[Hepatic phlebography before and after portacaval anastomosis].

Wedged hepatic venography has been utilized in the hemodynamic staging of portal hypertension in 33 patients. The immediate postoperative evaluation in 18 cases, has outlined a portal vein filling in 14 end-to-side portacaval shunts, in 5 with presence of hepatofugal collateral veins between the ligated hepatic and shunted splanchnic sides of the portal system, and in 2 side-to-side. The sinusoidal filling pattern has been relatively unchanged after end-to-side portacaval shunts. The venographic appearance of "spontaneous" reversal of portal blood flow in hepatic cirrhosis must be confirmed by selective hepatic arteriography before affirming its presence.

Control of gastroesophageal bleeding varices by percutaneous transhepatic portography.

We used the transhepatic approach to the portal system to obliterate bleeding gastroesophageal varices as an alternative to portosystemic shunt performed as an emergency. In eight of 13 patients, we obtained complete arrest of bleeding, while rebleeding was observed in five patients. The shunt was performed upon six patients, and only one patient died. Transhepatic obliteration of varices seems to be the more useful method for the control of untractable hemorrhage and to prepare the patient for an elective operation.

Differences in serum bile acid composition between unoperated cirrhotic patients and patients with portacaval or mesocaval shunt.

Serum bile acids were determined in 13 cirrhotic patients with portacaval shunt and in 15 with mesocaval shunt. 20 nonshunted cirrhotics were used as controls. Serum bile acid levels were higher in portacaval (26.7 +/- 14.3 micrograms/ml) than in mesocaval shunt patients (16.9 +/- 8.1 micrograms/ml) and controls (14.1 +/- 11.5 micrograms/ml). The cholic/chenodeoxycholic ratio was lower after portacaval shunt than after mesocaval shunt (0.36 +/- 0.15 and 0.63 +/- 0.26, respectively). It is hypothesized that these findings may be consistent with a larger hepatic blood flow present in mesocaval shunted patients and with chenodeoxycholic absorption in the upper small intestine.

Myoclonus epilepsy with cerebellar Lafora bodies. Report of a case.

A case is reported of an 18 year old man with progressive myoclonus epilepsy. Histopathological examination revealed the presence of numerous Lafora bodies in the cerebellar granular layer, without other significant changes in the central nervous system or in other organs.

[Intramuscular injection of lidocaine in prevention of complications and mortality in acute myocardial infarction: double blind study on 246 cases (author's transl)].

246 cases of acute myocardial infarction in subjects aged 70 or less, with clinical symptoms beginning no more than 8 hours before, altered enzymes, abnormal Q wave, ST segment and T wave, were studied. 99 of them received, on hospital admission, an intramuscular injection of 250 mg of lidocaine, 147 an injection of saline. In the following 3 weeks, significant differences were observed in the appearance of severe arrhythmias (no cases in the treated group and 18 cases in the placebo group, p less than 0,001) and of heart failure (39 cases in the treated group and 79 in the placebo group, p less than 0,05) and in mortality (3 cases in the treated group and 18 in the placebo group, p less than 0,05).

A multicenter double blind clinical trial on 3.4.5-trimethoxybenzoiyl-epsilon-aminocaproic acid (C-3) in acute myocardial infarction.

186 out of 391 patients with acute myocardial infarction were treated with C-3 and 205 with placebo in a multicenter, double-blind clinical trial. Ensuing complications were treated in the same way in both groups. C-3 was injected i.v. slowly at the dose of 2 g statim plus 6 g by continuous drip infusion over 24 hrs for 5 days. During treatment, clinical progress was influenced only in regard to cardiac failure since in the C-3 group the improvement was more significant than in the placebo one (P less than 0.0025). Mortality rates were 8.1% and 11.2% for the C-3 and placebo groups, respectively. The difference in mortality was significant (P less than 0.05) for patients treated with C-3 for more than 24 hours. Mortality in male patients treated with C-3 for more than 12 hours was significantly lower (P less than 0.025). In patients less than 60 years old mortality rate was significantly lower (P less than 0.05) and was more so in patients receiving C-3 for more than 12 hours (P less than 0.025). Mortality due to complications was lower in the C-3 group, with arrhythmias (9.8% vs 14.2%), cardiogenic shock (69.2% vs 75%), and cardiac failure (9% vs 19.4%). Results agree with the hypothesis that C-3 may be effective in acute myocardial infarction by improving the action of traditional antiarrhythmic drugs, and augmenting myocardial contraction energy.
