Hypertrophic osteoarthropathy as indication of aortic graft infection and aortoenteric fistula.

Diagnosis of an infected bifurcation graft or aortoenteric fistula can be extremely difficult, and delay in diagnosis may lead to the lethal complication of a ruptured aortoenteric fistula. Hypertrophic osteoarthropathy limited to the lower extremities was the initial symptom of aortoenteric fistula in two of our patients with infected aortic bifurcation grafts. Review of the literature discovered six additional patients with a similar diagnosis, indicating its possible value as an early diagnostic sign of aortoenteric fistula.

Benoxaprofen-induced photo-onycholysis.

Benoxaprofen (Opren) is a nonsteroidal anti-inflammatory agent used in the treatment of arthritis. Photosensitivity, including onycholysis, has been reported in the rheumatologic literature. This drug has recently been approved for use in the United States under the name of Oraflex. In order to alert the dermatology community, we report a case of photo-onycholysis which developed in a patient being treated with benoxaprofen and review the data on this drug and its side effects.

Summertime actinic lichenoid eruption (lichen planus actinicus).

A patient with an unusual lichenoid photosensitive eruption is presented. The features differentiating this entity from classic lichen planus, lupus erythematosus, and polymorphous light eruption are discussed. Phototesting confirmed the provocative influence of sunlight. We prefer the name "summertime actinic lichenoid eruption" to that of "lichen planus subtropicus" or "lichen planus actinicus," since this condition is not confined to subtropical countries, and, although light appears to initiate the eruption, its exact relationship to lichen planus is unclear.

Wrist arthrography.

Wrist arthrography is an adjunctive procedure that may be valuable to the clinician. Although it is sensitive to particular abnormalities, it lacks the specificity to completely diagnose a given condition. Information gained by arthrography can be used a supportive evidence, in conjunction with clinical and laboratory findings, to aid in the management of individual patients.

Localized vulvar syringomas.

Syringomas are benign appendageal tumors with differentiation toward the intraepidermal portion of the eccrine sweat duct. A 35-year-old black woman with lesions confined to the labia majora is presented. Syringomas involving the genitalia are rare, and usually the genital tumors are part of a more generalized distribution. A review of English literature showed only four reported cases of syringomas limited to the vulva. Vulvular syringomas present as multiple, bilateral, skin-colored papules over the labia majora that may or may not be pruritic. The lesions should therefore be considered in the differential diagnosis of multiple papular lesions of the vulva and are probably more common than is generally recognized.

Glandular odontogenic cyst.

Two cases of glandular odontogenic cysts are reported. The unique histological features, eg, the intraepithelial glandular structure, papillary processes, and eosinophilic cuboidal and larger granular superficial cells are sufficient to warrant glandular odontogenic cyst as a distinct entity. Electron microscopic examination of the superficial eosinophilic cuboidal cells are suggestive of a process similar to apoptosis. Eroded cortical plates suggest an aggressive behavior.
