A method of repair for quadriceps tendon or patellar ligament (tendon) ruptures without cast immobilization. Preliminary report.

The quadriceps tendon and patellar tendon (ligament) were repaired with a Dacron vascular graft used as a tension suture material. In cases of quadriceps tendon ruptures, the Dacron graft is passed transversely through the patellar ligament just below the patella and crossed transversely at the level of the musculotendinous junction with two loops applying tension to the tendon, which brings the tendon ends together by creating a solid structure. In cases of patellar ligament ruptures, the Dacron graft is passed through a hole in the tibia posteriorly to the tibial tuberosity instead of through the patellar ligament below the patella. This technique enables early mobilization on the first day after surgery. The technique was first tested on six dogs with severed quadriceps tendons and patellar ligaments that were repaired with this suture method. All of the animals recovered from surgery and walked and ran normally on the repaired legs within 27 days and with only mild limping after 17 days. The technique was then used on six patients, four with complete quadriceps tendon rupture and two with complete tear (avulsion) of the patellar ligament (tendon). In all of the patients, excellent surgical results were obtained and leg immobilization was virtually eliminated. Physical therapy was prescribed the first day after surgery. The rehabilitation period was significantly reduced.

[Thyroid hormones in rats receiving increasing doses of lithium].

Thyroid hormones were determined in rat serum, thyroid, liver and kidney tissues; the animals were fed for 40 days an equilibrate diet and various amounts of lithium carbonate were added to drinks. Serum lithium levels were related to their absorption. There was no change in rats drinking 300 mg Li2CO3/l solution for 5 weeks or 600 mg/l for 2 weeks, when compared to control rats. But 600 mg/l solutions for 5 weeks led to hormone increase in thyroid tissue. With 1200 mg/l acute hormonal decrease was seen in serum (thyroxine, T4: 3.2 micrograms/dl and control: 4, triiodothyronine T3: 54.8 ng/dl and control: 64) in liver tissue T3: 2.6 ng/g and control: 3.5), and in kidney tissue (T3: 4.8 ng/g and control: 6.4). In some animals receiving 600 or 1200 mg, lithium concentration was very high, body weights were very much reduced and there was large hormonal decrease in serum (T4: 1.4 micrograms/dl, T3: 48.5 ng/dl) in liver tissue (T4: 10.5 ng/g and control: 20.7; T3: 2.1 ng/g) and in kidney tissue (T4: 6.9 ng/g and control: 13.5 and T3: 3.3 ng/g). Hormonal changes were parallel to tubular kidney damage. These results are similar to those described in patients receiving lithium treatment.

[Return to work after myocardial infarction. Study of factors in return to work by the statistical method of analysis of correspondences].

A retrospective study has examined the predictive factors of return to work in 184 males patients with myocardial infarction between 1975 and 1982. The statistical method used was that of correspondence analysis which enables to define correlations between variables, here 12 in number. The chances of returning to work are positively correlated to young age (48-52 years), school education and social standing. They were not increased by coronary by-pass surgery. Cardiac insufficiency, and a large area of infarction contribute to the non-return to work, while subsequent angor and arrhythmias do not demonstrate any significant relation. Finally, a patient who has not returned to work after one year seems to have few chances of becoming active again.

Gastroenterostomy for treatment of gastroduodenal ulcer disease in 14 foals.

Gastroenterostomy was performed in 14 foals to treat gastric outflow obstruction caused by advanced gastroduodenal ulcer disease. The onset of excessive salivation and teeth grinding, without response to medical treatment, combined with endoscopic and radiographic evidence of gastric outflow obstruction, were indications for surgical intervention. Successful outcome in 5 foals was attributed to early diagnosis, patient stabilization, early surgical correction, and postoperative management including antibiotics and antiulcer medication.

Pain in Parkinson's disease.

We studied the prevalence and character of pain in Parkinson's disease (PD) and its association with motor fluctuations. Of 95 outpatients, 46% experienced pain they attributed to PD. Patients with pain were younger but no more disabled on objective motor scores than patients without pain. Musculoskeletal, dystonic, and joint pains were most frequent. Painful episodes, especially musculoskeletal cramps, usually occurred when parkinsonian disability was maximal.

Excretion of dipyrone metabolites in human breast milk.

Breast milk and plasma levels of dipyrone metabolites in 8 mothers given a single oral dose of the drug were determined by HPLC. Four metabolites were demonstrated by the analytical method: 4-methylaminoantipyrine (MAA), 4-aminoantipyrine (AA), 4-formylaminoantipyrine (FAA) and 4-acetylaminoantipyrine (AAA). A good correlation was found between the plasma and milk concentrations of the metabolites. The mean (+/- SD) milk to plasma concentration ratios were: MAA = 1.37 +/- 0.28, AA = 1.15 +/- 0.40, FAA = 1.03 +/- 0.09, AAA = 0.97 +/- 0.24. The disposition pattern of the dipyrone metabolites in milk was studied in two mothers. None of the metabolites was detectable 48 h after drug administration.

The susceptibility of Cryptococcus neoformans to an antimycotic agent (G2) from alfalfa.

Compound G2, 2-beta-hydroxy-3-beta-O-(beta-D-glucopyranosyl)-delta 12-oleanene-23,28-dionic acid, isolated from alfalfa roots, demonstrated considerable activity against Cryptococcus neoformans (MIC value of 2 micrograms/ml). Compound G2 exhibited rapid killing of this fungus (MFC value of 4 micrograms/ml) suggesting that it might be a useful active agent in the treatment of cryptococcosis.

H deficiency in two brothers with atypical dense intramembranous deposit disease.

We report an H deficiency in two Algerian brothers who had early-onset glomerulonephritis. In addition, one suffered from serious lung infections. The H deficiency was defined by undetectable CH50 and AP50, and low levels of H, C3 and B (less than 10% of normal levels). I and classical pathway components, including C4-bp were normal. CR1 was present on both patients' erythrocytes. No nephritic factor or other circulating alternative pathway activator was detected. The parents, who are first cousins, and a healthy brother and sister had half-normal levels of H. These findings favor an autosomal recessive transmission of the H defect. Although by electron microscopy renal biopsies from both patients were typical for dense intramembranous deposit disease, immunofluorescence microscopy showed an atypical pattern with abundant granular C3 deposits within the mesangium and along the capillary walls. Alternative pathway activators, possibly related to dense deposits, may allow the formation of membrane-associated C3/C5 convertases, unusually stable in the absence of H, since C5, C6, C7, C8 and C9 levels were decreased in both patients. This observation may represent an interesting clue to the relationship between nephritic factor, alternative pathway activation, and dense intramembranous deposit disease.

Activity of compound G2 isolated from alfalfa roots against medically important yeasts.

An antimycotic agent was isolated from roots of alfalfa and further purified to yield a nonhemolytic, homogeneous compound (G2). This compound contained considerable activity against 10 medically important yeasts. MICs obtained by both agar and broth dilution methods ranged from 3 to 15 micrograms/ml. Compound G2 was fungicidal at a relatively low concentration for nine different species of yeasts tested (minimum fungicidal concentrations ranged between 6 and 24 micrograms/ml). The considerable stability of compound G2 and its strong inhibitory and fungicidal activity against a broad range of yeasts suggest that after further development it might be useful as an active agent in the treatment of mycotic infections.

Aneurysmal bone cysts: MR imaging at 1.5 T.

Two patients with aneurysmal bone cysts of the pelvis were imaged using a 1.5-T magnetic resonance imaging device. Findings included multiple internal septations, cysts with fluid-fluid levels of varying intensity, and an intact rim of low-intensity signal completely surrounding the lesion. These findings allow a specific diagnosis of aneurysmal bone cyst to be made.
