Cardiac protection by long-term treatment with captopril in patients with acute myocardial infarction.

OBJECTIVES: To assess the effects of long-term angiotensin-converting enzyme (ACE) inhibitor treatment with captopril on cardiac function in acute myocardial infarction (AMI). METHODS: One hundred and one patients with AMI who were admitted to hospital within 72 hours of the onset of symptoms with no cardiogenic shock were randomly allocated to captopril (n = 52; group I) and conventional treatment (n = 49; group II). Left ventricular (LV) systolic performance and diastolic transmitral flow velocity profiles were assessed by Doppler echocardiography at admission (1.2 +/- 1.1 days), before discharge (27 +/- 10 days) and during follow-up (363 +/- 31 days). RESULTS: At one year follow-up, in group I LV end-diastolic volume decreased, and ejection fraction increased due to a disproportionate decrease in end-systolic volume. The incidence of cardiac dilatation was reduced. LV early diastolic filling velocity (E) increased and late atrial filling velocity (A) decreased, resulting in an elevation of E/A ratio. However, the mean values of LV systolic and diastolic functional parameters were unchanged in group II. CONCLUSIONS: Long-term treatment with captopril exerts a beneficial effect on cardiac protection for patients with AMI.

[McKay operation in the correction of rigid clubfoot].

For the evaluation of the result from McKay operation in the correction of rigid clubfoot, 71 patients (103 feet) were followed up postoperatively for 2 to 9 years. According to the criteria concerning the functional recovery of the foot, the external appearance of the foot after correction and the roentgenographic findings, the result was evaluated as excellent in 55 patients (78 feet), good in 11 patients (16 feet), fair in 5 patients (9 feet) and with no recurrence. It was considered that the favorable time for operation was when the child being 6 to 18 months old. In 7 patients (12 feet), there were morphological changes of the calcaneal and navicular bones in 12 feet, limitation of ankle movement in 9 patients and resultant flatfoot deformity in 5 patients (7 feet). The factors influencing the functional recovery of the foot and the relationship between the corrective result and the age of patient when the operation being given were discussed in details.

Production of human recombinant bone morphogenetic protein-2A by high density culture of Escherichia coli with stationary dissolved oxygen fed-batch condition.

The optimization of cultivation condition in 500 ml shake flasks was carried out to produce recombinant human bone morphogenetic protein-2A (BMP-2A) in recombinant Escherichia coli YK537/pDH-B2m, followed by a 5L fermenter batch and condition-controlled fed-batch culture to obtain BMP-2A. The comparison of these two methods indicated that cultivation by keeping dissolved oxygen at 30%-40% and limiting glucose concentration could obtain BMP-2A 2.78 g/L broth, the final cell density was OD600 53 (dry cell weight 21.2 g/L), and expressed BMP-2A was 25% of the total amount of protein in E.coli. The critical fermentation conditions included: (1) keeping appropriate dissolved oxygen concentration in the process; (2) limiting glucose concentration; (3) taking heat induction at the middle-log phase and maintaining 42 degrees C for 4 hours; (4) controlling specific growth rate around 0.3 h-1 in the duration of growth.

[Determination of copper, iron and zinc in hair of mentally retarded children by oscillopolarography].

The oscillopolarographic method (proposed by Ding Jianwen)for the simulltaneous determination of copper, iron and zinc in hair was further studied and applied to the investigation of copper, iron, zinc content in hair samples from 46 mentally retarded children and 49 healthy children. For copper, iron, zinc, the detection limits were 0.03, 0.03, 0.01 microgram/ml, dynamic range 0.05-15, 0.05-2.5, 0.2-10 micrograms/ml, the RSD of sample analysis 3.2%, 3.3%, 2.4%, the recoveries 96: 0%-105.2%, 2%, 97.3%-109.0%, 94.7%-107.7%, respectively. The difference between the results for the same sample by this method and the atomic absorption spectroscopic method was no statistical significance. The results showed that the copper content and iron content of hair samples from mentally retarded children were significantly lower than those of healthy children.

[HA-coated titanium implants used as orthodontic anchorage. An experimental investigation of implant stability and peri-implant neck tissue in dogs].

This study was aimed at the feasibility of using HA-coated titanium endosseous implants as orthodontic anchorage. These implants installed in dog's mandibles were loaded with the orthodontic force of 150 g for 3 months. The stability of the implant and the peri-implant neck tissue were investigated with radiograph and index evaluation. The findings demonstrated that no implant was mobile or loose or dislocated. The soft tissue around the cervical part of the implants had a slight inflammation because of bad oral hygiene and the stimulation of residual food attached to the stainless string. However, no resorption of marginal alveolor bone was found under sustained orthodontic force. The above results suggest that the HA-coated titianium implant can be used as anchorage for short-term orthodontic treatment.

[Role of hyperinsulinemia in pathogenesis of polycystic ovary syndrome and treatment by reduction of insulin secretion].

OBJECTIVE: To investigate the role of insulin in the polycystic ovary syndrome (PCOS) pathogenesis and application of Metformin in the treatment of PCOS. METHODS: Serum androgen, luteinizing hormone (LH) and sex hormone-binding globulin(SHBG) concentration during fasting and serum 17 alpha-hydroxyprogesterone (17-OHP), LH level in response to gonadotropin-releasing hormone agonist (GnRH-a) stimulation were determined. Oral glucose-tolerance tests(OGTT) before and after oral administration of metoformin for 8-12 weeks were performed in 12 obese and 11 lean women with PCOS. RESULTS: After oral administration of metformin for 8-12 weeks, fasting insulin concentration in obese group and area under curve (AUC) after OGTT in lean group decreased significantly. There were significant decrease in basal 17 alpha-hydroprogesterone, androstenedione, testosterone concentration, and increase in serum SHBG concentration in obese and lean groups. Basal LH and the response of serum 17-OHP, LH to GnRHa were not significantly changed. CONCLUSIONS: Hyperinsulinemia may play an important role in hyperandrogenism GnRH-a of PCOS, and metformin may be used in the treatment of PCOS.

[Effects of mifepristone on the serum nitric oxide level and uterine blood flow in early pregnant women].

OBJECTIVE: To study the effects of mifepristone on serum nitric oxide (NO) level and uterine blood flow in early pregnant women. METHODS: Serum NO concentrations of 58 early pregnant women and 15 normal nonpregnant women were measured by a copper-coated cadmium reduction method, and uterine artery and trophoblastic blood flow were determined by color doppler flow imaging (CDFI). The ultrastructure of villi and decidua were studied under electron microscopy. RESULTS: Compared with nonpregnant women, serum NO concentration was significantly higher in early pregnant women(P < 0.01). After administration of mifepristone, serum NO level was significantly decreased, resistance index(RI) and systolic to diastolic ratio(S/D) ratio of uterine artery and trophoblastic blood flow were significantly higher than that of normal early pregnancy(P < 0.05). CONCLUSION: Mifepristone may exert effects on uterine blood flow by decreasing serum NO level.

[The effect of nimodipine on retinal blood flow in pregnancy induced hypertension].

OBJECTIVE: To assess the effects of nimodipine on retinal blood flow in patients with pregnancy induced hypertension (PIH). METHODS: Fourteen patients were studied with color flow Doppler before and after 30 mg nimodipine administered orally, while other fourteen patients received intravenous magnesium sulfate as control group. Central retinal artery flow velocity waveforms were measured and pulse index (PI) were calculated. RESULTS: Nimodipine reduced the central retinal artery PI significantly from 0.92 +/- 0.14 to 0.75 +/- 0.22. It also reduced systolic blood pressure from 20.7 +/- 2.5 kPa to 19.8 +/- 1.5 kPa and diastolic blood pressure from 14.4 +/- 2.3 kPa to 13.6 +/- 1.9 kPa. CONCLUSIONS: Nimodipine may dilate central retinal arteries. Changes in pulsatility index in retinal arteries maybe indicative of similar changes in other cerebral vessels. Nimodipine may be another choice of PIH therapy.

[Gastro-intestinal reconstruction by substitution of "O" shaped jejunum after total gastrectomy].

OBJECTIVE: To discuss a satisfying method for alimentary continuity reconstruction after total gastrectomy. METHOD: An "O" shaped jejunum pouch or a new "stomach" was constructed in between esophagus and duodenum after total gastrectomy. RESULTS: Thirty-nine cases of gastric malignant tumor were treated by this method. The 3 and 5 years disease free survival rate were 67.7% and 55.6%. All the patients recovered their normal meal habit (3 meals a day) within 6 months postoperatively. Compared with the preoperative and the postoperative (one year after operation), body weight is equal level, increasing and decreasing in 71.1%, 15.8% and 13.1% of the cases respectively. There were no posterior sternal pain and food or digestive fluid reflux, no dumping syndrome in these patients. The volume enlargement of the "new stomach" was observed obviously, especially in "stomach fundus". CONCLUSION: This operation is a satisfying method for alimentary canal reconstruction after total gastrectomy and it is worthy to be recommended in gastric surgery.

[Glycogenosis in cochlea of aging mice].

CBA/Caj mice and C57BL/6Nsd mice (animal models widely used for study of presbycusis) were used to study glycogen metabolic changes in the aging cochlea by a quantitative analysis of histochemistry. Glycogen contents in both hair cells and stria vascularis were significantly increased in CBA aged mice (18 and 36 months) as compared with those in CBA young animals (6 weeks), indicating a glycogenosis in the cochlea within naturally aging CBA mice. The glycogen contents in the stria vascularis in C57BL mice (6 weeks) was higher than in CBA (6 weeks) mice, whereas no significant difference could be discerned in the hair cell glycogen level. The results suggest that the glycogen metabolic changes in the CBA mice, a model of the natural aging cochlea, were different from in the C57 mice, a model of the cochlear genetic defect.

[Experimental study of anti-tumor immunity induced by B7 vaccine of a highly malignant murine leukemic T cell line (L615)].

OBJECTIVE: To assess the potential of B7-1 vaccine in inducing immunity to leukemic cells. METHODS: B7-1 gene was introduced into L615 cells and then the positive clone (L615-B7) highly expressing B7-1 was selected. Tumorigenic and immunoprotective activities of L615-B7 cells were studied in vivo. T cell functions of cytotoxicity, proliferation and growth factor secretion were detected in vitro.

[Parapharyngeal space neoplasms and surgical treatment].

This paper reports surgical treated 66 cases of parapharyngeal space neoplasms. They consist of primary 52 and secondary 14 cases, which were 59 benign and 7 malignant tumors. The surgical procedures which have been used are oral approach in 5 cases, the parotid approach in 5 cases, the cervical approach in 47 cases, the cervical approach with mandibular swing in 7 cases and the postaurical C incision approach in 2 cases. The procedures and indications of the cervical approach or (and) its combination with mandibular swing are outlined. It is considered that these two surgical procedures can provide very safe and efficacious approaches to the removal of the primary parapharyngeal space neoplasms encountered.

[Evaluation of accuracy of Healthdyne Nightwatch to monitor sleep breathing disorder].

OBJECTIVE: To study the advantage of computerized portable polysomnograph (Healthdyne Night Watch, NW) compared to standard paper scoring polysomnograph (PSG). METHOD: NW was used to monitor the same patient with sleep apnea syndrome at the same night and compared with standard PSG. RESULT: Apnea and hypopnea index (AHI) recorded by PSG was 28.6 +/- 6.9 versus 28.7 +/- 7.0 recorded by NW. Mean lowest SaO2 recorded by PSG was 73.5% +/- 2.8% versus 74.5% +/- 3.2% recorded by NW. Using a PSG value of AHI > or = 5 and > or = 10 as the diagnostic criteria, the diagnostic agreement of NW was 93.7% and 95.3%, respectively. CONCLUSION: The computerized portable polysomnograph (NW) saves time and labour. It could be used in patient's home or ward and could replace PSG in most situations.

[Clinical analysis of syndrome of obstructive sleep apnea (OSA) induced acute respiratory failure].

OBJECTIVE: To study the clinical characteristics of syndrome of OSA induced acute respiratory failure that might allow earlier recognition and treatment in critical care. METHOD: The clinical and laboratory characteristics of 9 patients were reviewed. RESULT: 9 patients (8 female, 1 male) presented with obesity and mental disturbance, with a BMI being 44.97 kg/m2, (45.25 kg/m2 in the female). The mean age of the group was 67.89 years (61-74 y). All had respiratory acidosis (mean pH 7.1732), hypercapnia (mean PaCO2 12.56 kPa) (8.53-19.09 kPa), and hypoxemia (mean PaO2 5.20 kPa) (3.87-5.87 kPa). During period of clinical stability all but 2 had awake hypercapnia (mean PaCO2 6.23 kPa) (5.10-7.29 kPa). Four of the 9 patients had pulmonary function test showing FEV1% > 70%. CONCLUSION: OSAS induced acute respiratroy failure has a sudden onset and various presentation and can be reversed with early and proper treatment. The severity of abnormal pulmonary function was less than what would be expected to cause respiratroy failure.

[Comparison of continuous positive airway pressure (CPAP) with uvulopalatopharyngoplasty in the treatment of obstructive sleep apnea syndrome].

OBJECTIVE: To compare the efficacy, side effects, and effects on parameters of sleep of continuous positive airway pressure (CPAP) and uvulopalatopharyngoplasty (UPPP) therapy in OSAS and evaluate their role in the treatment of OSAS. METHOD: 60 patients with OSAS were studied, 36 patients treated with CPAP, 24 patients with UPPP, and the parameters of polysomnography (PSG) obtained before and after treatment were analysed. RESULT: Respiratory disturbance index and sleep hypoxemia were improved significantly in both groups, but the CPAP group was better than UPPP group (P < 0.01). The efficacy was 97% in CPAP and 46% in UPPP group (P < 0.01). The longest apnea duration shortened in CPAP group (from 89 +/- 46 to 39 +/- 19) but there was no difference in UPPP (from 76 +/- 26 to 62 +/- 30) group. The apnea duration in 10 patients was lengthened. CONCLUSION: The effects of UPPP in OSAS were limited. CPAP was more effective than UPPP. CPAP may be indicated as the first choice in the treatment of OSAS.
