A prospective one-year study of horse-riding accidents was performed in an area with a very high frequency of amateur riding. Totally 174 patients with riding injuries were registered--66% children--making an incidence of only 0.7 per thousand riding occasions. However, many of these injuries were of a severe nature, one leading to death and 27 of the patients reporting persisting symptoms at follow-up one year after the accident. Eleven per cent of the accidents lead to cerebral injuries and 44% to fractures, mainly of the upper extremities. Eleven per cent required hospitalization and 17% operation under general anaesthesia. Clarification of the circumstances concerning the accidents indicated that many of the severe injuries could probably have been avoided by use of better safety equipment.

Affinity systems based on specific molecular recognition are valuable tools for detection, purification and immobilization of recombinant proteins. Here, novel multipartite affinity fusion vectors were assembled and investigated to allow flexible binding and elution conditions. The rationale for the assembly of different combinations of affinity domains was to take advantage of the wide variety of molecular interactions of these domains for purification, solubilization, detection and immobilization. In total, seven different affinity tags representing five different types of tag-ligand interactions were studied: (i) monoclonal antibodies-peptides (T7-tag and FLAG peptide); (ii) streptavidin-peptide (Strep-tag); (iii) hexahistidyl-metal ions (His6-tag; (iv) bacterial receptors-serum proteins (staphyloccal protein A-Fc and streptococcal protein G-serum albumin); (v) streptavidin-biotin (in vivo biotinylated peptide). Selected tags were evaluated for the production and purification of Escherichia coli DNA polymerase I (Klenow fragment). On the basis of the results, a vector (pAff2c) was assembled using a novel combination of affinity domains: (i) an in vivo biotinylated peptide; (ii) a His6 sequence, and (iii) a highly soluble serum albumin binding region. Using these three affinities, a wide variety of conditions can be employed for both the binding and the elution steps.

BACKGROUND AND OBJECTIVES: Wound infiltration with bupivacaine is often used for pain relief after inguinal hernia surgery. We hypothesized that the lower systemic toxicity of another long-acting local anesthetic of similar potency (ropivacaine) would make it possible to increase the dose to above that recommended for bupivacaine and thereby achieve more effective pain control. METHODS: Elective unilateral open hernia repair was performed on 144 patients at 4 hospitals. Surgery was performed under general anesthesia and, in a double-blind manner, the operating field was infiltrated with 40 mL ropivacaine 7.5 mg/mL (in = 73) or bupivacaine 2.5 mg/mL (n = 71 ) for postoperative pain relief. Pain at rest, on mobilization, and on coughing was assessed repeatedly during 24 hours using a visual analog scale. The patients' ability to walk and the need for supplementary analgesics were also recorded. RESULTS: No statistically significant differences were found between the two groups with respect to pain scores, which the patients reported to be less than 15% (median) of the worst pain imaginable in all examinations performed at rest, or in the consumption of supplementary analgesics. Those who received ropivacaine could walk with no or only minor problems at an earlier stage than the bupivacaine patients (P < .03). Both treatments were well tolerated. CONCLUSIONS: Wound infiltration with long-acting local anesthetics resulted in low pain scores after hernia surgery. Bupivacaine 100 mg was as effective as ropivacaine 300 mg.
Item memory and memory for modality format were assessed in young, young-old, and old women. Participants were presented with mixed-modality lists and items were related to four modality formats: auditory, olfactory, visual, and tactile. Memory was tested immediately after and 48 hours after inspection. Results indicated that both the young-old and old adults were disproportionately impaired in source memory relative to item memory, although they were not selectively impaired for any type of information in item memory. However, when recollection of source was required, the older age groups generated disproportionately more confusions to auditory items for olfactory information at delayed testing, and the reverse was true for the auditory information, which was most often confused with odors. Taken together, these results suggest that elaborating and reconstructing the source of olfactory information may be particularly difficult for older adults.
Dendritic cells are a system of antigen-presenting cells with an essential role in the initiation and development of immune responses against infections or tumors. Their unique capacity to stimulate T cells is being adapted for use in immunotherapy. In this review, we focus on their ability to interact with dead cells and, notably, to present exogenous antigens acquired from them to CD8+ T cells. We also discuss the role of this unique antigen presentation pathway for immunotherapeutic development.
