Procoagulant activity and thrombelastography in Korean hemorrhagic fever.

Twenty male patients with Korean hemorrhagic fever were evaluated with thrombelastography (TEG) to assess the changes in coagulation system, and the results were compared with those of conventional coagulation tests. Procoagulant activity in the plasma was determined by comparing the reaction time "r" of the normal plasma and that of the mixture of equal parts of the normal plasma and the patient's plasma. The TEG was found to be a useful measure of the changes in the coagulation profile, and provided instant accurate assessment of the patient's hemostatic function. Presence of the procoagulant activity was demonstrated in the plasma of the patients and indicated occurrence of active intravascular coagulation during the early stage of the disease.

Community integration through therapeutic recreation: day-treatment patients compete in a softball league.

A team of male psychiatric patients drawn from two community mental-health day-treatment centers competed in a community-based softball league. Treatment objectives were twofold: therapeutic recreation and participation in a community activity apart from a traditional psychiatric setting. Points of success, while anecdotal, seem to suggest avenues through which further efforts toward community integration may be achieved.

Primary intramedullary spinal cord lymphoma: report of a case.

A case of primary intramedullary spinal cord malignant lymphoma is described. The patient was a 24-year-old female with a preceding history of a lupus-erythematosus-like disorder. The presenting symptom of the spinal cord tumor was spastic paresis of the lower extremities. Radiological examinations including magnetic resonance imaging (MRI) of the spinal cord revealed an intramedullary tumor from levels C2 to T6. A decompressive laminectomy was performed with the pathological diagnosis of poorly differentiated lymphocytic lymphoma with T-cell surface marker. Postoperative cerebrospinal radiation up to 40.5 Gy was performed. The patient is alive without disease 18 months after completion of the therapy. MRI proved to be a very useful method for evaluation of the response to therapy and for follow-up. Primary involvement of the spinal cord with non-Hodgkin's lymphoma is unusual and this is the ninth case of primary intramedullary spinal cord lymphoma reported.

Seroepidemiology of human T-cell leukemia virus type-I in the Republic of Korea.

We investigated the prevalence of antibodies to human T-cell leukemia virus type I (HTLV-I) associated antigens in various patients and healthy individuals in the Republic of Korea. Seventeen out of 6,255 individuals (0.27%) were seropositive. The positive rate in males was 0.21% and that in females was 0.36%, the sex difference being similar to that in endemic areas in Japan. HTLV-I carriers were found in Seoul and neighboring areas, but not in Busan city or Jeju Island, which are adjacent to Kyushu, Japan. The prevalence of sera with definite anti HTLV-I antibodies seems to be very low in the Republic of Korea.

[Resected hepatocellular carcinoma with macroglobulinemia--a case report].

A 54-year-old man had been diagnosed as having macroglobulinemia 10 years earlier; Ultrasonography and liver scintigraphy showed a space-occupying lesion in the right lobe of the liver; CT scan showed abnormal density in the same region. IgG was more than 7,000 mg/dl. The tumor was isolated, and liver function was good. On laparotomy, biopsy of the tumor and one of the enlarged lymph nodes in the hepatoduodenal ligament was undertaken. The diagnosis the from frozen tumor section was hepatocellular carcinoma; malignancy was not found in the lymph node. Right lobectomy was done. The patient left the hospital two months later after an uneventful recovery.

Modulatory action of serotonin on glutamate-induced excitation of cerebellar Purkinje cells.

The effects of serotonin (5-hydroxytryptamine, 5-HT) on glutamate-induced excitation of Purkinje cells were examined. Pulsatile iontophoretic applications of glutamate (1-25 nA) induced consistent increases in the spontaneous activity of Purkinje cells. When serotonin was applied continuously with currents that elicited minimum changes in the spontaneous rate, it inhibited or blocked glutamate-induced excitations significantly in most Purkinje cells. We also examined the effects of high currents of serotonin on spontaneous activity of Purkinje cells. High currents of serotonin induced 3 different effects: inhibitions, biphasic effects comprising transient inhibition followed by excitation, and excitations. Nonetheless, whether it inhibited or excited the activity of Purkinje cells, serotonin inhibited glutamate-induced excitations consistently. The effect of a putative 5-HT antagonist methysergide (UML) was also examined. Methysergide consistently attenuated or antagonized the inhibitory effects of serotonin on glutamate-induced excitations. This finding suggests strongly that inhibitory effects of 5-HT on glutamate excitations observed in the present study is the specific action of serotonin.

Determination of phenylethanolamine N-methyltransferase by high-performance liquid chromatography with fluorescence detection.

A highly sensitive assay method for phenylethanolamine N-methyltransferase in rat adrenal medulla and brain is described which employs high-performance liquid chromatography with fluorescence detection. Epinephrine formed enzymatically from the substrate norepinephrine and isoproterenol (internal standard), after chromatography on a small cartridge of a cation exchanger, Toyopak SP, are converted into the corresponding fluorescent compounds by reaction with 1,2-diphenylethylenediamine, a selective fluorescence derivatization reagent for catechol compounds. The derivatives are separated by reversed-phase chromatography on TSK gel ODS-120T. The detection limit for epinephrine formed enzymatically is 0.66 pmol per assay tube.

Delayed gentamicin elimination in patients with severe preeclampsia.

Gentamicin is an aminoglycoside antibiotic frequently used in the treatment of mixed polymicrobial infections. Because patients with preeclampsia exhibit compromised renal function, decreased intravascular volume, and increased extravascular fluid, it was theorized gentamicin may be handled differently in normotensive versus preeclamptic patients. Eighteen patients with postpartum endometritis receiving gentamicin were divided into three treatment groups based on the presence and severity of preeclamptic symptoms. After the gentamicin steady state was achieved, serum samples were collected at 30, 150, and 450 minutes following completion of a timed 30-minute infusion. The mean half-life and gentamicin clearance in severe preeclamptic patients (3.15 hours and 91.5 ml/min) were significantly different from normotensive patients (2.38 hours and 135.9 ml/min) and mild preeclamptic patients (1.87 hours and 142 ml/min). No significant differences in volume of distribution were seen. A high correlation between elimination rate and distribution volume was noted in normotensive patients; however, a moderate correlation was seen in patients with preeclampsia. Therefore, alterations in renal function in patients with severe preeclampsia, and not changes in volume of distribution, may be the primary cause for delayed aminoglycoside elimination.

A biotin-avidin enzyme immunoassay for the quantitation of serum ferritin.

A biotin-avidin enzyme immunoassay (EIA) for measurement of ferritin in human serum is described. This procedure involves the use of specific antibody immobilized on beads, biotin-labeled specific antibody and enzyme-labeled avidin. Reproducible results were achieved within 2 h for ferritin in serum in the range of 5-500 ng/ml. Ferritin levels as low as 0.1 ng/ml can be measured. The biotin-avidin EIA, a radioimmunoassay kit, and an EIA kit were used to determine serum ferritin levels in healthy adults. Good agreement was found between the biotin-avidin EIA and the other methods.

Long internal inverted repeat in a yeast viral double-stranded RNA.

The Saccharomyces cerevisiae viruses are non-infectious double-stranded (ds) RNA viruses present in most laboratory strains of yeast. Their genome consists of one or more dsRNAs separately encapsidated in particles composed mainly of one polypeptide, which has a Mr of 88 kdaltons in the best-studied viral subtype. A large viral dsRNA (L1, of 4.7 kb) encodes the capsid polypeptide. We have determined the sequences of a number of cDNA clones homologous to portions of L1 and mapped them by a novel heteroduplex technique. Several of these clones originate from a region of L1 2.3-2.5 kb from the 5' end of the plus strand that contains stop codons in all three reading frames in the plus strand. We therefore suspect that the capsid polypeptide gene lies in the 5' 2.3-2.6 kb of the plus strand. One of the cloned cDNAs has an inverted repeat of 170 bp that appears to be present in its parental RNA. The inverted repeat in L1 is the longest known inverted repeat in a viral dsRNA and the only known non-terminal inverted repeat. It might serve the function of creating two mRNAs from one viral dsRNA.

Azathioprine and steroids are not more effective in decreasing multiple sclerosis intra-blood-brain-barrier IgG synthesis than steroids alone.

Intra-blood-brain-barrier IgG synthesis rates and oligoclonal IgG banding patterns were examined in 9 patients with multiple sclerosis who were treated with azathioprine and steroids for 2 to 4.5 years. The IgG synthesis rates of 5 patients were significantly decreased from the pretreatment mean values 1 month after treatment, and their synthesis rates remained at the decreased levels throughout treatment. However, among the remaining 4 patients, the rates exceeded the pretreatment means. This continuous suppressive effect of the combined azathioprine and steroids upon the IgG synthesis rate was similar to that of steroids, suggesting that azathioprine and steroids in combination were not more effective in reducing intra-blood-brain-barrier IgG synthesis than steroids alone. Oligoclonal IgG patterns in all cerebrospinal fluid samples were not significantly altered during the study.

Potentiation of chemically induced cleft palate by ethanol ingestion during gestation in the mouse.

The influence of ethanol consumption on cleft palate induction by methylmercury, cortisone, and retinyl acetate was investigated in Swiss white mice. Consumption of 20% ethanol throughout gestation significantly increased the incidence of cleft palate compared to water-fed mice, when methylmercury was given on four consecutive days (days 9-12, 5 mg/kg of body weight). Ethanol also increased the incidence of cleft palate in mice given retinyl acetate (3,400 or 5,100 IU) on day 12, compared to retinol acetate-treated mice given water, but did not affect cleft palate induction by cortisone (2.5 mg/d, days 8-11). Ethanol significantly reduced fetal weight in the presence or absence of the three teratogens, but the results do not support a hypothesis that growth retardation is directly responsible for the potentiating action of ethanol. It may be that ethanol acts to increase cleft palate induction by some teratogens by retarding fetal developmental processes.
