Child psychiatric consultation in pediatrics.

The results of a three-month study of the incidence of child psychiatric consultation requests in three different locations (emergency room, primary care center, and pediatric wards) within a pediatric service in a teaching hospital are reported here. Future trends and roles for child psychiatry and pediatric collaboration are discussed.

Reduction of coagulation factor XIII concentration in patients with myocardial infarction, cerebral infarction, and other thromboembolic disorders.

Coagulation factor XIII and plasma fibrinogen chromatographic assays have been performed serially in patients suffering from acute myocardial and cerebral infarction, and in others with disseminated intravascular coagulation. The findings were compared with 2 groups of "controls"; normal clinically-well subjects and hospitalized patients with cerebral infarction who exhibited minimal, stable, or improving neurological deficits. Substantial depression of factor XIII concentrations developed in the 3 patient groups, together with concomitant significant increases in the proportion and concentration of plasma high molecular weight fibrin(ogen) complexes (HMWFC). An inverse correlation (p less than 0.05) between coagulation factor XIII concentration and percentage of HMWFC was demonstrated in the early stages of the illness. These findings suggest that depression of coagulation factor XIII concentration in these states, is secondary to extravascular or intravascular coagulation and may reflect its degree.

Vitamin A receptors. II. Characteristics of retinol binding in chick retina and pigment epithelium.

Gel filtration studies demonstrate that retinol receptors of chick retinal and pigment epithelial cytosols are (1) of very similar nature (2) of small molecular size (about 18000 daltons) and are different in character from serum proteins. Citral inhibits the binding of [3H]retinol to the retinal 2 S receptor. Retinol acetate competes with retinol for binding to 2 S receptor in both retina and pigment epithelium whereas retinol palmitate is an effective competitor only in the pigment epithelium. Dithiothreitol maximizes 2 S binding in retina and pigment epithelial cytosol; its absence does not lead to receptor aggregation however. A limited number of high affinity binding sites (2 S receptor) appear to be present in retina and pigment epithelium. A 5 S binding species is also present in pigment epithelium; it is similar in character to [3H]retinol binding in serum and may arise from serum contamination of the pigment epithelial preparation. Binding affinity in retina is high with possibly two classes of retinol binding sites present of KD about 1 - 10(-9) and 4 - 10(-8).

Experimental infarct sizing using computer processing and a three-dimensional model.

A method for noninvasive sizing of myocardial infarction, in which data from technetium-99m stannous pyrophosphate scintigrams and a three-dimensional model were used, was tested on experimental, acute anterior infarcts in dogs. The results indicate that the method does size experimental anterior infarcts accurately, but further testing will be necessary to assess the capabilities of the technique for sizing other types of infarcts.

Retinol receptors in corneal epithelium, stroma and endothelium.

Specific receptors for retinol are present in the cytosol fraction of corneal epithelium as demonstrated by sucrose density gradient centrifugation. These appear to be (1) protein in nature (2) of small molecular size (2 S) (3) specific for retinol and (4) present in several species. Assuming a receptor molecular weight of 15 000 and a single mole of retinol bound/mole of receptor protein, the association constant value is 5.26-10(7) with deltaG degrees = -8.53 kcal/mol. 2-S receptors are also observed in stroma and endothelium along with another binding species of approximately 8 S. Binding of [3H]retinol in bovine epithelial cytosol can also be demonstrated by disc gel electrophoresis and gel filtration. Immunodiffusion techniques demonstrate that monkey corneal epithelial and stromal cytosol samples do not contain contaminating serum retinol binding-protein.

Rh isoimmunization, Manitoba, 1963-75.

The number of Rh-isoimmunized pregnancies in Manitoba has been reduced from 223 and 228 in the years ending Oct. 31, 1963 and 1964 to 60 and 62 in the years ending Oct. 31, 1974 and 1975. The number per 1000 total births in the same years has decreased from 10.0 and 10.6 to 3.4 and 3.5 Perinatal mortality rates in those years decreased from 13.8 amd 15.7% to 0 and 2.2%, respectively. The number of perinatal deaths has been reduced from 55 in the first 2 years reported to 1 in the last 2 years. Among the 121 isoimmunized women pregnant in the 2-year period ending Oct. 31, 1975, isoimmunization was due to failure to give Rh immune globulin after delivery in 33 and failure to give it during pregnancy in 48. Of the remaining 40, 37 were immunized before Rh immune globulin became available. Complete prevention of Rh isoimmunization and therefore of all perinatal deaths from Rh erythroblastosis can only be achieved through universal Rh testing prenatally and immediately after delivery, and institution of an antenatal Rh prophylaxis program.

Results and complications of intraaortic balloon pumping in surgical and medical patients.

The intraaortic balloon was attempted for therapy in 94 patients and successfully placed in 86. The balloon catheter could not be passed through the femoral or iliac artery in 12 patients (13 percent) of the group; in 4 of these the balloon was inserted through an aortic arch graft. The medical indications were cardiogenic shock and preinfarction angina. Ten of the 14 patients in the group with shock survived when treated with an aortic balloon without emergency surgery. Indications for balloon pumping in the surgical group included inability to wean the patient from the pump-oxygenator, postoperative shock and prophylactic placement of the balloon for poor ventricular function. Inability to remove a patient from pump-oxygenator support was the most common surgical indication, and 47 percent of patients were long-term survivors. Only 1 of the 17 patients for whom balloon pumping was used prophylactically died. Complications occurred in 17 percent of the entire group of 86 patients although the rate for medical patients with cardiogenic shock was 50 percent. The most common complication was arterial insufficiency requiring removal of the balloon. Four patients had permanent damage to the legs from ischemia, one patient requiring bilateral amputation. The overall incidence of serious arterial obstruction was 10 percent. Other complications included balloon displacement with arterial obstruction and pericardial tamponade from anticoagulant agents resulting in death.

Radioimmunoassay of free thyroid hormone concentrations in the investigation of thyroid disease.

Direct radioimmunoassay of free triiodothyronine (fT3) and free thyroxine (fT4) in serum dialysate has been performed in fifty-two normals, twenty-one hyperthyroid, and twenty-one hypothyroid patients. In health, there is an almost equimolar concentration of fT3 and fT4. In hyperthyroidism, the free thyroid hormone concentrations were clearly discriminated from the normals. In a proportion of the symptomatic hypothyroids studied, the free hormone values fell within the normal range, challenging the concept that free thyroid hormone concentrations are the sole determinants of thyroid status. In the population studied, there were statistically significant correlations between fT3 and fT4, as well as the free hormones with their respective total hormone levels. It is concluded that fT3 and fT4 estimated are unlikely to play a major role in the routine investigation of thyroid disease.

Colton blood groups in Canadian Caucasians: frequencies, inheritance and linkage analysis.

In a series of 726 random Caucasian adults tested with anti-Coa and anti-Cob allele frequencies were Coa 0.9470, Cob 0.0530. The distribution of Colton phenotypes in 205 children of 57 selected families was in accordance with expectation. A linkage analysis of Co and 26 other loci indicates that Co is not closely linked to AcP1, ABO, AK1, Ch, Do, GLO, Gm, GPT, HLA, Hp, K, Lu, MN, PGM1 or Rh and does not lie between PGD and PGM1 on chromosome 1.
