Cyclosporin in Behçet's disease: results in 16 patients after 24 months of therapy.

The aim of this study was to evaluate the usefulness of cyclosporin in Behçet's disease. Sixteen subjects (10 males and 6 females; mean age 25.2 years) affected by the complete type of Behçet's syndrome received 5 mg/kg/day of cyclosporin for 24 months. A marked improvement of the symptoms was observed after three months of therapy. Within 6 to 12 months of treatment 14 of the 16 patients obtained a complete clinical remission. Biochemical and immunological parameters were controlled periodically to evaluate the clinical response and the possible side-effects. Two patients dropped out of the study because of anaemia and renal dysfunction, which returned to normal when cyclosporin was withdrawn. Our results confirm the efficacy of cyclosporin in the treatment of Behçet's disease.

[The efficacy of nedocromil sodium in light or moderate allergic bronchial asthma].

Inhaled nedocromil sodium was administered 4 times a day for 8 weeks to 34 patients (20 males and 14 females) with mild to moderate allergic asthma to grass pollen. The study was conducted during the pollen season and patients were allowed extra medication only with beta 2 adrenergic drugs. On a diary card patients had to report daily variation of peak expiratory flow rate, severity and number of asthma attacks, severity of cough and use of bronchodilators. At the end of the study we observed a statistically significant improvement of all the parameters considered. No side effects were reported. Nedocromil sodium is therefore a useful and safe drug for the treatment of mild and moderate allergic asthma.

[Rheumatological manifestations following hepatitis B vaccination. A report of 2 clinical cases].

In the literature there are a few occasional case reports of rheumatic manifestations following vaccination. The link between vaccination and musculoskeletal complaints was established on the grounds of the chronological succession between the two events. The occurrence of an individual genetic predisposition has been stressed many times. With regard to immunopathological mechanisms, it has been proposed the vaccination as a trigger of a very autoimmune disease or an immune complex-induced disease. In this paper we describe two females who underwent vaccination against hepatitis B virus. One complained of polyarticular pain that, even if self-limiting, was accompanied by the positivity of RA-test and Waaler-Rose reaction; the other showed migratory arthritis, urticaria and oedema of the glottis and the upper lip, all successfully treated with a short course of corticosteroids. We think these reports are of interest at the time when vaccination against hepatitis B virus is becoming a mass practice.

[Efficacy of salmeterol in patients with allergic bronchial asthma].

Inhaled beta 2-agonists play a central role in the treatment of acute bronchial asthma. A new long-acting beta 2-receptor agonist has recently become available: salmeterol. It improves FEV 1, has an antiinflammatory effect on bronchial airways, provides a strong protection against histamine-and allergen-triggered bronchoconstriction, thus leading to better control of asthmatic symptoms during day and night. The aim of this study was to evaluate the efficacy of salmeterol in 22 patients with allergic bronchial asthma. Our results show that inhaled salmeterol (50 micrograms twice daily) can successfully control diurnal and nocturnal asthmatic attacks and can induce an important increase of PEFR. During the study no further treatment with bronchodilator drugs was necessary.

[Safety of azithromycin in patients allergic to penicillin and cephalosporin].

We have evaluated the safety of a new antibiotic, azithromicin, in 48 patients with allergy to penicillin and/or to cephalosporin. Diagnosis of allergy was based on clinical history, skin test and detection of serum specific IgE. The most common symptoms were urticaria, oedema, pruritus, oral aphthosis. Azithromicin was administered at the increasing dosage of 100-200-300 and 400 mg every 2 days. Our patients did not show any reaction to azithromicin. This antibiotic is therefore a valid alternative to penicillin and/or cephalosporin in patients allergic to these two drugs.

[The efficacy of long-term specific immunotherapy for Dermatophagoides pteronyssinus in patients with atopic dermatitis].

The aim of this study was to evaluate the efficacy of hyposensitization therapy with Dermatophagoides pteronyssinus vaccine in patients with moderate and severe atopic dermatitis and sensitization to house mite dust. We studied 32 patients who showed strongly positive Skin Prick Test and high levels of circulating specific IgE to Dermatophagoides pteronyssinus. All the patients were treated with sodium alginated dialyzed allergenic extract (Bayropharm, Milan), realized by increasing progressively subcutaneous injections for three years. Erythema and lichenification were monitored. The Skin Prick Test positivity and serum level of specific and total IgE were measured before and after the treatment. We divided our subjects in two groups on the basis of presence or absence of respiratory symptoms such as rhinitis and asthma. All the patients showed substantial improvement of the cutaneous lesions and respiratory symptoms. Analysis of specific circulating IgE and Skin Prick Test showed significant tendency to normalization. The total IgE serum level showed no significant difference. Our results showed a definite benefit from a DPT hyposensitization regime in patients with atopic dermatitis and sensibilization to house mite dust.

[Current trends in the specific immunotherapy for allergic syndromes due to Dermatophagoides pteronyssinus].

House dust mites of the species Dermatophagoides pteronyssinus are among the most prevalent perennial allergens. Measurement of allergen exposure is so crucial to establish the natural history of the allergic reaction. The most common clinical manifestations induced by Dermatophagoides are perennial asthma and perennial rhinitis. Recently there are a number of studies indicating that a high degree of exposure to house dust mites may represent a risk factor for severe disease in patients with atopic dermatitis. The diagnosis was performed by in vivo and in vitro tests. Nowadays the treatments regard allergen eradication and specific immunotherapy. Mite specific immunotherapy is only effective under optimal conditions, including an appropriate selection of patients and the use of high-quality standardized extracts. The specific immunotherapy received by subcutaneous injection is the one mostly used until now and the results were really satisfactory, even if its evaluation has been made on clinical data or on bronchial challenges. Immunologic changes were found only when using same extracts. The incidence of systemic reactions during the immunotherapy was low. Recently local immunotherapy was used and it has been shown to be significant by safety, but further clinical investigations are needed to clarify the clinical efficacy in direct comparison with traditional parenteral immunotherapy.

[The efficacy of cyclosporin for topical use in oral lichen planus].

Oral lichen planus is a disease characterized by long symptomatic phases unresponsive to the usual therapy. Many groups have used different drugs in the treatment of lichen planus: topically applied retinoic acid, temarotene, antimycotic agents corticosteroids and immunosuppressive agents, with unsatisfactory results. Recently it has been suggested that topical cyclosporine might improve the lesions of oral lichen planus. The aim of this study was to evaluate the usefulness of this therapy in our patients. Fourteen patients, 6 males and 8 females, mean age 47 years, with oral lichen planus were enrolled in the study. All the patients were instructed to swish 5 ml of solution (500 mg) of cyclosporine in the mouth three times a day for three months. Clinical evaluation was performed before therapy and every two weeks afterwards. At each visit serum levels of cyclosporine, creatinine, total and direct bilirubin and complete blood count were performed. No side effects or blood test alterations were detected in any patient and cyclosporine serum level was always undetectable. Symptoms and oral lesions had a beneficial effect already after one month of therapy. Our results confirm that cyclosporine is useful in the treatment of oral lichen planus.

[Anaphylaxis induced by exertion].

Exercise induced anaphylaxis (EIA) is a relatively new syndrome described in 1980. It is associated with different kinds of exercise, although jogging is the most frequently reported. The clinical manifestations progress from pruritus, erythema and urticaria to some combination of cutaneous angioedema, gastrointestinal and laryngeal symptoms and signs of angioedema and vascular collapse. In the full-blown phase a differential diagnosis must be done with the following syndromes: exercise-induced asthma, idiopathic anaphylaxis, cardiac arrhythmias, carcinoid syndrome. An elevated serum histamine level during experimentally-induced attacks and cutaneous degranulation of mast cells after attacks proved a mast cell participation in the pathogenesis of the syndrome. As predisposing factors, a specific or even aspecific sensitivity to food has been reported and such cases are called "food-dependent EIA". Another precipitating factor includes drug intake; moreover a familial tendency has been reported in some studies. Although the prognosis of this syndrome is not well defined, a reduction of attacks occurs in 45% of patients by means of elimination diets and behavioural changes. Treatment of attacks should include all the manoeuvres efficacious in the management of conventional anaphylactic syndrome, including the epinephrine administration. Prevention of attacks may be achieved by limitation of the exercise program or interruption of the program at the appearance of the first premonitory symptoms. The use of H1 antihistamine-receptor antagonists in maintenance therapy seems to be useful, although no controlled data are available.

[Effectiveness of loratadine vs. placebo in the treatment of urticaria-angioedema syndrome in patients with food allergy].

Loratadine is a new, highly selective, non sedating, H 1-receptor antagonist, without central nervous system activity. In a randomized double-blind, crossover study, we evaluated the effects of loratadine and placebo administered once daily in 184 food intolerant patients affected by urticaria-angioedema. The difference between loratadine and placebo treatment was significant in relieving symptoms. Adverse reactions reported in the treatment were mild, in fact somnolence was reported by 3.4%, dry mouth by 2.2% of patients.

[Symmetric multiple lipomatosis with Charcot's joint and neuropathic ulcer. Description of a clinical case].

Multiple symmetric lipomatosis (MSL) (or Madelung's disease or Launois-Bensaude syndrome) is a rare inherited disease clinically characterized by a massive development of large symmetric unencapsulated lipomas on the subcutaneous tissue of face, neck, trunk and arms, resulting in a grotesque aspect of the patient. Less frequently the accumulation of excessive fatty tissue can spread deeply to the superficial fascia. Peripheral neuropathy, macrocytic anemia and chronic hepatopathy have been reported to cohesist. Macrocytic anaemia and chronic hepatopathy are probably secondary to high alcohol consumption, that is frequently associated. MSL, that was first described by Sir Benjamin Brodie in 1846, affects mainly the men (ratio man/woman 30:1), with an incidence in Mediterranean area of 1:25.000 men; the ages at onset range from 20 to 50 years. It is not known yet the pathway of inheritance and the molecular basis of the genetic defect responsible for the development of fat accumulation. It has been postulated a defective lipolytic response to catecholamines; this altered response could be due to an abnormal amount or a defective function of Gs-protein, the coupler between beta-adrenergic receptors on the surface of adipocytes and adenylate cyclase, or, alternatively, the defect could be in the catalytic unit of adenylate cyclase. The number and function of alpha- and beta-adrenergic receptors and the lipolytic response to cAMP (the second messenger) are normal. Recently it has been hypothesized that the defective lipolysis is due to a disorder in the mitochondria of brown fat, whose distribution is similar to the peculiar position of the lipomas in this pathology; the brown fat, unlike white adipose tissue, has abundant mitochondria. The alcohol abuse, frequently present in these subjects, might facilitate the clinical expression of the molecular defect. The therapy of lipomas is essentially surgical, but this approach is not easy, because the lipomas are not capsulated and extremely vascularized. Moreover the surgical excision is not always a successful treatment for the lipomas as they frequently recurrent after a short period from the exeresis. In this report we describe a 59-years old white man, alcohol abuser, with a typical clinical picture of MSL, developed when he was 37 years old. The patient presented multiple lipomas around the shoulders, face, neck and arms, that had been surgically excised eight times. Magnetic resonance imaging showed the presence of fat deposits also in the mediastinum, that caused a tracheal compression. Hepatic cirrhosis and serious side effects from peripheral neuropathy, represented by Charcot's joint and neuropathic ulcer on the sole foot were observed.(ABSTRACT TRUNCATED AT 400 WORDS)

[Native anti-DNA antibodies and the synovial fluid. A report of a problematic clinical case].

The authors report the unusual case of a 30-years-old woman affected by bilateral gonarthritis with anti-ds DNA antibodies at higher level in synovial fluid than in serum. This finding, and the presence of an intensive infiltration of polyclonal plasma cells in the synovial membrane, studied by biopsy, lead to suppose a local production of autoantibodies with a probable pathogenetic role.

[Diet manipulations and nickel sulphate-induced contact dermatitis evaluated in 24 cases].

The wide distribution of nickel in the environment as well as in animal and vegetable foods has led a large number of authors to investigate the oral absorption of nickel. The aim of this study was to evaluate the effect of a free-nickel diet in 24 patients affected by nickel sulphate-induced contract dermatitis. The diagnosis was made on the basis of their past history and of Patch test. A complete remission of symptoms was obtained with a nickel-free diet for two months; Patch test became partial negative in 50% of patients and completely negative in 33%. In conclusion our results confirm the relationship between nickel-induced dermatitis and oral nickel absorption and suggest that diet manipulations can be useful in cases of refractory skin manifestations.

Controlled study of oxatomide vs disodium chromoglycate for treating adverse reactions to food.

A controlled, cross-over trial was carried out to compare the efficacy and safety of oxatomide vs disodium chromoglycate (DSCG) for treating food allergy and intolerance. Twenty patients (15F, 5M; mean age 36.6 years), with chronic urticaria (twelve patients) or eczema (eight patients) caused by food allergy (ten cases) or food intolerance (ten cases), were treated with oxatomide (60 mg/day in a single evening administration) and with DSCG (2000 mg/day) for six weeks. The two treatments were separated by a 3-week wash-out period. All the patients completed the trial. During the treatment, both drugs succeeded in controlling the symptoms. With oxatomide, the wheals totally disappeared from 75% of the patients (p = 0.00135), the eczematous lesions disappeared from 64% (p = 0.056), and the itching from 70% (p = 0.00012); the figures for DSCG were 33%, 50% (p = 0.038) and 50%, respectively. Both drugs were well tolerated and there was no need to discontinue the treatment of any of the patients.

[Food allergy and asthma].

The prevalence of asthma in patients suffering from food allergy or food intolerance had been studied in 1339 subjects. Of them, 165 patients (12.8%) were affected by asthma. However only 77 (5.7%) had positive prick tests for food antigens and 52 of these have a significant benefit when a suitable oligoallergenic diet was introduced. 35 out of patients were positively tested with a double-blind food challenge. The most frequent food antigen involved were milk and its derivatives and cereals, but some importance have also the additives to the food. In conclusion, in our patients suffering from food allergy or food intolerance asthma was present in 12.8% of the cases, but in only 77 (5.7%) the symptoms might be due to diet. When tested with oligoallergenic diet or with food challenge the percentage of positivity fall to 3.9% and 2.6% respectively.

[Myasthenia gravis, palindromic rheumatism and systemic lupus erythematosus. A clinical case report].

We describe a case of a 59-year-old white woman, that first developed a myasthenia gravis picture, then palindromic rheumatism and, in the end, systemic lupus erythematosus. We have searched in the literature the common features and the differences between myasthenia gravis and systemic lupus erythematosus. In this report we emphasize the possible connection between palindromic rheumatism and autoimmune diseases.
