[Dynamic CT and angio-CT following liver transplantation].

This is a prospective study of the value of dynamic CT with a rapid scan frequency for the control of liver transplants. In 28 patients dynamic CT was performed with table movement and angio-CT without table movement, using a standard contrast bolus. In 97% of cases the central portions of the portal and hepatic veins could be delineated. The peripheral vessels were better demonstrated by angio-CT (79%) than by dynamic studies with table movement (47%). Parenchymal lesions, such as infarcts, and changes round the liver, such as lymphoedema or haematomas, were adequately demonstrated by either method. In addition, angio-CT provided information on abnormalities of perfusion within the transplant.

[Differentiation of malignant liver tumors using rapid dynamic CT].

In this prospective study 39 patients with malignant liver tumours were examined by rapid CT. CT was performed before contrast, as dynamic CT, with and without table movement and, in some patients, images were obtained four hours after contrast injection. Time/density curves of hepato-cellular carcinomas (nine cases) showed more rapid and more intense enhancement of the tumour than of the liver parenchyma, whereas tumours of biliary origin (ten cases) showed a slower and less marked enhancement than surrounding tissue. In addition, it was possible to differentiate hypovascular metastases (18 cases) and hypervascular lesions by means of the time-density curve. Best diagnostic information was obtained by dynamic bolus CT. This method without table movement is best for differentiating focal lesions, whereas dynamic CT with table movement shows the extent and number of tumour lesions.

[The value of 3D-CT in maxillary surgical diagnosis and therapy planning].

In 25 patients in whom oral surgery was required, we examined what additional information can be obtained via three-dimensional CT compared with conventional transverse CT scans. We showed that D-3 CT facilitated the spatial orientation by complex osseous destructions. Better therapy planning and postoperative control was therefore feasible. However, CT diagnosis continued to be based on transverse scans.

[Magnetic resonance tomography in orthotopic liver transplantation].

14 patients with orthotopic liver transplantation were prospectively studied with MR imaging. MR imaging demonstrated a perivascular collar surrounding central portal venous branches in 14 patients. The perivascular collar had low signal intensity on T1-weighted and high signal intensity on T2-weighted images. In 10 of the 14 patients, a perivascular collar was also observed around peripheral portal venous branches. The perivascular collar is probably caused by intraoperative interruption of hepatic lymph vessels and resultant impaired lymph drainage. It is a normal postoperative phenomenon in hepatic transplants. MR imaging is particularly useful in the distinction of postoperative hematoma from other intra- and extrahepatic fluid collections and may provide a noninvasive assessment of liver transplant rejection.

[The Berlin Radiology Communication Project. The initial clinical experiences].

As part of a radiological communication project (RADKOM), two PACS systems of the radiological clinic in the Rudolf Virchow University Clinic in Charlottenburg and Wedding were connected via the BERKOM network of the German post office. Within the radiology departments, all CT and MRT systems were joined through a local network. The first results have shown that the PACS stations are of value for providing uniformity of diagnosis and for radiological conferences. It is evident, however, that the speed of transfer and the architecture within the PACS system would be of crucial importance for their practicability for radiological conferences.

A patient education program to improve adherence rates with antituberculosis drug regimens.

The design, logic, and results of a two-year health education study directed at improving rates of patient adherence to antituberculosis medical regimens are presented. An incentive scheme to reward positive health behaviors plus targeted educational counseling sessions was implemented in a randomized clinical controlled trial. The 205 subjects who participated in the study are categorized according to patients with active tuberculosis (n = 88) or preventive patients with no evidence of active disease (n = 117). Patients in each of these groups were randomly assigned to a special intervention (SI) group or a usual care (UC) control group and were followed monthly throughout their treatment program. While SI patients with active tuberculosis demonstrated higher levels of appointment-keeping behavior and mean percent of medication taken compared to UC patients, no statistically significant differences between the two groups were found. Preventive therapy patients assigned to the SI group, however, were significantly more likely than UC patients to remain in care during their 12-month regimen (64% vs 47%; p = .003). Furthermore, SI patients had significantly higher levels of adherence to their medical regimen compared to UC patients (68% vs 38%; p less than .001). These results demonstrate the positive effects of a structured health education program on the improvement of continuity of care and adherence behavior among patients with tuberculosis.

Couple relationship, birth preparation and pregnancy outcome: a prospective controlled study.

In a prospective, controlled trial we assessed correlations between couple structure, birth preparation and fetal outcome in 158 couples (316 individuals). 88 couples (study group, SG) attended 8 hours of birth preparation classes (BPC). 70 couples served as controls. For couple diagnostic, we used OLSON's two dimensional circumplex model with the FACES III questionnaire, which evaluates the dimensions cohesion and adaptability. In the SG "structured-connected" couples prevailed as compared to the CG, which showed more variability (p less than 0.01). Within the SG, the before-after BPC comparison revealed a tendency toward more functional couple structures. Fetal outcome and complication rate did not differ between groups. In the SG Cesarean section rate correlated positively, premature birth negatively with age and socio-economic status; complication rate in general correlated positively with couple satisfaction. Couple structure should be recognized in routine pregnancy care, BPC ought to be organized in a way that all different types of couples feel invited. Methodologically, the circumplex model proved to be applicable in obstetrics.

Vaginal agenesis and congenital adrenal hyperplasia. Psychosocial sequelae of diagnosis and neovagina formation.

Eleven patients with Mayer-Rokitansky-Küster syndrome (MRK) and 7 patients with adrenogenital syndrome (CVAH) were interviewed and tested pre- and postoperatively. MRK subjects were operated on with a modified Vecchietti procedure, CVAH subjects underwent genitoplastic and clitoroplastic operation. Psychosocial sequelae of and coping with malformation and treatment were assessed with semistructured interviews and the Giessen test. Anatomical and functional results of the vaginoplastic operation were excellent, and sexual satisfaction correlated with coping. 7/11 MRK patients were capable of good to fair adaptation to the malformation, whereas CVAH patients suffered severe distortions of body image. The malformation caused narcissistic damage in all cases. Circumstances of diagnosis, prevailing defense mechanism and consequences for counselling are discussed.

[Liver and thyroid gland. Physiopathologic and clinical relationships].

The liver plays a dominant role in the metabolism of the thyroidal hormones; it is here that the 5' deiodase acts to convert part of T4 to T3. There are eight further circulating iodothyronines: the rT3, mainly derived from T4, appears to be the major inhibitor of T4 and T3. Thus, if rT3 increases, the metabolic effects of T3 and T4 can be quite different. In the course of some chronic systemic diseases (e.g. hepatic cirrhosis) rT3 increases simultaneously with the decrease of T3 levels. Therefore we can describe particular alterations of the thyroidal pattern typical of chronic liver diseases: low T3 syndrome, low T3 and T4 syndrome, high T4 syndrome, mixed forms. T3 and T4 diminish due to inefficient hepatic deiodination and defective hepatocellular uptake. Inefficient hepatic deiodination and defective hepatocellular uptake. T4 levels decrease, most likely because of an inefficient production of thyroid binding globulin, or the action of a peripheral binding inhibitor. During acute liver diseases and primitive biliary cirrhosis, we can observe an increase of T4 and TBG together with an increase of the acute phase proteins. Such complex hormonal mechanisms are not influenced by TSH, which appears normal or inhibited, as the TRH stimulus test is normal. The explication can be found in an enhanced conversion of T4 to T3 in the pituitary gland. The biological and clinical significance of these mechanisms might be that of creating a "protective" state for an organism in a catabolic state by reducing the circulating T3. A relationship has been found between circulating thyroidal hormones levels, particularly the T3, rT3 and rT3/T3 ratio, and the state of hepatic functional insufficiency.(ABSTRACT TRUNCATED AT 250 WORDS)

[Angiographic diagnosis in liver transplantation. I: Evaluation before transplantation].

Our study covers angiographic pretransplant evaluation from 9/88-4/90. 57 out of 73 patients (78%) underwent pretransplant angiography. In all cases aortography, angiographies of the coeliac axis, the superior mesenteric artery, the splenic artery, and portograms were obtained. The arterial studies evaluated the arterial blood supply of the liver, the diameter of the common hepatic artery, aneurysms and a steal effect of the splenic artery. Portography revealed the exact venous collateralisation. Thus angiography remains a valuable procedure for pretransplant evaluation besides ultrasongraphy.

[Double kidney with ectopic ureteral orifice: a case report].

An ectopic ureteric opening is a relatively rare malformation, diagnosed mostly in young age because of typical signs and symptoms and more often found in female patients. The diagnosis of this malformation in adults is unusual and can be difficult because of untypical signs and symptoms, as represented in the following case report.
