[Postero-lateral surgical approach to soft cervical herniated discs. Technical note].

Most ruptured cervical discs are operated on by an anterolateral approach. The posterior approach is an older procedure, nowadays underused because it was associated with a high morbidity, especially in terms of medullary complications. However, posterior approach has evolved in posterolateral route, which is not so devastating and has a very low morbidity rate. It gives excellent functional results when surgery is dedicated to monoradiculopathy from soft posterolateral cervical hernias. It seems appropriate to consider this surgical route as an alternative to anterolateral surgery in these very selected cases. The authors describe and comment the technique.

[Postoperative hydrocephalus. Automatic control of intracranial pressure in patients treated with external drainage].

An experimental analogical device is described; it is able to stabilize the intracranial pressure (ICP) at an adjustable value in subjects exhibiting hydrocephalus. The mean ICP is measured by the intraventricular catheter, using a pressure measurement apparatus of conventional electric design. The result and two practical examples are presented.

[Closed-circuit anesthesia].

Closed circuit anesthesia answers the objections to open and semi-closed systems that are related to high delivery flow rates: pollution, cost, efficiency and the hydration of inhaled gases. But closed circuit anesthesia requires a good understanding of the amount of anesthetic agenda needed, and a convenient method of administering it. It is necessary use a reliable oxygen analyser, a carbon dioxide monitor and a circuit volatile agent analyzer.

[A retrospective multicenter study of severe aneurysms].

From these 196 aneurysms of high grade at the admission 61% die, but 16% have a good recovery. The high mortality of early surgical operations must be moderate by the seriousness of rebleeding in attempting a further surgery.
