[Left cor triatriatum. Clinical and echocardiographic polymorphism].

Four cases of cor triatriatum in children aged from 4 days to 12 months are described. In one case isolated classical form of the disease occurred, and in the other it was associated respectively to ventricular septal defect, anomalous pulmonary venous connexion and persistence of left superior venae cava. One child had successful surgical correction, two died before surgery could be attempted and one waits operation. Relevant clinical and investigational data are described and the role of echocardiography as an excellent diagnostic tool is stressed. We conclude that isolated form of cor triatriatum can simulate primary lung disease and when associated to other cardiac anomalies it has an earlier diagnosis due to early referral. Surgical results and prognosis depend not only on associated anomalies but also on early diagnosis.

[Heart surgery without previous heart catheterization, in pediatric cardiology].

STUDY OBJECTIVE: To determine the influence of echocardiography in the preoperative diagnosis of heart diseases in children. DESIGN: Retrospective study. SETTING: Hospitalized children admitted in Departments of Pediatric Cardiology and Cardiothoracic Surgery. PATIENTS: Children of both sexes, aged from neonate to 14 years old, with heart disease who underwent cardiac surgery. MATERIAL AND METHODS: From January 1989 to July 1990, 220 consecutive cardiac surgeries were performed in children with heart disease. The initial diagnosis was based on data from clinical examination, electrocardiogram, thorax X-Ray and echocardiogram (M-mode, 2D, conventional and colour Doppler). Patients were separated in three groups according to their ages: newborn infants less than 28 days old; infants less than 12 months old; children more than 1 year old and less than 14 years old. Patients were separated according to the investigations used further for preoperative diagnosis: whether they had or not cardiac catheterization performed prior to surgery. RESULTS: From 220 surgical interventions performed, 124 were "open heart" surgeries (9 neonates, 28 infants and 87 children) and the remaining had "closed heart" operations (14 neonates, 37 infants and 45 children). Preoperative diagnosis was mainly dependent on echocardiography, dispensing catheterization in 90 cardiac interventions. The diagnosis was confirmed at surgery or at autopsy. There were 9 deaths, 3 of which occurred in patients not submitted to cardiac catheterization. CONCLUSIONS: Echocardiography is a reliable method for investigation and establishment of the preoperative diagnosis of heart diseases in children. Its use may further reduce the need for diagnostic catheterization in children, particularly in risk groups.

Malnutrition among children of adolescent mothers in a squatter community of Recife, Brazil.

In a study of 357 first time mothers in a squatter community of Recife it was found that 1 in 10 were less than 15 years old and 60 per cent less than 20 at the time of their first delivery. Two groups of mothers were distinguished, viz. adolescent mothers with a mean (SD) age at first delivery of 16.7 (0.78) years and older mothers with a mean (SD) age at first delivery of 25 (0.79). More than a third were living in common-law union. Of the adolescent mothers only 58 per cent were living with the child's father. A further 23 per cent had received financial support from their own parents during the pregnancy. The rest were expelled from home. The adolescent mothers were generally worse off with a per capita income generally half of that of older mothers. About a quarter of all children studied were below the 10th centile of NCHS standards (23 per cent by weight/age; 28 per cent by height/age). The undernourished first born was 2.5 times more likely to have an adolescent mother, four times more likely to be in a household with low income, and two times more likely to have an illiterate mother as compared to the first borns who remain well nourished. The implications of these findings for urban primary health care programmes are further discussed.

[New technic for the treatment of high intraabdominal cryptorchism: testicular autotransplantation with venous anastomosis only].

A new technique in the treatment of high intraabdominal cryptorchidism is proposed: testicular autotransplantation with venous anastomosis only. The technique consists in performing microsurgical anastomosis of the spermatic vein and homolateral inferior epigastric vein only for adequate venous drainage, leaving irrigation of testis to the deferential artery alone. We have utilized this technique in 16 cases and excellent results have been achieved. It has permitted performing surgery in younger patients (2 years). Surgical trauma and the duration of the operation are less compared to the Silber technique. There is a higher success rate and the procedure is easy to perform.

[Changes in the coronary arteries in Kawasaki disease: echocardiographic aspects].

OBJECTIVE: Evaluation of coronary anomalies in children with Kawasaki disease (KD) by two-dimensional echocardiography. SETTING: Outpatient and Laboratory of Echocardiography in the Pediatric Cardiology Department of Santa Marta Hospital. PATIENTS: Forty four children (M/F = 1.7/1) with a mean age of 2 years and 8 months (range from 2 months to 8 years) underwent Outpatient and serial echocardiographic study from July 1984 to December 1988. RESULTS: Coronary artery anomalies were demonstrated in 13 (52%) of 44 children; 11 (25%) had uniform dilatation and 12 (27%) had aneurysms. These anomalies were never observed before the first week of illness. During a mean follow-up of 8.4 months, we have observed normalization of coronary arteries lumen, in 10 (43.4%) of the 23 cases. The anomalies remained in 13 (56.6%). CONCLUSIONS: Coronary artery anomalies are a frequent complication of KD. In about 50% of patients, coronary artery anomalies remained in a mean follow-up of 8 months. Two-dimensional echocardiography is a good method to study coronary anomalies in KD.

[Sex change in children with Morris syndrome].

Eleven boys with Morris syndrome underwent feminizing genitoplasty utilizing a segment of the sigmoid colon for neovaginal reconstruction. Bilateral orchiectomy was performed for cosmetic reasons and to prevent neoplasms. Patient follow-up ranging from 6 months to 13 years have demonstrated good results were obtained. No intra- or postoperative complications were observed. Patient psycho-social-sexual identification and satisfaction were achieved.

A comparison of nizatidine with the three other histamine receptor antagonists for duodenal ulcer therapy.

Nizatidine (Axid) is the newest of four histamine receptor antagonists approved by the Food and Drug Administration for treatment of duodenal ulcer and maintenance therapy. It is as effective as the other histamine blockers in healing active duodenal ulcers and in the prevention of relapse. Nizatidine is comparable in price to cimetidine (Tagamet). Nizatidine does not interfere with the hepatic metabolism of other drugs. No significant central nervous system side effects have been reported. It is safe to use in elderly patients or in patients with renal insufficiency. The dosage, however, should be decreased when renal function is impaired.

Hydrolysis of cellulose by a mixture of Trichoderma reesei cellobiohydrolase and Aspergillus niger endoglucanase.

Two endoglucanase-containing fractions were separated from Aspergillus niger cellulase by gel filtration and fast protein liquid chromatofocusing (FPLC). They possessed no ability to bind to or hydrolyze insoluble microcrystalline cellulose (Avicel) but were active toward soluble carboxymethylcellulose. No synergism was observed between Trichoderma reesei cellobiohydrolase I and either endoglucanase from A. niger. These findings may indicate that the role of the endoglucanase component of cellulase in insoluble microcrystalline cellulose hydrolysis is dependent upon its ability to be adsorbed upon the substrate.

The role of cellulase concentration in determining the degree of synergism in the hydrolysis of microcrystalline cellulose.

Microcrystalline cellulose (10 mg of Avicel/ml) was hydrolysed to glucose by different concentrations of the purified cellulase components endoglucanase (EG) II and cellobiohydrolases (CBH) I and II, alone and in combination with each other, in the presence of excess beta-glucosidase. At a concentration of 360 micrograms/ml (160 micrograms of EG II/ml, 100 micrograms of CBH I/ml and 100 micrograms of CBH II/ml) the degree of synergism among them was negligible. As the concentration of cellulase decreased, the degree of synergism increased, reaching an optimum at 20 micrograms/ml (5 micrograms of EG II/ml, 10 micrograms of CBH I/ml and 5 micrograms of CBH II/ml). There was no apparent relationship between the ratio of the components and the degree of synergism. The latter is probably due, though it could not be proved, to the level of saturation of the substrate with each component. Inhibition of Avicel hydrolysis was observed when the substrate was incubated with saturating and nonsaturating concentrations of a mixture of EG II and CBH I respectively. A similar result was also observed with a combination of EG I and EG II.

Correlation between B-cell mitogenicity and immunosuppressor effects of a protein released by porcine monocytes infected with African swine fever virus.

Virus-free supernatants of cultured swine monocytes infected by African swine fever virus (ASFV) suppressed in vitro proliferation of porcine and human blood mononuclear cells in response to phytohemagglutinin and the in vivo primary immune response of C57BL/6 mice against sheep RBC. The supernatants were fractionated by discontinuous ion-exchange chromatography and subfractionated by double-step preparative isoelectric focusing. The pool of the most purified active subfractions (F5'EP-ASFV) is made up of heat-unstable material, can be stained by silver nitrate, and has an isoelectric point of 3.88, a maximal optical density at 280 nm, and a mass of 36,000 daltons. In vivo kinetic studies in nonimmunized C57BL/6 mice were performed on days 1, 2, 3, 5, and 7 after injection with 50 micrograms of F5'EP-ASFV protein. Compared with the untreated mice, the treated mice had a noticeable increase in nonspecific immunoglobulin-secreting splenic plaque-forming cells (PFC) with the following isotype profile: IgG2a greater than IgG2b greater than IgG3 greater than IgG1 congruent to IgM. Three days after treatment with the active material, specific IgM PFC against sheep RBC increased up to 23-fold. In C57BL/6 mice immunized against sheep RBC 2 days after treatment with F5'EP-ASFV, the increase in nonspecific PFC was followed by a suppression of specific PFC response in the respective isotype. When C57BL/6 mice were treated after priming with sheep RBC, however, there was little or no suppression of specific PFC and the increase in nonspecific PFC was considerably lower than that in the other F5'EP-ASFV-treated mice. In this case, kinetic curves of specific vs nonspecific PFC of each isotype were mirror images. Mice treated with 200 micrograms of F5'EP-ASFV protein died with hemorrhagic diastasis.

Immunoregulation in human schistosomiasis by idiotypic interactions and lymphokine-mediated mechanisms.

Anti-idiotypic (anti-Id) T cells from schistosomiasis patients or former patients proliferate upon exposure to polyclonal or monoclonal anti-soluble egg antigen (SEA) antibodies. Chloroquine does not inhibit, the response, which is induced by F(ab')2 (but not soluble Fab) fragments of these antibodies. Purified T cells from former patients require macrophages or exogenous IL-1 to respond to anti-SEA Ids and can respond to matrix-bound Fab fragments in the presence of IL-1. These anti-Id T cells recognize the Ids directly. Chronic schistosomiasis patients immunoregulate the production of a non-IL-2 lymphokine that stimulates IL-2 receptor expression on resting T cells. This regulation is reversed upon chemotherapeutic cure.

Duplication of the entire small intestine--a case report.

The authors report a case of duplication of the entire small intestine manifested by frequent haemorrhages originating from a peptic ulcer located distally to the duplication. The peptic ulcer was provoked by the ectopic gastric mucosa of the duplication. Surgery consisted "stripping" the mucosa of the duplicated tract according to Wrenn's technique. Other surgical methods for treating this malformation are discussed in detail. The literature describing similar cases is reviewed.

Autotransplantation of the testicle in paediatric utilising microsurgical technique.

This paper reports on the experience of the Paediatric Surgery Clinic of the University of Bologna in testicular autotransplantation utilising microsurgical techniques. Besides evaluating indications for the intervention, the authors present case histories they have accumulated over twenty months (September 1982-May 1984). Subject to discussion are some notes on surgical technique with particular attention to the dissection of the funiculus and to the construction of microsurgical vascular anastomoses. The short-term and medium-term results of 4 autotransplants are presented and commented upon.

Correction of an in vitro immunoregulatory defect in atopic subjects by the immunostimulating drug fanetizole mesylate (CP-48,810).

The effect of Fanetizole mesylate or CP-48,810, a new immunostimulating drug, on suppressor cell function and IgE synthesis in vitro was evaluated in atopic patients with allergic rhinitis and/or asthma and eczema. In the absence of the drug, histamine (10(-3)M) stimulated blood mononuclear cells from 23 atopic patients suppressed concanavalin A-induced lymphocyte proliferation by a mean (+/- S.E.M.) of 9.3% +/- 3.5 (compared to 25.1% +/- 2.7 for histamine stimulated mononuclear cells from non-atopic controls). The addition of the drug (2.5 X 10(-4)M) in vitro significantly increased histamine suppressor cell activity of atopic patients to 26.6% +/- 3.9 (compared to 24.7% +/- 2.8 for control cells in the presence of the drug). In order to determine a possible mechanism through which CP-48,810 might enhance histamine-induced suppressor activity, we examined the effects of the drug on the production of histamine-induced suppressor factor (HSF) by lymphocytes and the production of prostaglandin E2 by blood monocytes in the presence of HSF. Supernatants generated from histamine (10(-4)M) stimulated patient lymphocytes caused a 9.0% +/- 1.8 suppression of concanavalin A-induced lymphocyte proliferation (compared to 25.0% +/- 3.1 caused by supernatants from normal subjects). If the drug (2.5 X 10(-4)M) was added at the beginning of culture, HSF activity in supernatants derived from atopic lymphocytes increased significantly to 20.2% +/- 1.8 (compared to 23.3% +/- 3.9 for drug treated control supernatants). Prostaglandin E2 production by atopic monocytes exposed to HSF was less than that of normal monocytes in the absence of the drug.(ABSTRACT TRUNCATED AT 250 WORDS)

Left ventricular outflow tract obstruction due to tricuspid valve prolapse through a high ventricular septal defect.

A rare case is described of fixed subaortic obstruction due to a prolapsed redundant fibrous septal leaflet of the tricuspid valve, herniated through a membranous ventricular septal defect in a boy aged 11 years. The type of obstruction was not suspected before surgery and there was no shunt through the septal defect. Left ventriculography, was attempted and abandoned due to dangerous arrhythmia. Echocardiogram showed definite fixed subvalvular left ventricular outflow obstruction. Corrective surgery was carried out through aortotomy and right ventriculotomy. Ruptured chordae were resected and the tricuspid valve was replaced into the right ventricle. Redundant fibrous tissue was used to close the septal defect. The postoperative recovery was uneventful after complete AV block in the first 5 days.
