[Effects of high carbohydrate and high fat diet on plasma apolipoproteins AI, AII, B100, CI, CII, CIII and E in normal and hypertriglyceridemic humans].

Effects of high carbohydrate (CHO) and high fat diet on plasma apolipoproteins (apo) AI, AII, B100, CI, CII, CIII and E were assessed in 10 healthy subjects and 10 patients with endogenous hypertriglyceridemia. High CHO diet consisted of 80% CHO, 5% fat and 15% protein of total calories, and high fat diet consisted of 40% CHO, 45% fat and 15% protein. After 5 days normal diet (60% of total calories from CHO, 25% from from fat and 15% from protein), the subjects studied received sequentially high CHO diet and normal diet for 5 days and high fat diet and normal diet for 5 days. Fasting blood samples were drawn on days 1, 3, and 5 of high CHO and high fat diet period, and 5 days after taking normal diet. Results showed that fasting plasma apo AI slightly decreased (P less than 0.01) on day 3 of the high CHO diet period and slightly increased (P less than 0.01) on day 3 of the high fat diet period, plasma apo AII increased after intake of high CHO diet, but no changes were observed after the subjects were placed on high fat diet. Fasting plasma apo B100 decreased (P less than 0.001) in hypertriglyceridemics on the 3rd day of the high CHO diet, but no differences were noted in normal subjects on the same diet. On the 3rd day of the high fat diet period, apo B100 increased (P less than 0.01) in normal subjects, but no changes were found in hypertriglyceridemics. Apo CII and CIII levels declined during the high CHO diet period but increased in normal subjects after intake of high fat died.(ABSTRACT TRUNCATED AT 250 WORDS)

[Transforming growth factor-alpha in human lung cancer tissue].

12 cases of human lung cancer tissue were analyzed for the presence of transforming growth factor-alpha with RIA. Histologically 9 were squamous cell carcinoma and 3 were adenocarcinoma. Except one squamous carcinoma, TGF-alpha could be detected in 11 cases. TGF-alpha level was from 0.26 ng to 1.26 ng per gram tissue. There was no significant difference in TGF-alpha level between squamous cell carcinoma and adenocarcinoma. The relationship between TGF-alpha and human lung cancer remains for further study.

[Observation on the annual and seasonal changes of anatomic characteristics of the main root of Panax ginseng C.A. Meyer].

Annual dynamic of the main root of Panax ginseng shows that the cross-section of various tissues increases gradually with age, but the ratio of phloemxylem in cross-section decreases. Seasonal dynamic shows that cells of phellogen and cambium divide actively in June-July and gets retarded in August. Ecological conditions can influence annual and seasonal dynamics of the structure of the main root.

[A preliminary study on the pharmacology of the compound prescription huangqin tang and its component drugs].

Results obtained from the pharmacological experiments demonstrated that the compound prescription Huangqin Tang and its component drugs, roots of Paeonia lactiflora, Scutellaria baicalensis and Glycyrrhiza uralensis, and the fruit of Ziziphus jujuba showed marked anti-inflammatory effect. The compound prescription and its component drugs, except the peony root, possessed significant antispastic effect. Antipyretic effect of Radix Scutellariae, Radix Glycyrrhizae and Huangqin Tang was suggested. Huangqin Tang, Radix Paeoniae alba, and Radix Glycyrrhizae showed marked analgesic effect. In addition, the experiments confirmed that both Huangqin Tang and single Radix Scutellariae have sedative effect.

Microcirculation of the dental pulp and its autonomic control.

Pulpal vessels are innervated with sympathetic adrenergic vasoconstrictor fibers. Electrical stimulation of the cervical sympathetic nerve causes pulpal arteriolar constriction and a reduction of arteriolar and venular flow rate (in mm3/s) in rat incisor pulps and a decrease in pulpal blood flow (in ml/min/100 g) in canine pulps of cats and dogs. The alpha-antagonist phenoxybenzamine and the alpha-1 antagonist prazosin attenuated the decrease in pulpal blood flow (PBF) caused by sympathetic stimulation. Reflex excitation of the sympathetic nerve system by hemorrhage caused pulpal vasoconstriction and a reduction of PBF. Induction of hemorrhagic hypotension in dogs subjected to cervical sympathectomy and adrenalectomy caused less pronounced pulpal vasoconstriction and flow reduction than in normal dogs. Pulpal vessels are also equipped with alpha-1, alpha-2 and beta-adrenergic receptors. Activation of alpha-1 and alpha-2 receptors by intraarterial injection of phenylephrine and clonidine caused a reduction in PBF in dogs and decreases in arteriolar and venular diameters and volumetric flow in rats. These responses were blocked by the alpha-1 and alpha-2 antagonists. Activation of beta-2 receptors by i.a. injection of isoproterenol caused a paradoxical reduction of PBF in dogs. In rats isoproterenol caused a transient increase in the flow rate followed by a reduction, and arteriolar dilation was accompanied by venular constriction. These flow responses to isoproterenol were blocked by the beta-2 antagonist propranolol. Results of microcirculatory studies in dogs and rats indicate that pulpal hemodynamics are regulated significantly by the sympathetic adrenergic system.

Superior mesenteric vein severance from blunt abdominal trauma: report of a case.

Injury of the superior mesenteric vein from blunt abdominal trauma is an uncommon but frequently fatal injury. A 63-year-old man was injured in a motorcycle accident. He was admitted to our hospital in profound shock. Immediate celiotomy revealed superior mesenteric vein injury which was repaired by end-to-end anastomosis. Rapid fluid resuscitation and expeditious exploration are the key in saving patients with this condition. Indications for exploration of retroperitoneal hematomas, techniques of exposure, modes of reconstruction of the injured vein and the importance of a second-look operation are discussed.

Identification of the simian rotavirus SA11 genome segment 3 product.

Previous studies on rotavirus gene-coding assignments failed to clearly identify the simian rotavirus SA11 genome segment 3 protein product. This question was reexamined by using new conditions of electrophoresis with improved resolution of proteins in the high-molecular-weight range. Our results showed that the SA11 genome segment 3 codes for a protein with an apparent mol wt of 88,000. This protein normally comigrates with the protein product of genome segment 4. The gene 3 protein was located in viral core particles by comparing the electrophoretic patterns of [35S]methionine-labeled viral polypeptides from infected cells and from purified double-shelled, single-shelled, and core particles. To confirm the identity of the gene 3 product, we also studied two reassortant viruses in which genome segment 3 was reassorted from each of two parental virus strains (SA11 and NCDV). The gene 3 and gene 4 products of these viruses were identified by (i) their separation by two different polyacrylamide gel systems, (ii) their location in distinct particle types, (iii) their differential sensitivity to trypsin digestion, and (iv) their distinctive protease peptide maps. We propose that the genome segment 3 product be called VP3 and that the gene 4 product be named VP4 from now on.

Design and prospective evaluation of an algorithm for penetrating truncal injuries.

The major aim was to develop a branch-chain decision tree for penetrating truncal injury and to subject this to a prospective trial of its feasibility to track management decisions. In contrast to the conventional trauma study which focuses on highly selected, well-defined surgical problems, this algorithmic approach was designed to look at the whole gamut of problems of unselected patients with penetrating injury as they enter the ED. The branch-chain algorithm primarily focuses on priorities, order of procedures, and the immediate therapeutic options. The algorithm was used to track clinical management of 280 consecutive patients with penetrating truncal injuries; 31 were dead on arrival, 12 were alive on admission but died during their hospitalization. Of 209 patients whose management was in satisfactory compliance, four (2%) died; eight (20%) of 40 patients who had major deviations from the algorithm died (p less than .01).

Development and testing of a decision tree for blunt trauma.

The aim of the present study was to examine the essential problems in a retrospective study of 381 organ injuries in 260 patients, to identify problems, to define criteria, to describe decision rules, and to organize these rules into branch-chain decision trees or clinical algorithms. The basic hypothesis of this study is that criteria organized into a prioritized decision tree can provide objective standards to evaluate the quality of trauma care and to compare alternative approaches. The algorithm was designed to provide prompt therapy for the most life-threatening problems: respiratory and cardiac arrest, shock, head injury, tamponade, lacerations of the great vessels, cardiac contusion, ruptured parenchymal organs, lacerated viscera, and injury to other intraperitoneal organs. Resuscitation from shock, correction of circulatory problems, and monitoring of physiologic variables were prioritized to evaluate the presence of circulatory deficits and the adequacy of specific therapy to correct them. Concomitantly, diagnosis of the underlying problems was approached using peritoneal lavage, abdominal and chest x-rays, iv urograms, cystograms, endoscopy, upper and lower GI barium or hypaque studies, ultrasound, scintograms, and CT scans. In emergency conditions these are limited to a large extent by time factors. The diagnostic accuracy, priorities, and limitations of each of these were evaluated in emergency conditions. The algorithm was used to track management decisions in a prospective series; the mortality of 51 patients with satisfactory compliance was 4% and 44% in nine patients with major deviations from the algorithm.
