[Hospital infections: the problems and ways to resolve them].

The authors present the critical analysis of the existing situation in the spread of purulent septic infections in surgical hospitals of the USSR and the state of epidemiological surveillance. Considering this situation to be highly unfavorable, the authors propose a number of urgent organizational measures (the inclusion of hospital epidemiologists into the medical staff of large clinics, the system of training in this problem). The article deals with the specific features of the epidemiological process in purulent septic infections and the main principles of the organization of the epidemiological surveillance system for this group of infections.

[The mechanism of the development of the epidemic process in Sonne dysentery].

An epidemic outbreak of Sonne dysentery has been studied. The data of epidemiological monitoring before and after the epidemic have been analyzed. The real prognostication value of controlling the biological properties of Shigella sonnei and the increase of their intrapopulation heterogeneity at the period of the activation of the epidemic process of Sonne dysentery has been established.

[Characteristics of the epidemic process in pyoseptic infections in a heart surgery hospital].

The authors conducted an epidemiologic analysis of hospital pyoseptic infections on the basis of study of the outcomes of 1,517 operations on the heart and the results of bacteriologic tests. A high frequency of secondary infectious endocarditis in acquired heart diseases was revealed, which allows operations in this group of patients to be related to the category of "conventionally clean" and shows that the proper antiseptic regimen must be organized during the operation without fail. The valves of the heart in 59.3% of patients with acquired heart diseases are affected with the causative agents of hospital pyoseptic infections. The epidemic process is characterized by high intensity of manifestations, dependence on the degree of aggression and invasion of the therapeutic and diagnostic process, the type of the operative intervention, and polymorphism of the clinical manifestations and the etiological structure. The process is of the intermittent dynamic type and is marked by a tendency towards growth of morbidity and mortality with prevalence of an endogenous mechanism of contamination. The main sources and routes of transmission of infection are shown.

[Experimental study of the interaction of phages and bacteria in the environment].

For the first time the possibility of the interaction of phages and bacterial cells in the environment, consisting in the adsorption and subsequent replication of viral particles, has been shown. The data obtained in this investigation provide grounds for using phages in the environment. Experiments with Escherichia coli M-17 and phage FM17, carried out on experimental models, have demonstrated that the use of phages in the environment completely prevents the realization of the transfer of enteral infections through everyday contacts and the contamination of children and adults under hospital conditions.

[Epidemiological diagnosis of suppurative-septic hospital infections in surgery].

The authors have analyzed the results of epidemiologic diagnosis of suppurative-septic hospital infections after surgery, that helped them develop a preventive system permitting a decrease of the incidence rate of suppurative-septic infections in surgery 1.8-fold within a year. The main features of the epidemic process in suppurative-septic infections is described. A total systems approach to epidemiological diagnosis is necessitated.

[Effect of the heterogeneity of persons vaccinated, with respect to the state of their immune system, on the intensity of the immune response].

Prior to the immunization of children aged 3-7 years with parotitis vaccine the state of their immune system was evaluated by the determination of the concentration of IgM, IgG and IgA, the ratio and absolute numbers of T- and B-lymphocytes, the intensity of the blast transformation of lymphocytes. This study revealed that the group of immunized children was essentially heterogeneous with respect to the state of their immune system: in 79.9% of the children all immunological characteristics were normal, in 20.1% of the children some deviations from the characteristics considered to be normal for their age (gammopathy, eosinophilia, the deficiency of T- and B-lymphocytes, a decrease in the intensity of their blast transformation, the leftward shift of the leukocytic formula) were noted. The intensity of humoral immune response in children with deviations from normal characteristics peculiar to their age was significantly lower than in children with normal immunological characteristics. On the basis of these results, the immunological rehabilitation of the risk group among the children to be immunized is recommended.

[New tasks in an old discipline (the teaching of epidemiology in a therapeutics department today)].

To bring the system of training in epidemiology at the therapeutic department in full accordance with the requirements for professional skill, future physicians should be trained in the epidemiological (population) method of investigation. Effective primary prophylaxis is possible only when future physicians learn to evaluate the state of public health to reveal risk factors and risk groups.

[The theory and practice of an epidemic control service for the population].

The data on the epidemic-control medical care of the population of the Kuznetsk coal fields and on the creation of the specialized system of the epidemiological surveillance of influenza, measles, viral hepatitis, hospital infections are presented. The possibility of using the theory of the self-regulation of the epidemic process in practical work is discussed.
