A 38 base pair insertion in the pro alpha 2(I) collagen gene of a patient with Marfan syndrome.

Abnormalities in type I collagen have been recognized in a number of connective tissue disorders. In the Marfan syndrome, an autosomal dominant condition producing a generalized abnormality in connective tissue, no consistent abnormality has been identified, although one individual has been found to have an elongated pro alpha 2(I) collagen chain [Byers et al, Proc Natl Acad Sci USA 78:7745, 1981]. To determine the nature of the alteration in the gene that produced this abnormality, we studied the pro alpha 2(I) gene from this individual by genomic blotting and gene cloning. Genomic mapping studies detected no abnormalities. However, analysis of the cloned segment of the pro alpha 2(I) collagen gene from the Marfan individual indicates that the gene contains a 38 base pair insertion in an intron near the collagenase cleavage site. Although the relationship of this insertion to the protein abnormality is unclear, it may be a useful marker for the diagnosis of the Marfan syndrome.

In-situ neoplasia in squamous cell carcinoma of the parotid. A case report.

This case report demonstrates the origin of a primary squamous cell carcinoma of the parotid gland in squamous metaplasia of the parotid ducts. Comparison is made between parotid ductal intraepithelial neoplasia and cervical intraepithelial neoplasia. The clinical importance of establishing the primary origin of these tumours and the need for extensive tissue sampling is emphasized.

Synovial infection with Mycobacterium kansasii.

Atypical mycobacteria have been recognised as saprophytic organisms for many years, but it was only with the development of better microbiological culture techniques that they became recognised as potentially pathogenic to man. Infections of tendon sheaths and joints by these organisms may present diagnostic problems, and we report here 3 cases in which Mycobacterium kansasii was responsible for disease at the hand and wrist.

Increased alpha-fetoprotein concentration in association with ileal adenocarcinoma complicating Crohn's disease.

A considerably raised serum alpha-fetoprotein concentration was found in a patient with an ileal adenocarcinoma. alpha-fetoprotein was shown in both the primary tumour and the hepatic metastases by an immunoperoxidase technique. A raised alpha-fetoprotein concentration in association with ileal adenocarcinoma has not been described previously, and it is notable that the first such case should occur as a complication of longstanding Crohn's disease.

Prevalence of abnormal involuntary movements ("spontaneous dyskinesias") in the normal elderly.

Advancing age has been associated with increased prevalence of abnormal involuntary movements in neuroleptic-treated patients. In order to investigate the possibility that the aging process itself might produce involuntary movements clinically indistinguishable from tardive dyskinesia, a group of healthy elderly volunteers were systematically examined. A 4% prevalence rate of mild involuntary movements was found, suggesting that the aging process itself is not likely to produce dyskinesias without other contributing factors coming into play.

Comparison of oral prostaglandin E2 and intravenous oxytocin for induction of labour.

A carefully controlled, prospective study was undertaken in 100 patients to compare orally administered prostaglandin E2 capsules with intravenous oxytocin in the induction of labour at term. Orally administered prostaglandin E2 induced labour in slightly more patients than did intravenous oxytocin (48 out of 50 v. 45 out of 50) and a larger trail is in progress to evaluate this observation further. Mild gastro-intestinal symptoms occurred in 28% of the patients when oral prostaglandin was given. However, the oral route is more convenient and more comfortable for the patient and the administration requires less adjustment by the nursing staff.

Neuroendocrine carcinoma of the larynx.

A rare case of moderately differentiated neuroendocrine carcinoma (atypical carcinoid) of the larynx is presented. The role of immunohistochemistry in the diagnosis of the lesion is described. The importance of considering the diagnosis in a patient with prolonged cervical pain with features of glossopharyngeal neuralgia, is discussed.

Feulgen-DNA content of human sperm and outcome of in vitro fertilization.

Feulgen-DNA content of sperm was quantified in fresh semen samples of 20 consecutive couples during preparation for in vitro fertilization to measure the condensation state of sperm chromatin. Results were expressed as a percentage of expected haploid DNA value (1c). No significant differences were found between those who did not get pregnant, those who went full term, and those who aborted. Because fertilization can occur with a minority of normal sperm, it is concluded that the present findings do not provide useful clinical information about pregnancy outcome and in vitro fertilization.
