Bilateral acoustic schwannoma: post-operative hearing in the contralateral ear.

A patient with neurofibromatosis two (NF2) presented with bilateral acoustic neuroma. Pre-operative audiometry revealed a dead right ear and severe left-sided sensorineural hearing loss. Following surgical removal of the larger right acoustic neuroma we have documented a sensorineural improvement of 45 dB in the contralateral ear on pure tone audiometry, which as far as the authors are aware has not previously been described.

Hearing aids--a case for review.

An analysis of a series of 100 patients aged 60 years or over, referred by their General Practitioner to the Department of Otolaryngology, Addenbrooke's Hospital, specifically for the provision of a hearing aid, is presented. Forty-six per cent of patients failed to satisfy the criteria suggested by the Technicians, Therapists and Scientists in Audiology as a screening for treatable pathology. This study has shown that the existing referral pattern of general practitioner to consultant otolaryngologist for the provision of a hearing aid reflects the highest quality of care for the patient. Direct referral or open access of patients for hearing aid fitting may risk failure to diagnose early and treatable conditions.

Reliability of the House and Brackmann grading system for facial palsy.

The House and Brackmann grading system has been recommended as a universal standard for assessing the degree of facial palsy. This study examined the inter-observer reliability of this system. Three observers assigned a grade to each patient, examined independently, on the same day. Forty patients with a unilateral facial palsy of varying aetiology and severity were assessed. Of the 120 judgements, eight were in dispute, by a maximum of one grade, giving an inter-observer reliability of 93 per cent. We conclude that the House and Brackmann grading system is a simple and robust method of assessing facial function.

Unterberger's stepping test in acoustic neuroma.

A prospective study of 100 patients with a unilateral, subsequently histologically proven, acoustic neuroma seen in the Department of Otoneurosurgery at Addenbrooke's Hospital between 1983 and 1988, demonstrated that Unterberger's Stepping test achieved a higher sensitivity (71 per cent) than the Romberg test (39 per cent). The discrepancy between tests is at a maximum for small acoustic neuromas, and therefore the Stepping test may help in the earlier detection of vestibular abnormalities.

Melanoma metastatic to the head and neck.

A 42-year-old man presented with a two-week history of right-sided otalgia, deafness and nocturnal tinnitus. Examination revealed a black mass on the posterior meatal wall. Two years before presentation, he had a malignant melanoma removed from the left hip region. Biopsy of the lesion in the posterior canal wall and another which had developed within two weeks on the anterior wall, confirmed metastatic disease. Treatment with radiotherapy and later chemotherapy were unsuccessful and the patient died 15 months later.

An unusual presentation of metastatic squamous carcinoma of tongue.

Distant metastases from head and neck carcinoma are becoming more common, with lung, bone and liver being the most frequent sites. We present an unusual case of a metastasis from a tongue primary presenting with severe abdominal pain and pseudo large bowel obstruction.
