. The patient underwent a conservative partial cystectomy, and 12 months postoperatively there was no evidence of disease. It is unclear whether the patient developed a primary urinary bladder choriocarcinoma or late metastatic disease from the previous molar pregnancy.

Development of vaginal adenosis following combined 5-fluorouracil and carbon dioxide laser treatments for diffuse vaginal condylomatosis.

BACKGROUND: The management of vaginal human papillomavirus (HPV) lesions may be difficult. We report an unusual long-term consequence of treatment. CASE: A 40-year-old woman presented with vaginal adenosis following treatment of multiple vaginal condylomata by endovaginal applications of 5% 5-fluorouracil cream and vaporization of the remaining subclinical condylomata by CO2 laser. CONCLUSION: The approach to the management of vaginal condylomatosis should take into consideration the possible later occurrence of adenosis.

[Severely handicapped persons living at home].

The records of 34 economically demanding cases according to the Danish legislation on public assistance section 48, part 3 or the legislation on pensions section 17 were reviewed. The persons concerned were mainly young people who remained in their own homes despite severe handicap. Where 2/3 were concerned, progressive disease appeared to be involved. In addition, economical deliberations and assessment of the quality of life were considered.

Anatomic evidence of spontaneous intrauterine closure of a ventricular septal defect.

An infant, born to a mother on carbamazepine monotherapy, had a complex cardiac anomaly, consisting of double-outlet right ventricle (DORV), right-sided aorta, pulmonary artery hypoplasia, left ventricular endocardial fibroelastosis, and anatomic evidence of a spontaneously closed muscular ventricular septal defect (VSD). The last finding is one which, to the best of our knowledge, has never been illustrated before.

Respiratory function in chronic primary fibromyalgia.

Since patients with severe chronic primary fibromyalgia (CPF) report effort dyspnoea, respiratory function was studied in 87 consecutive women with CPF according to Yunus' criteria. Bernstein spirometry, maximum inspiratory (MIP) and expiratory (MEP) pressures were obtained in patients, and in a reference group of 61 healthy women. MIP was considerably lower in patients than in controls (3.6 +/- 2.0 vs. 8.0 +/- 2.2 kPa, p less than 0.0001), as was MEP (3.1 +/- 2.1 vs. 8.3 +/- 2.2 kPa, p less than 0.0001). Patients who had previously reported dyspnoea at a bicycle exercise test showed significantly lower values of respiratory pressures than patients without dyspnoea. Respiratory pressures were reproducibly low in CPF patients. Spirometric values were normal among patients and controls. We conclude that maximum expiratory and inspiratory pressures are low in CPF, a finding which may indicate respiratory muscle dysfunction in this syndrome.

Reliability of articular indices and function tests in a population study of rheumatic disorders.

Reliability and method error were assessed for 25 clinical tests by test-retest on 31 subjects with rheumatic disorder (RD) and on 28 consecutive non-RD of a reference group (REF) from a female population sample. Low systemic differences were found for joint mobility tests in the RD group and correlation coefficients were generally above 0.7. A significant difference in the RD group was found only for climbing stairs, and in the REF group for shoulder external rotation and flexion, wrist extension and grip strength. The relative method error of grip strength was large (17%). A new assessment of activities of daily living could not be fully evaluated because the population studies had limited disability, but the error was acceptable. Articular indices (Lansbury, Ritchie, and American Rheumatism Association) showed large method errors (9-25%), acceptable test-retest correlations (greater than 0.75), and a systematic difference only in the Ritchie index. The analyses showed that a number of clinical tests are adequate for population studies of RD, but the metrical properties of these tests must be considered in the planning of clinical and population studies.

Dyspnoea in chronic primary fibromyalgia.

We investigated the prevalence of dyspnoea, according to a 5-grade score proposed by the World Health Organization (WHO), among 87 consecutive women (age 44.0 +/- 8.4, range 26-65 years), with severe chronic primary fibromyalgia (CPF). In total 73 women (84%) were dyspnoeic, 47, 17, six and four patients reported dyspnoea corresponding to WHO grades 1, 2, 3 and 4, respectively. In multivariate analysis WHO dyspnoea grade was the most important determinator of exercise capacity. The higher WHO dyspnoea grade reported, the lower exercise capacity and heart rate were reached at graded bicycle exercise tests (r = -0.48, P less than 0.001 and r = -0.40, P less than 0.001). WHO dyspnoea grade correlated directly to breathing frequency at rest (r = 0.45, P less than 0.001) and inversely to maximum inspiratory pressure (r = -0.29, P less than 0.01). It did not correlate with maximum expiratory pressure, nor with spirometric measurements of pulmonary function. It is concluded that dyspnoea is common among CPF patients and is not explained by cardiac or pulmonary causes, but may partly be due to diaphragmatic muscular insufficiency and physical inactivity.

The population study of women in Gothenburg 1980-81--the third phase of a longitudinal study. Comparison between participants and non-participants.

A representative population sample comprising 1,462 women was studied in Gothenburg, Sweden in 1968-69, and a third follow-up study was carried out in 1980-81. The participation rates in the baseline study and during the follow-up studies were high. In 1980-81 women in two new age strata, aged 26 and 38, were added. Women who had moved to or from Gothenburg during the study period were not found to differ from those who were living in Gothenburg during the total study period, while there were a few differences of statistical significance between refusers and participants in 1980-81. The mortality among initial refusers was about doubled compared to that of those who participated in the baseline study.
