Neonatal erosions and ulcerations in giant congenital melanocytic nevi.

We report 10 cases of neonatal erosions or ulcerations in giant congenital melanocytic nevi. Histopathologic examination, performed in eight cases, demonstrated benign findings. Clinical follow-up for an average of 3.8 years (range 2 weeks to 8 years) has not revealed the development of cutaneous melanoma. We conclude that erosions or ulcerations in giant melanocytic nevi in neonates are often benign and do not necessarily signify the presence of cutaneous melanoma.

A pilot study of client progress in short-term drug abuse treatment.

This paper is a pilot study of client progress in two short-term (23-day) drug abuse treatment programs. Client progress is defined as those changes in cognitive functioning, emotional well-being, and social support resources that are th proximate goals of drug abuse treatment. In this study, client progress was measured by changes in cognitive functioning, emotional well-being, and social support domains over the course of treatment. This pilot study showed that client progress can be detected in short-term treatment and can be related to indicators of the quantity and quality of treatment. In general, results showed that both relationships and emotional well-being changed over the course of treatment. Cognitive functioning variables did not generally change over time, nor were they related to indicators of treatment quality. Evidence in this pilot study suggests that more attention needs to be given to the relational and emotional aspects of the drug abuse treatment process.

An algorithm for selecting a communication technique with intubated patients.

Communicating with intubated patients in the intensive care unit is a frequent and frustrating situation. In order to communicate effectively, nurses need to select from a variety of communication techniques. The author presents an algorithm which guides the nurse in choosing the most effective communication technique for each patient.

Developing an orientation program for assistive personnel in the ICU.

Assistive personnel are being used in intensive care unit (ICU) settings in response to the demand for more cost-effective nursing care. Nurse managers, educators, and advanced practice nurses must address the unique orientation needs of these workers. A comprehensive, competency-based orientation program for assistive personnel can help the ICU technician be successful. Despite the multitude of assistive personnel being used in the critical care settings, little information is available regarding the development of an orientation program for ICU technicians. This article discusses one ICU's experience in developing an orientation program for their ICU technicians.

Making the most of learning needs assessments.

Identifying learning needs of nurses is an important role of the staff development educator. However, few educators have specific training in the preparation and evaluation of a learning needs assessment. This article describes the assumptions, benefits, and potential barriers when planning a needs assessment survey. Specific strategies to maximize the benefits of the survey are shared. Competent nurses are the cornerstone of professional nursing care, and identifying and responding to learning needs through a needs assessment is a critical step in assuring competence.

Paediatric outreach services.

Paediatric outreach services are provided in Australia and overseas to regional and rural communities. More recently telehealth services have been established to support the delivery of paediatric services and clinical management at distant sites. It is suggested that with the large distances separating centres in Australia that a combination of telehealth clinics and outreach visits may provide the most efficient means of delivering paediatric specialty and subspecialty care to these centres.

Long-term hospitalization of women with high-risk pregnancies. A nurse's viewpoint.

The addition of high-risk pregnancy units to perinatal care systems provides a setting for daily maternal-fetal surveillance during complicated pregnancies. These units permit safe prolongation of pregnancy when the fetus is immature and timely delivery when either the mother's or infant's health is in jeopardy. Hospitalization also encourages maternal rest with its beneficial effects during complicated pregnancy. Nursing functions and goals as developed in the High-risk Pregnancy Unit at Parkland Memorial Hospital in Dallas, Texas, are presented.

The general solution to the Bloch equation with constant rf and relaxation terms: application to saturation and slice selection.

The general solution to the Bloch equation in the rotating frame which includes the effects of relaxation during a constant amplitude, off-resonance rf field has been developed. The solution is used to monitor the transient approach to steady-state saturation levels during off-resonance irradiation applied continuously or in pulses to systems with arbitrary relaxation rates. The time course of the magnetization during amplitude modulated rf pulses is followed with the general solution and the transverse relaxation time dependence of typical slice profiles is examined. The calculations serve to illustrate the generality of the solution. A thorough discussion of various computational concerns is provided.
