1. Osmotic pressure determinations of dissociated arachins are a particularly suitable method for determination of the number of sub-units in the protein, because they yield a number-average molecular weight. 2. Arachin, in 8m-urea-0.1m-sulphite, produces 12 sub-units from the form of molecular weight 345000. 3. When the urea concentration is varied the molecules became fully dissociated at 6m-urea-0.1m-sulphite. Although sulphite is necessary to break disulphide bridges, concentrations greater than 0.1m cause a re-aggregation of the sub-units. Similar results were obtained in guanidine solutions. 4. A new form of arachin has been discovered, A1, migrating more rapidly than arachin A. 5. The N-terminal residues of arachin have been re-investigated on more highly purified samples: they are glycine, valine and (iso)leucine in the proportions 4:1:1. 6. The three forms of arachin have the structure (B) beta(4)gammadelta, (A) alpha(2)beta(2)gammadelta and (A1) alpha(4)gammadelta, for the forms of molecular weight 170000. 7. Dissociation in 8m-urea produces some fragments, detected by gel electrophoresis, which appear to be dimers of the type alpha-S-S-beta, beta-S-S-beta, held together by disulphide bonds.

Although it is unlikely that the legislative endeavors of 1996 and 1997 relating to the health care industry will be matched in 1998, implementing regulations of the Balanced Budget Act and the Health Insurance Portability and Accountability Act will predicate the successes or shortcomings of these new laws in 1998. It is important that NPs stay in close contact with their state legislators and Medicaid officers responsible for implementing the SCHIP. NPs should also closely track efforts at the Federal level to ensure inclusion of non-physician providers in legislation that is important to NPs, such as bans on "gag" clauses--a protection that should include all health care providers.
Bloodletting has been practised in most cultures for the past two millennia. Originally widely applied, it is now only indicated for a small number of specific medical conditions. Bloodletting declined in the 19th century at a time of revolution in medical thought, and examining its decline illustrates the forces that have shaped modern medicine.
Pleomorphic adenoma is the most common neoplasm of the parotid gland, generally presenting as a slowly enlarging, firm, well-circumscribed, painless nodule. Occasional cases have presented after a short period of rapid growth or have been associated with pain. The vast majority of these tumors are solid, but rare examples have been associated with cystic degeneration or hemorrhage. Spontaneous and tumor-associated infarction of the parotid has been reported, but these examples have been limited to infarctions of Warthin's tumors and postoperative infarctions of salivary glands. We present the case of a 48-year-old male with a one-year history of a painful, enlarging, left parotid mass associated with paresthesia of the tongue. Computed tomographic examination of the parotid demonstrated a left superficial lobe mass with a rim of enhancement and low attenuation center. Fine needle aspiration yielded necrotic debris and atypical squamous elements that were thought to be compatible with carcinoma. A superficial parotidectomy with intraoperative frozen section revealed a pleomorphic adenoma with extensive central necrosis. To our knowledge, this represents the first reported case of an infarcted pleomorphic adenoma and illustrates the potential for misinterpretation of these cytologic and radiologic findings as indicative of malignancy.
This report provides a brief overview of the July, 1991 mental health team visit to the Magadan and Chukotka regions. A previous trip to the region was made by a mental health team in the summer of 1990. The sites covered were Anadyr, Magadan, Yagodnoye and Sinegorye. There are several follow-up activities and projects currently underway, such as a Fetal Alcohol Syndrome project, alcohol treatment comparisons, and Native Elder exchanges. The Soviet economy has had a marked effect in the type of treatments available in hospitals and clinics for all the types of physical and mental health problems. Because of the openness and cooperation of the Soviet medical personnel, much data and information has been gathered and will be used to determine the areas to be addressed for joint study and application.
A number of "behavioral health" exchange activities between Alaska and the Russian Far East took place during 1992-93. These included visits by a psychiatrist running one of the first "private polyclinics" in Magadan and by a narcologist to discuss the latest developments in alcohol services; a Behavioral Health Conference where Native health workers from Russia and Alaska shared experiences; beginning a multi-year epidemiology project; and a project to assess needs for a fetal alcohol syndrome (FAS) prevention program in Magadan.
We report the first documented case of the use of peripheral blood stem cell autografting in the treatment of a Jehovah's Witness with acute myeloblastic leukemia. This case illustrates the complex ethical and clinical issues that arise in the treatment of such patients.
