Adenosine A1 receptor activation preferentially protects cultured cerebellar neurons versus astrocytes against hypoxia-induced death.

Administration of adenosine A1 receptor agonists in vivo is neuroprotective in various stroke models. Experiments using either mixed cultures of neurons and astrocytes or brain slices, in which several cell types are present, have demonstrated that activation of A1 receptors also id protective against hypoxia and/or hypoglycemia in vitro. In this study, we have examined the effect of the A1 agonist cyclopentyladenosine (CPA) on cellular damage, measured by efflux of lactate dehydrogenase (LDH), in highly enriched primary cultures of either neurons of astrocytes exposed to different metabolic insults. CPA reduced neuronal LDH release induced by a combination of hypoxia and substrate deprivation ("simulated ischemia"; IC50 = 28 nM) of by hypoxia alone (IC50 = 170 nM). In contrast, CPA had no effect on neuronal damage induced by substrate deprivation alone, not did it affect ischemic death to astrocytes. The neuroprotective effect of CPA during simulated ischemia and hypoxia were reversed by the A1 antagonist 1,3-dipropyl-8-cyclopentylxanthine (DPCPX). These data demonstrate that activation of an adenosine A1 receptor on neurons, but not astrocytes, is protective against cellular damage of death induced specifically by hypoxia as opposed to other metabolic insults such as hypoglycemia.

Postnatal pup brain dopamine depletion inhibits maternal behavior.

The interactions between dams and their pups and among siblings were investigated in litters with (a) all pups depleted of striatal dopamine by 6-hydroxydopamine (6-OHDA on PND3), (b) all pups treated with vehicle, or (c) half of the pups depleted of dopamine and the other half treated with vehicle. On PND10, two sets of four pups from each litter were videotaped in a novel environment with the dam:pup and maternal behaviors were later scored by blind observers. We observed a 70% decrease in striatal dopamine in 6-OHDA-treated pups but found no effect of treatment on pup weight gain. Dams with some or all DA-depleted pups (a) were slower to retrieve a pup and establish a nest, (b) retrieved pups less frequently, and (c) spent less time huddling with pups than dams with only vehicle-treated pups. When compared with DA-depleted pups in homogeneous litters, DA-depleted pups in mixed litters were less hyperactive and spent more time huddling with other pups than in isolation. These results suggest that DA-depleted pups receive compromised maternal care but can benefit from interactions with normal siblings.

Successful control of Clostridium difficile infection in an elderly care unit through use of a restrictive antibiotic policy.

Toxin-producing Clostridium difficile is the commonest bacterial cause of nosocomial diarrhoea and is a well recognized cause of hospital outbreaks in elderly care units. High C. difficile disease rates have been associated with the use of broad-spectrum antibiotics, especially cephalosporins. An outbreak of C. difficile infection in the elderly care unit at Gloucestershire Royal NHS Trust continued despite increased ward cleaning and strict implementation of infection control measures. A restrictive antibiotic policy that would maintain colonization resistance in the gastrointestinal tract was introduced throughout this unit. Patients admitted with suspected infection were prescribed intravenous (i.v.) benzylpenicillin 1.2-1.8 g every 6 h to cover streptococcal infections and i.v. trimethoprim 200 mg twice daily to cover urinary tract pathogens and Haemophilus influenzae. If the patient had septic shock a single iv dose of gentamicin was given (120-180 mg) to cover more resistant gram-negative bacilli. The following were monitored before and after the policy change. The number of cases of C. difficile toxin-positive diarrhoea; cefuroxime and total antibiotic use on the elderly care wards; patient mortality rates; and length of hospital stay: two hundred and fifty-two and 234 patients respectively with a discharge diagnosis of infection were admitted before and after the antibiotic policy change. Mortality rates and length of hospital stay were unchanged. Cefuroxime prescribing and total antibiotic prescribing costs fell by 5150 pounds sterling and 8622 pounds sterling respectively in the 7 month period after the change. Thirty-seven cases of C. difficile diarrhoea occurred in the period before and 16 in the period after the policy change. The incidence of C. difficile diarrhoea and of cefuroxime use has remained low since then. The use of narrow-spectrum antibiotics for hospital treatment of community-acquired infections in the elderly should be encouraged. Outbreaks of C. difficile diarrhoea should be managed with the combined approach of infection control and strict antibiotic policies.

The isolation and characterization of genomic and cDNA clones coding for a cdc2-related kinase (ThCRK2) from the bovine protozoan parasite Theileria.

The tick-transmitted protozoan parasites Theileria annulata and Theileria parva are important intracellular pathogens of domestic cattle in tropical and subtropical regions. Proliferative phases take place within both lymphocytes and erythrocytes. The lymphocyte is stimulated to enter the cell cycle by the parasite and the multinucleate parasite can establish a state in which karyokinesis and cytokinesis occur in phase with the host cell. The link between parasite nuclear division and cytokinesis is altered during the formation of merozoites (a non-dividing, invasive, extracellular stage). These features imply a high degree of control over parasite nuclear division and cytokinesis. Two different approaches have been used to identify clones from both species which are extremely highly conserved homologues. These encode a cdc2-related kinase which is > 60% identical to eukaryotic cyclin-dependent kinases of the p34cdc2/p32CDK2 subfamily. There is typical conservation of kinase domains, implying an in vivo protein kinase activity for the polypeptide. The PSTAIRE region, implicated in cyclin binding, is well conserved suggesting that ThCRK2 will bind cyclin molecules closely related to the eukaryotic A/B-type cyclins. However, there is divergence in certain key motifs potentially associated with binding of molecules that regulate the activity of the kinase. Expression patterns of RNA and protein indicate that ThCRK2 is likely to function in all dividing stages of the parasite and, taken together, the results point to a central role in the regulation of nuclear division.

A hybrid sigma subunit directs RNA polymerase to a hybrid promoter in Escherichia coli.

Most of the sigma (transcriptional initiation specificity) subunits of RNA polymerase, from a wide range of eubacteria, show strong elements of amino acid sequence similarity. There is evidence that two of the "conserved" regions, 2.4 and 4.2, are involved in recognition of the consensus DNA sequences centred near -10 and -35, respectively, which define promoter sites for the initiation of transcription. Since all the alternative sigma subunits of the above type function by binding to a common core polymerase enzyme in a given bacterium, it can be predicted that a hybrid sigma might be functional, and if so should permit RNA polymerase to initiate only at a correspondingly hybrid promoter. To test these predictions, a hybrid gene encoding the amino-proximal 529 amino acids of the major Escherichia coli sigma protein, sigma 70 (including region 2.4) followed by the last 82 amino acids of the heat-shock sigma protein, sigma 32 (including region 4.2) was constructed and fused to Plac on a plasmid. Major-consensus, heat-shock and hybrid promoters were fused to a chloramphenicol acetyl transferase (CAT) reporter gene on a compatible plasmid. CAT assays showed that, as predicted, a promoter with a "heat-shock" -35 consensus and a "major" -10 consensus sequence (PHM) required Plac-dependent production of the hybrid sigma (sigma 70-32) for activity in vivo. PHM then became a strong promoter. The hybrid sigma gene has potential advantages over its parents for structure-function studies.

A cluster of cases of streptococcal necrotizing fasciitis in Gloucestershire.

We describe the first cluster of cases of necrotizing fasciitis (NF) in this century in the United Kingdom (UK). Between 1 January and 30 June 1994 there were six cases (five confirmed, one probable) of Streptococcus pyogenes NF in west Gloucestershire, population 320,000. Two cases died. The first two patients probably acquired their infections during the course of elective surgery performed in the same operating theatre, possibly from a nasopharyngeal carrier amongst the theatre staff. The remaining infections were community-acquired. Of 5 S. pyogenes isolates there were 2 M1 strains, 1 M3, 1 M5 and 1 M non-typeable strain. S. pyogenes NF had not been recorded in west Gloucestershire in the preceding 10 years and the incidence of S. pyogenes bacteraemia in England and Wales had not risen in the past 5 years. The two presumably theatre-acquired infections raised several issues. The need for detailed bacteriological investigation of all cases of post-surgical NF was confirmed. Clusters of S. pyogenes infection following surgery should be managed by closure of the operating theatre until all staff have been screened for carriage. Closure of an operating theatre and screening of staff following a sporadic case is probably not justified because of the infrequency of surgical cross-infection with S. pyogenes. Regular, routine screening of theatre staff is neither practical nor necessary.

The RSRF/MEF2 protein SL1 regulates cardiac muscle-specific transcription of a myosin light-chain gene in Xenopus embryos.

We have examined the role of two RSRF/MEF2 proteins in the onset of skeletal and cardiac muscle differentiation in early Xenopus embryos. In normal development, zygotic expression of SL1 (MEF2D) precedes that of SL2 (MEF2A) by several hours, but neither gene is expressed prior to the accumulation of MyoD and Myf5 transcripts in the somitic mesoderm. Ectopic expression of the myogenic factors in explants of presumptive ectoderm induces expression of both SL1 and SL2, whereas in reciprocal experiments, neither RSRF protein activates the endogenous myoD or Myf5 genes. We conclude that SL1 and SL2 lie downstream of these myogenic factors in the skeletal myogenic pathway. SL1 is distinguished from SL2 in being expressed in the presumptive heart region of the early tailbud embryo, prior to detection of any markers for cardiac muscle differentiation. Furthermore, ectopic SL1 induces the expression of an endogenous cardiac muscle-specific myosin light-chain (XMLC2) gene in cultured blastula animal pole explants, whereas SL2 has no comparable effect. These results demonstrate that in addition to a possible role in skeletal myogenesis, SL1 also acts in vivo as a regulator of cardiac muscle-specific transcription.

Cloning of a cDNA encoding ovine interleukin-3.

A cDNA encoding the ovine hematopoietic growth factor interleukin-3 (IL-3), has been cloned. A 560-bp cDNA, containing the entire protein coding region, was amplified from activated mesenteric lymph node cells. The cDNA was found to share 58% nucleotide identity with hIL-3 and 48% identity with mIL-3. The predicted ovine amino-acid sequence shares 36 and 30% identity, respectively, with the corresponding human and murine proteins.

The utility of nasal bone radiographs in nasal trauma.

A prospective study was performed to assess the value of nasal bone radiographs taken in the accident and emergency department. The study population consisted of 100 consecutive patients who had nasal bone radiographs in our accident and emergency (A&E) department following trauma. We looked at the casualty officers', radiologists' and ENT surgeons' assessment of the cases. Thirty months later we reviewed the patients' notes to identify the number who sought medico-legal reports on their injury in that interval. We found sporting injuries to be the commonest mechanism of injury, followed closely by accidental falls. The remaining third was made up predominantly of cases of personal assault and road traffic accidents. Only two patients had a naso-pharyngeal history recorded on their visit to A&E. Thirty-five patients were referred to ENT out-patients, only 24 kept their appointment. Thirty-one of the 35 ENT referrals were felt to have a fracture demonstrated on their radiographs. However, 19 of those discharged were also thought to have an X-ray-proven fracture. Thirty months later only two patients had requested a medico-legal report. We demonstrate that the decisions regarding treatment of nasal trauma are based on clinical findings and that nasal bone radiography has no place in the decision making process and should therefore be abandoned.

Use of aprotinin in knee replacement surgery.

We have studied the effect of aprotinin on blood loss and subsequent blood transfusion in 17 patients undergoing knee replacement surgery. Patients receiving aprotinin (total dose 2,000,000 kallikrein inhibiting units) received fewer units of blood than control patients (P < 0.05), although there was no significant difference in blood loss between the two groups. The study was stopped when one patient in the aprotinin group needed an above-knee amputation because of ischaemia secondary to arteriovenous thrombosis after knee replacement surgery. Although the patient had peripheral vascular disease which could have accounted for the thrombosis, the role of aprotinin under tourniquet conditions is unclear.

Breathing systems: effect of fresh gas flow rate on enflurane consumption.

The vaporization rates of enflurane were measured in 412 anaesthetics using appropriate fresh gas flow rates in Bain (12 litre min-1), Magill (6 litre min-1) and circle systems (3 litre min-1, 1 litre min-1 and "closed"). In all patients reducing the fresh gas flow rate resulted in lower enflurane consumption. The percent savings were 18-86% depending on the initial fresh gas flow rate and the size of the change in fresh gas flow. The reduction in enflurane use was more marked in inpatients (long cases) than in day-case patients (short cases).

Multiple copies of genes coding for electron transport proteins in the bacterium Nitrosomonas europaea.

The genome of Nitrosomonas europaea contains at least three copies each of the genes coding for hydroxylamine oxidoreductase (HAO) and cytochrome c554. A copy of an HAO gene is always located within 2.7 kb of a copy of a cytochrome c554 gene. Cytochrome P-460, a protein that shares very unusual spectral features with HAO, was found to be encoded by a gene separate from the HAO genes.

Induction of cardiac muscle differentiation in isolated animal pole explants of Xenopus laevis embryos.

We have isolated a cDNA fragment encoding a portion of the myosin heavy chain alpha-isoform (XMHC alpha) in the amphibian, Xenopus laevis. The XMHC alpha transcript is highly enriched in adult heart RNA and is expressed exclusively in embryonic heart tissue. It therefore provides a tissue-specific marker for cardiac muscle differentiation during early embryogenesis. Using an RNAase protection assay, we can detect the onset of cardiac muscle differentiation in an anterior, ventral region of tailbud embryos, many hours before the appearance of a beating heart. Whole-mount in situ RNA hybridisation indicates that expression of the XMHC alpha gene is restricted to the developing heart primordium. XMHC alpha gene expression can also be induced in isolated animal pole explants of blastulae by treatment with the growth factor, activin A. Induction is dose-dependent, requiring high doses of the growth factor compared with that required for myotomal (skeletal) muscle differentiation. In contrast, no XMHC alpha transcripts are detected in explants incubated with basic FGF, despite the induction of myotomal muscle differentiation. Activin-induced explants show a similar temporal pattern of XMHC alpha gene expression to that found in normal embryogenesis. Furthermore, cells expressing this gene appear clustered in one or two foci within fused explant aggregates, which often show regular, spontaneous contractions after several days in culture. These results show that terminal differentiation of cardiac muscle can occur in growth factor-induced explants and may be distinguished from skeletal muscle differentiation by the dose and nature of the inducing factor.

The cloning and expression of the gene for ovine interleukin-3 (multi-CSF) and a comparison of the in vitro hematopoietic activity of ovine IL-3 with ovine GM-CSF and human M-CSF.

The ovine interleukin-3 (IL-3) gene has been cloned. It encodes a protein 146 amino acids (aa) in length and is situated approximately 10 kb from the gene encoding granulocyte-macrophage colony-stimulating factor (GM-CSF). A comparison of the ovine gene sequence with that of the human IL-3 gene reveals that the protein coding regions share between 49 and 59% identity, while the introns and other noncoding regions share between 63 and 78% identity. The gene promoters also share approximately 72% identity. Recombinant ovine IL-3 (rovIL-3) was expressed in Chinese hamster ovary (CHO) cells, and its biologic activity was compared to that of rovGM-CSF and recombinant human macrophage colony-stimulating factor (rhM-CSF) on ovine bone marrow cells. rovIL-3 predominantly stimulated the growth and development of mast cells and macrophages in liquid cultures and colonies of mixed cell phenotype, megakaryocytes, erythroid burst-forming units (BFU-E), and basophilic granular cells in soft agar cultures of bone marrow cells. In common with rovGM-CSF, IL-3 also stimulated eosinophil and macrophage colonies, which were increased in size and number of cells in cultures containing both cytokines. Maximum macrophage colony numbers were achieved with the combination of rovIL-3, rovGM-CSF, and rhM-CSF. rovGM-CSF stimulated neutrophil colony formation, whereas rovIL-3 did not.
