[Labor management following previous cesarean section].

Between January 1, 1982 and March 31, 1984, 300 patients were delivered at Graz Gynaecological Clinic who had undergone Caesarean section during their previous delivery or deliveries at the clinic. It was possible to deliver 166 patients (55.3%) of their children vaginally. The rate of spontaneous labour was as high as 91.8% in a group of 61 women who had given birth vaginally once or several times either before or since the Caesarean section. Successful vaginal delivery occurred in 73.5% or 80.6% if the indication for the previous Caesarean section was a breech presentation or a foetal distress, and in 31.7% or 30.6% of those patients whose indication for their previous Caesarean section was a cephalopelvic disproportion or a prolonged labour. The Caesarean section had to be repeated in a total of 134 parturients (44.7%). Of these, the second Caesarean section was indicated primarily (electively) in 29.1%, secondarily in 70.9%. A rupture in the area of the old Caesarean scar occurred in 5 out of 300 cases (1.7%). In four cases this was diagnosed intraoperatively, and once during post partum palpation of the uterine cavity.

[Surgical tumor reduction in extensive ovarian cancer].

The value of radical resected residual tumors and the involvement of the lymph nodes are discussed on a series of 206 patients with ovarian carcinoma. The following results were achieved: macroscopically no tumor 100 patients; residual tumors smaller than 2 cm 14 patients, residual tumors larger than 3 cm 92 patients. Different chemotherapies were also applied. The survival rate of these groups after 2 years was as follows: 80.5%, 85.7%, 38.6%. After 5 years: 58.7%, 0%, 13.1%. In 48 patients after radical surgery and lymphadenectomy who had no post-operative residual tumors the survival rate was 100%, depending on the involvement of the lymph nodes.

Combined Modality Therapy↗

[Lithium carbonate--a preventive agent against leukopenia during cytostatic therapy].

A group of 51 patients treated surgically for ovarian carcinoma was randomly subdivided to test the efficacy of lithium carbonate in the prevention of leucopenia during systemic chemotherapy. In 26 patients receiving sequential lithium carbonate the side effect of myelosuppression was mitigated and the leucocyte count during treatment, which determines the dosage of the cytostatic drugs, was significantly higher in this group. It was, thus, possible to reduce the number of cytostatic courses of treatment in the lithium group in comparison with the treatment courses in the 25 control patients. Lithium therapy had to be discontinued due to side effects in seven patients, which were then excluded from the trial.

Cyclophosphamide↗

[Clinical pathological analysis of peripartum maternal mortality (author's transl)].

All maternal deaths which occurred in relation to labour at the Department of Obstetrics and Gynaecology, University Hospital, Graz, between 1963 and 1978 were reviewed and analysed. 24 mothers died over this 15-year period. The maternal death rate was 0.34 per thousand. In all cases a post mortem examination was performed. The youngest woman was 19, the oldest 42 years old. Most of the decreased mothers were primiparae. The main cause of maternal death was post-partum haemorrhage, followed by infection of the uterus especially after Caesarean section. Three mothers died of liver distrophy. The remaining causes were eclampsia (2 cases); pulmonary artery embolism after Caesarean section (2 cases); irreversible shock from amniotic fluid and air embolism (one case each); uraemia due to glomerulo- or pyelonephritis (one case each); malignant melanoma (one case). It is demonstrated that special risk factors are advanced maternal age, low social status and lack of antenatal care.

Comparison of oral prostaglandin E2 and intravenous oxytocin for induction of labour.

153 oral prostaglandin E2 (PGE2) inductions of labour were compared with 172 inductions with oxytocin. Details of these inductions are presented. After review of the data we assumed that induction of labour with oral PGE2 is less hazardous to mother or infant than with oxytocin. The similarity between the contractions of spontaneous labour and those labours produced by PGE2 is represented by the caesarean section rate in cases of effective induction. The operative delivery rate in the oxytocin-treated group was significantly higher. The need for close observation of the progress of labour during PGE2 inductions is emphasized.

A randomised, double-blind, parallel-group study to compare the efficacy and safety of ondansetron (GR38032F) plus dexamethasone with metoclopramide plus dexamethasone in the prophylaxis of nausea and emesis induced by carboplatin chemotherapy.

A double-blind, parallel-group study in 189 ovarian cancer patients compared the efficacy of ondansetron 8 mg i.v. (OND) and metoclopramide 60 mg i.v. (MET) both in combination with dexamethasone 20 mg i.v. in the prevention of carboplatin-induced emesis. On day 1, complete or major control of emesis (0-2 emetic episodes) was observed in 97% patients from the OND group compared with 74% patients from the MET group (p < 0.001). Similarly, a worst-day analysis over days 1-3 showed complete or major control of emesis in 87% patients (OND) compared wth 66% patients (MET) (p < 0.001). Similar findings in favour of the OND group were observed for nausea grade on day 1 and the worst-day grade during days 1-3. OND was better tolerated than MET. Fewer patients from the OND group (13%) reported adverse events compared with the MET group (21%). Extrapyramidal type symptoms were observed in 6 (6%) patients from the MET group (paraesthesia, involuntary movement of the jaw and tongue, and restlessness), compared with none from the OND group. Ondansetron plus dexamethasone is a highly effective and well-tolerated treatment and is significantly superior to metoclopramide plus dexamethasone in the prevention of carboplatin-induced emesis.
