Proper treatment of compartment syndrome is fasciotomy. Secondary skin defects often complicate the course and require skin-grafting procedures. The present investigation is a retrospective study based on 17 fasciotomies. Their courses are reported, and the possibility of primary skin excision with the purpose of delayed grafting is demonstrated. This procedure would probably reduce the number of hospital days and the number of surgical procedures.
This investigation is a review of the injuries due to fireworks seen in five casualty departments in Denmark over a period of four years. These casualty departments have a total catchment population of 675,000 (13% of the population of Denmark). The review is based on data from the Danish section of EHLASS-project. (Europea Home and Leisure Accident surveillance system). During the period of observation, 358 injuries due to fireworks were observed. 60% of the injuries occurred in persons under the age of 18 years. In 51%, the mechanism of the injury consisted of direct heat while 33% of the patients were hit by flying fireworks and 10% had small foreign bodies in the eye. The regions most frequently injured were the hands/fingers (40%) or the eyes (24%). 6% of the patients required admission to hospital. The estimated number of injuries due to fireworks in Denmark is 690 annually. The majority of injuries due to fireworks were due to incorrect use of fireworks of absence of protective measures.
During the ten year period 1981 to 1990, a total of 436 children aged 0-5 years were admitted for scalds at the Burns Unit of Hvidovre Hospital. We did not find any reduction in the number of admissions during this period nor was any change in the patterns of scalds found. Water, tea and coffee were the main causal agents of scalds and the dominant group of scalded children was between the ages one to two years. In 37% of cases, the scalds required grafting and the average stay in hospital was 16 days. The background for this unchanged pattern could be that the prophylactive campaigns have been insufficient.
A 47 year old man with a history of alcoholism presented with symmetrical lipomas on the shoulders, neck, and chest. He also had lipomatous areas on the trunk and thighs. He was treated with a combination of surgery and liposuction, and made an uneventful recovery. Histological examination confirmed the diagnosis of multiple symmetric lipomatosis, a rare disorder only about 200 cases of which have been reported. The aetiology is unknown, but is often seen in patients with a history of alcoholism.
In the surgery of burns a good healing of donor areas is essential. A controlled trial of 17 cases is presented. Mirror-image donor areas in the same patient were treated on one leg with a traditional dressing of paraffin gauze, Jelonet, on the other leg with Kaltostat, a calcium sodium alginate wound dressing. Healing time, haemostasis, ease and comfort with removal, dressings, infection and quality of the regenerated skin, were estimated. The Kaltostat dressing seemed to be superior due to the quality of the re-epithelialized donor site, the haemostatic quality and the ease and comfort with removal. There was a marginal profit in healing time.
Hypertrophic burn scars of the neck region are disfiguring and cause functional disturbances such as itchiness, tenderness, blistering or even contractures. Since 1970 the Burns Unit in Copenhagen has treated hypertrophic scars of the neck with a special soft neck collar as a routine. This pressure treatment is controlled in the Jobst day-clinic by a team consisting of a surgeon, physiotherapist, occupational therapist and a nurse. The clinical results of 17 cases are reported and commented on.
The object of this work is briefly to draw attention to a new type of accident as the cause of scalding in children. The increasing use of electric kettles has resulted in an increased number of scalding accidents caused by these kettles. Within a period of six months, eight children under the age of five years were treated in the Department for Burns, Hvidovre Hospital on account of overturned electric kettles, a form of accident which had not previously been observed in this department. All of the children had pulled the cable. Seven out of eight children were admitted to hospital and transplantations proved necessary in three children.
Plasmid profiles of strains of Lactobacillus curvatus and L. sake isolated from meat or sauerkraut were analysed to investigate plasmid homology and distribution in relation to the ecology of these organisms in fermenting foods. A hybridisation probe was constructed by cloning of pLc2, a cryptic, 2.6-kbp plasmid from L. curvatus LTH683, into the Escherichia coli plasmid pRV50. In Southern hybridisations with the digoxygenine labeled pLc2 probe, pLc2-related small plasmids were frequently detected in meat-borne strains of L. casei subsp. pseudoplantarum, L. curvatus, L. sake, L. alimentarius, L. farciminis and L. halotolerans and in L. curvatus and L. sake isolated from sauerkraut. Among 27 Lactobacillus type strains originally isolated from habitats other than meat this type of homology was detected only with plasmids of L. buchneri and L. mali. Restriction-enzyme mapping of six small cryptic plasmids from L. curvatus and L. sake revealed strong structural homology but no similarity to previously characterized plasmids of lactobacilli. The presence of a variable region in addition to a conserved one and the occurrence of deletions during cloning of pLc2 suggest that vectors derived from these plasmids are likely to be structurally unstable.
Employment of electric surgical instruments involves a potential risk of burning at the neutral electrode. These are frequently third degree burns. Skin grafting may shorten the course of the illness. The importance of good contact over the entire neutral electrode plate is emphasized.
The paper reports results from balance trials with a total of 42 volunteers testing glycated casein samples containing the Maillard-(Amadori-) product fructoselysine (= FL, analysed as furosine). On a almost FL free diet only traces of FL were excreted. If test meals with 0.8-5.0 g FL were given, only 2.0-1.2% were found in the urine. In the feces of 3 persons eating 0.96 g FL in a single meal 2.6-5.6% were excreted. It is concluded that digestion is the main limiting factor for the uptake of FL. Possibly also the transit time of the ingesta through the gastro intestinal tract is important. Since there is no indication from animal studies for a utilization of FL it can be assumed that the microorganism in the hind gut decompose the main part of the not recovered more than 90% of FL. Obviously the bacterial flora is more active and able to attack such components as assumed until now.
The increasing popularity of skateboards and rollerskates has resulted in an increased number of contacts with the casualty department in Denmark after accidents. As part of the Danish share in the EHLASS project (European Home and Leisure Surveillance System), 120,000 consecutive contacts with the casualty departments were reviewed. Out of these 516 were due to accidents with skateboards and rollerskates (181/335). A total of 194 of these injuries (38%) were fractures and 80% of these were in the upper limbs. Twenty fractures required reposition under general anaesthesia and two required osteosynthesis. Nine patients were admitted for observation for concussion. One patient had sustained rupture of the spleen and splenectomy was necessary. A total of 44 patients were admitted. None of the 516 patients had employed protective equipment on the injured region. Considerable reduction in the number of injuries could probably be produced by employment of suitable protective equipment.
In major knee trauma fractures and/or dislocations can be associated with arterial lesions. Two case reports demonstrate the difficulties in the diagnosis of intimal lesions in the popliteal artery. The presence of a pedal pulse present at the time of admission can be a false sign of security. The importance of prompt diagnosis and treatment is emphasized. The use of arteriography is discussed.
Fifty-six St. Georg prostheses (23 unicompartmental and 33 bicompartmental) were introduced and 34 of these patients were followed-up with a mean period of observation of 4.6 years (2.1-8.9). The results were assessed according to the New York Hospital for Special Surgery Knee Rating Scale. Out of the unicompartmental replacements, 15 out of 19 obtained excellent or good results in contrast to 14 out of the 28 bicompartmental replacements. The St. Georg prostheses may be recommended for treatment of unicompartmental arthrosis. This review suggest that bicompartmental St. Georg prostheses provide unsatisfactory results compared with the results obtained with total condyle prostheses.
Dislocation of the shoulder caused by electric shock is extremely rare and is frequently diagnosed late in the course of the condition. Late diagnosis and treatment make reduction difficult and increase the risks of permanent injuries. A case of unilateral anterior dislocation of the shoulder after a shock of 380 volts is presented here. The dislocation was diagnosed immediately and both reduction and recovery were uneventful.
We report a case with aseptic necrosis of the capitate in a young patient with Gaucher's disease and no fracture of the carpus. The value of technetium bone scintigraphy and computer tomographic scanning are discussed.
