Intravascular and interstitial fluid dynamics in rats treated with minoxidil.

Edema is a major complication of vasodilatory therapy. However, the pathophysiologic mechanisms leading to the formation of vasodilator-mediated edema are insufficiently understood. The present study therefore examined the effect of the chronic administration of the potent arteriolar vasodilator, minoxidil (Mx), on extracellular fluid dynamics in rats. Extracellular volume (ECV), plasma volume (PV), interstitial fluid volume (IV), arterial pressure (AP), and interstitial fluid pressure (IP) were measured in rats treated for 10 days with Mx (1.5 mg/kg/day) and in control animals. In addition to a decreased AP, Mx-treated animals had diminished water and sodium excretion. ECV, PV, and IV and plasma renin concentration (PRC) were also increased in the Mx-treated rats. IP, which was subatmospheric in control rats (-2.6 +/- 0.04 mm Hg), was near zero in Mx-treated animals (-0.2 +/- 0.02 mm Hg, p less than 0.05). Saline ECV expansion (20 min, Ringer infusion, 3% body weight) or rat albumin injection (300 mg/2 ml) induced similar changes in the volume of the extracellular fluid compartments in both groups. However, changes in IP were blunted in Mx-treated rats. These results, therefore, show that Mx-treated rats have changes in interstitial fluid dynamics prior to any macroscopic evidence of edema accumulation. These alterations in the extracellular compartment dynamics may be a consequence of the sustained arteriolar vasodilation induced by Mx.

[Carcinoma of the esophagus. Palliative treatment with endoscopic prosthesis].

The efficacy and safety of endoscopically inserted esophageal prosthesis was evaluated in 33 patients with non-operable carcinoma involving the esophagus. In 30 cases, the origin of the tumor was esophageal, and in 3 cases, bronchopulmonary. Esophago-pulmonary fistula was present in 9 patients. Histologic exam of the tumors revealed 11 adenocarcinomas and 22 epidermoid carcinomas. In every case, Atkinson prosthesis was used and previous esophageal dilatation was needed in 21 patients. Normal oral feeding was achieved in 29 patients. Mean hospital stay after insertion of the prosthesis was 5 days. Survival rate was 84.8% at one month, 63.6% at 4-6 months, 42.4% survived more than 6 months, and 21.2% were still alive one year after the procedure. Complications occurred in 3 patients (9%) -esophageal perforation in two cases, and pneumonia in one case. One patient died. In conclusion, in patients with advanced carcinoma involving the esophagus, palliative therapy with endoscopically inserted esophageal prosthesis is effective, relatively safe and cheap.

[Thoracic pain possibly of esophageal origin. Results of a prospective study].

We have studied the prevalence of esophageal pathology, by means of esophagogram, esophagogastroscopy, manometry, 24-hours ambulatory pH measurement and acid perfusion of the esophagus, in 44 patients with thoracic pain suggestive of angina. Esophageal pathology was demonstrated in 30 (68.3%) cases; 25 patients were diagnosed of reflux and 5 of primary motor alteration. All the patients received specific treatment; the response to treatment was good in 22 of them (17 with reflux and 5 with motor alterations). In conclusion, esophageal pathology is a common cause of thoracic pain, and reflux is the most frequent finding.

[Early complications of pneumatic dilatation in the treatment of primary motility disorders of the esophagus].

We analyze the incidence and evolution of the early complications of 96 consecutive patients with primary esophagus motor disorders, treated with pneumatic dilatation under endoscopic control (1.4 sessions per patient). In 4 (0.042/patient, 0.029/dilatation) patients the esophagus was perforated; the diagnosis was made in the first 24 hours; pneumomediastinum was a constant finding in the radiological exploration. In three cases the complication was suspected because of the apparition of sustained thoracic pain after the dilatation maneuver and in one case the presentation symptom was bleeding of cardial mucosa, larger than usual, at the end of the dilatation. The four patients evolved favorably with conservative treatment (avoidance of oral food intake, gastroesophageal aspiration, antibiotic therapy and parenteral nutrition).

Idiopathic myelofibrosis terminating in erythroleukemia.

A 55-year-old woman with a one-year history of idiopathic myelofibrosis progressed to erythroleukemia. This is the first reported occurrence of erythroleukemia progression from idiopathic myelofibrosis. Certain patterns of leukemia transformation from myeloproliferative disorders are favored: Myeloblastic or myelomonocytic. The rare incidence of erythroblastic transformation is discussed.

Slow infusion of vincristine in the treatment of refractory thrombocytopenic purpura.

Eight patients with idiopathic thrombocytopenic purpura (ITP), who were refractory to glucocorticoid therapy, were given slow infusions of vincristine (VCR) over a 4- to 6-hour period at weekly intervals for 4 weeks. Three patients showed a return to normal platelet counts maintained for 3 months or longer. A transient recovery was observed in 1 patient and a partial response was observed in 3 patients. All patients tolerated therapy well, without side effects. In conclusion, therapy with slow infusion of VCR can be effective in refractory ITP.
