[Descriptive epidemiology of Ewing's tumor--analysis of German patients from (EI)CESS 1980-1997].

BACKGROUND: Due to low absolute morbidity rates a reliable descriptive epidemiology of the Ewing family of tumors (EFT) has been mainly provided by large population-based cancer registry reports. To date, only few reports on this topic have been published. PATIENTS: The present report is based on data of 945 German patients who were enrolled into the (EI)CESS therapy studies between 1980 and 1997. It is assumed that under the age of 15 years an almost complete registration of all German patients with Ewing's sarcoma has been achieved. Diagnoses in all patients were ascertained by biopsies. METHODS: In this analysis we looked at the associations between year of first diagnosis, age at first diagnosis, gender, the histological subtypes and the primary skeletal localisation of the tumours. RESULTS: The majority of patients with EFT was male (60%). More than half of the patients developed the disease between age 10 and 19 (57%) while about 20% were younger and almost a quarter was diagnosed after age 20 years. The percentage of patients older than 15 years was higher in men (55%) than in women (45%). The age-standardized annual incidence before age 15 years was estimated as 3 per million, and the cumulative incidence up to that age as about 46 per million. Long bones of the lower extremities (32%, femur alone 16%) and the pelvis were the most common sites of primary localisation. While 70 to 80% of all tumours in the long bones of the extremities were composed of Ewing's sarcomas, about one third in the central skeleton and over 20% in the pelvic bones were Primitive neuroectodermal tumours (PNET). Atypical Ewing's sarcoma accounted for about 10% of tumours in all sites. CONCLUSIONS: The large (EI)CESS database enables the meaningful and reliable description of epidemiological characteristics of the rare occurrence of the Ewing family of tumours. Further analyses of this database seem to hold great promise.

[Fatal course of a fungal pneumonia in glucocorticoid-treated recurrent febrile nodular panniculitis; Pfeifer-Weber-Christian syndrome].

The history of a 63 yr. old woman with a serious form of Weber-Christian's disease is reported. During treatment with glucocorticosteriods and antibiotics bilateral condida pneumonitis developed, which ended fatally. A short review of Weber-Christian's disease is given, and the problems concerning its treatment are outlined. The systemic application of glucocorticosteroids in this disease should be reserved for the serious forms.

Free vascularized muscle flap transplantation in the treatment of chronic osteomyelitis.

We are reporting our experience with the efficacy of free tissue transfer in treatment of chronic lower-extremity osteomyelitis; 23 latissimus dorsi flaps and 11 rectus abdominis flaps were used. In our series, success was achieved after a mean follow-up of 36.1 months in 79% and after additional wound debridement in 94%. Indications, methods, results and complications are reported.

Sensory recovery following total and subtotal amputations distal to the wrist and digital replantations.

Apart from the basic aim of revascularization, other criteria for the success of digital replantation are functional and sensory recovery. Tactile gnosis can only be restored in cases of replantation distal to the wrist by paying great attention to the technical details for nerve repair. Of 253 patients who underwent 312 digital replantations, after total or subtotal amputations, over 16 years, 162 with 215 successful replantations and 377 nerve reconstructions returned for follow-up evaluations. Of 320 patients who underwent 395 nerve repairs after isolated severed digital nerves, 198 with 249 nerve reconstructions returned for follow-up. Our aim was to determine in both groups, the level of sensory recovery for primary nerve repair as well as for primary and secondary nerve grafting by evaluating the results according to the score of Highet. The majority of the patients in both groups reached stage S3 to S4 on the sensory recovery score of Highet. Patients with isolated severed nerve reached stage S3+ more often than the patients with nerve reconstruction after replantation. The evaluation of results takes into consideration factors influencing digital sensibility such as the level and type of injury, the length nerve grafts, cold intolerance and the patient's age.

Chemotherapy resistance in breast cancer.

Chemotherapy resistance is a major problem in the management of patients with breast cancer. Clinical resistance of solid tumors such as breast cancer is likely to be multifactorial and heterogeneous. Usually, patients refractory to chemotherapy exhibit resistance to multiple cytotoxic agents of different structure (and often function). A comparable experimental phenomenon has been termed multidnug resistance or MDR. MDR can be caused by various molecular mechanisms. One of these mechanisms is overexpression of MDR1/P-glycoprotein, which cna be expected in around 30-40% of primary and 50% of metastatic breast cancers. Preliminary evidence suggests that P-glycoprotein-positivity is associated with poor treatment outcome in both primary and advanced breast cancer. Studies of MDR reversal in metastatic breast cancer have generally yielded negative results. Recently, however, we found dexverapamil to be able to induce partial remissions to epirubicin in 4/23 patients (17%) with metastatic breast cancer refractory to the same dose and schedule of epirubicin alone. Dexverapamil did not increase the toxicity or the area under the plasma concentration-time curve of epirubicin. More carefully designed and conducted studies are needed to conclusively determine the clinical relevance of various resistance mechanisms in breast cancer and whether chemosensitizers such as dexverapamil or cyclosporins are able to enhance chemotherapy efficacy in this tumor.
