The effect of conjugated estrogens on coagulability in menopausal women.

To investigate the effect of conjugated equine estrogens on the coagulability in menopausal women, blood samples were drawn from 69 menopausal females--35 receiving estrogens and 34 receiving no replacement therapy. Using tests for antithrombin and heparin-antithrombin activities, 14.7% of the control patients were found to be hypercoagulable. This is a 289% increase in the incidence of hypercoagulability in the group treated with estrogens. The particular relevance of this finding to menopausal women and the importance of the use of the coagulation profile in managing such patients is discussed.

Intensive care of the pregnant diabetic.

Over the last 2 years at Metropolitan Hospital Medical Center 245 diabetic women have been seen on the obstetric service. This complication of pregnancy comprises 7.9 per cent of 3,076 deliveries, resulting in an over-all uncorrected perinatal mortality rate of 2.9 per cent. We believe that early detection and strict rigid control of diabetes is the key to improved perinatal outcome. This article presents the classification, diagnosis, and management which have led to these results.

Improvement of faculty teaching performance in a department of obstetrics and gynecology by student evaluation.

The primary purpose of a faculty evaluation program is to upgrade the over-all quality of teaching and to identify the strengths and weaknesses of the individual teacher. Effective medical educators can thereby be suitably identified by objective and subjective criteria. Improvement of faculty teaching performance enhances student learning and can improve recruitment into our discipline.

Antithrombin and heparin antithrombin patterns in pre-thrombosis and thrombosis.

An antithrombin assay (AA) and an antithrombin assay modified by the addition of heparin (H-AA) were performed using sera from healthy subjects, patients predisposed to thrombosis, and patients with thromboembolic disease. Characteristic AA, and H-AA patterns were found in each group. Normal controls and four of 39 "pill" patients (10%) had normal AA and H-AA findings (Pattern A). Normal AA, with "decreasing" H-AA (Pattern B) was found in sera of 31 of 39 "pill" patients (80%) and three patients who had non-embolic arterial occlusive disease. Low AA and "decreasing" H-AA (Pattern C) occurred in sera of four "pill"-takers (10%) and one patient who had an arterial embolus. Low AA and low H-AA (Pattern D) developed in sera of eight patients with thrombophlebitis and seven patients with pulmonary embolus. It is concluded that the AA and H-AA assays help to identify thrombosis-prone (Pattern C) and thrombotic (Pattern D) individuals for whom antiplatelet (aspirin; dipyridamole) or anticoagulant therapy might be beneficial.

Conservative surgical management of tubal pregnancy.

In an attempt to maintain optimal fertility in the patient treated for midtubal pregnancy, an investigation of conservative surgical procedure was made. The technic developed utilizes tubal excision, reanastomosis, and a single hydrotubation with Dextran. Tubal patency was maintained without the use of stents that may damage the fimbria. The procedure is compared to other conservative technics and its advantages discused. Thus far 2 patients have undergone this excision and reanastomosis, and both patients successfully established intrauterine gestations within 2 months of returning to normal active coital frequency. Both were in the midtrimester of pregnancy at the time of this report.

Antibodies to spermatozoa. VI. Comparative studies of sperm-agglutinating activity in groups of infertile and fertile women.

In order to evaluate the significance of sperm antibodies in relation to infertility, studies have been made on both infertile and fertile populations. The infertile population consisted of about 150 couples with long-standing infertility, predominantly of the unexplained type. The fertile population consisted of 78 antepartum women and 35 postpartum women. In all these individuals, sperm antibody was sought by two methods of serum testing, namely, the Kibrick (K-B-M) and the F-D methods of sperm agglutination. In the infertile group, the Kibrich test was positive for 23.1 per cent of the women and 9.6 per cent of the men, whereas the F-D test was positive in 16.4 per cent of the women and 5.0 per cent of the men. In contrast to these values, it was found in the fertile group that the Kibrick test was positive in 2.7 per cent of these women and the F-D test was also positive in 2.7 per cent of them. A number of technical details and precautions have also been emphasized. These principles are intended to minimize the occurence of nonspecific and false-positive results.

Endometrial regeneration after voluntary abortion.

Endometrial biopsies were obtained on postabortal days 1 to 36 in 42 women without contraception, in 28 women using oral contraceptives, and in 12 women with intrauterine devices. Tissue samples were examined for day-to-day sequential changes, specifically: a)regenerative, repair, proliferative and secretory activity, b)involution of pregnancy changes, c)inflammation. In the absence of contraception, the endometrium promptly regenerated. The earliest evidence of ovulation was on day 9. One-third of the women ovulated before the first period. Incomplete placental and decidual involution was seen in 45% of patients, but its frequency progressively decreased toward the end of observation. Inflammation seen in 26% was limited to the retained decidua and was, therefore, thought to be a reactive rather than an infectious phenomenon. In women taking oral contraceptives, initial regeneration was also prompt. "On pill" changes resembled the nonpostabortal picture except for a few cases of cystic glandular dilation, or Arias-Stella-like phenomena. Retained decidua and inflammation were more frequent (75% and 68%) than in the noncontraceptive group, and the diminishing trend was not observed. In the intrauterine device group, inflammation was more frequent (83%) than in the other two groups and was not limited to the decidua.
