Osteoporosis and fractures of the neck of the femur: some epidemiologic considerations.

The prevalence of osteoporosis in Jerusalem during the years 1967-71 was studied in a general population sample and in patients with fractures of the neck of the femur. Osteoporosis was determined by qualitative examination of lateral X-ray films of the lumbar spine. The prevalence of definite osteoporosis in a random population sample was 8.6%, and was higher in females (14.5%) than in males (2.9%). The prevalence of definite osteoporosis among 389 patients with fractures of the neck of the femur was 32.2%--37.8% in females and 17.4% in males. The results of this survey support the conclusion that osteoporosis plays an important role in the pathogenesis of fractures of the neck of the femur.

Amyloidosis of the kidneys. Review of patients and literature.

The clinical findings in 49 patients (27 males) who had histologically confirmed amyloidosis of the kidney, are reviewed. In 28 patients, the diagnosis was arrived at by percutaneous renal biopsy, and in 21 patients it was made at autopsy. The youngest patient was diagnosed at the age of 16 years. In 11 patients (22,5%) no associated disease was found, in a further 11 patients the amyloidosis was of the type connected with familial Mediterranean fever, and in 27 patients (55%) the amyloidosis was of the secondary type. Forty-six patients (94%) presented with proteinuria at some stage of their disease. Twenty-one patients developed the nephrotic syndrome. Thirty-two patients died, and in 18 instances uraemia was the cause of death. In 18 patients there was evidence of renal pathology other than amyloidosis, and in some patients this may have contributed to the impairment of renal function and the appearance of proteinuria.

Coincidence of Sarcoidosis and Hashimoto's thyroiditis.

A 59-year-old, female patient died of pulmonary embolus 16 days after an operation for a Moore prosthesis. At autopsy, previously unsuspected sarcoidosis and Hashimoto's thyroiditis were found. The sarcoidosis was noted in one focus in the lung and in all lymph nodes examined. In the thyroid gland, Hashimoto's thyroiditis was observed. In addition, structures that could not be differentiated morphologically from papillary Hurthle cell carcinoma were seen in the thyroid.
