Pregnant adolescents' perceptions of their babies before and after realtime ultrasound.

1. Research has shown that adolescent mothers both overestimate and underestimate their infant's abilities. 2. Results indicated that the majority of the adolescents exhibited a different perception of their fetus postultrasound, especially in their babies' kinesthetic characteristics. 3. Nurses must develop visual aids that are attractive, bright in color, contain more pictures than words, and are kinesthetic in nature. These visual instructional processes regarding fetal characteristics including movement, heart beating, and developmental capabilities should be incorporated in pregnant teen educational programs.

Child care beliefs and practices of Hispanic mothers.

Even though Hispanics are the fastest growing minority in the United States, they face difficulties in accessing the health care system because of perceived health care needs, culture, income, language and lack of health insurance benefits. As a result, children of Hispanic parents suffering from such ailments as viral and bacterial infections, upper respiratory infections and gastroenteritis may not be treated at all or may be treated in the home. It is unclear as to how much a role traditional, cultural practices continue to play in their health care and how long medical treatment may be delayed. The purposes of this exploratory pilot study were to examine the types of cultural health care provided by Mexican-American mothers to their ill children at home, the extent of the child's illness and the time frame involved that determined the mother's need to seek medical attention. The mothers' use of lay healers and/or folk remedies prior to seeking medical attention for their child was also investigated. Implications and recommendations for culturally-relevant and culturally-competent care, and sensitive health education are presented.

Clinical pathways in subacute care settings.

There is a dramatic growth of subacute care settings because of mandated shorter hospitalization and the growing older adult population. The cost parameters of a fixed reimbursement model of payment led to development of clinical pathways to enhance quality care, maximize resources and decrease costs, facilitate the patient's recovery and reduce length of stay. Seven essential elements are described that comprise the comprehensive framework, along with suggested criteria.

Socialization into nursing: a developmental approach.

This paper explores the process of socialization into the profession of nursing through developmental models and examines the differences in the degree of professional socialization among nursing students at three academic levels. The conceptual framework around which this study was organized was Richard Hall's concept of professional socialization. The descriptive, comparative cross-sectional study utilized Hall's professional inventory as modified by Snizek to ascertain degree and differences of professional socialization in the three groups. The total mean score indicated a moderate degree of professional socialization for the sample as a whole. The findings also indicated that the sophomore nursing students had a statistically significantly higher degree of professional socialization than either the senior or masters nursing students. Implications and recommendations for nursing education, practice, and future research are discussed.

Critical thinking skills and dispositions of baccalaureate nursing students--a conceptual model for evaluation.

To date, little has been written on specifically what critical thinking is and what the scores reported on critical thinking instruments actually represent. This lack of specificity provided the impetus to examine critical thinking skills and dispositions of baccalaureate nursing students, to ascertain whether or not a significant difference exists between academic levels, and to ascertain, using the California Critical Thinking Skills Test (CCTST) and the California Critical Thinking Disposition Inventory (CCTDI), whether a relationship exists between nursing students' critical thinking skills and their critical thinking dispositions. The conceptualization of critical thinking consisting of two dimensions, cognitive skills and affective dispositions, developed by a panel of experts comprised the conceptual framework for this study. A cross-sectional, descriptive, comparative, and correlational study was undertaken in which two instruments, CCTST, Form A, and the CCTDI were administered to a convenience sample of nursing students representing five academic levels. The sample in the statistical analyses comprised 328 students. Results indicated that the total critical thinking mean score for students at each level reflected percentile ranking ranging between 48 and 65 per cent. The mean score for the critical thinking subscale, inference, had the lowest percentile rankings, ranging between 37 and 55 per cent. The analysis of variance (ANOVA) statistical analysis indicated that students at the junior I level had the highest critical thinking mean scores, and students at the sophomore II level attained the lowest scores, reflecting a significant difference (P < or = .05). ANOVA also indicated a significant difference in the total critical thinking disposition mean scores between students at the junior I and senior I and II levels and those at the sophomore II level (P < or = .0000). Results indicated weak truth-seeking disposition scores among students at all levels. Pearson Product Moment Correlation Coefficient yielded a significant positive relationship between critical thinking skills and critical thinking dispositions (P < or = .01). The conceptual model, as well as its relevancy to the professional standards of Essentials for College and University Education for Professional Nursing (AACN, 1986), is presented. Recommendations are discussed in regard to nursing education, curriculum, and research.

A powerful method to strengthen preferred ways of learning and broaden learning styles to meet changing circumstances is described.

Unveiling the mystique of gender dysphoria syndrome.

There are common myths surrounding the condition of gender dysphoria and individuals seeking therapy. The diagnosis of gender dysphoria syndrome is applied to individuals who feel ill at ease with their biological body and either seek hormonal reassignment therapy or hormonal therapy and sex reassignment surgery. The author discusses standards of care, diagnostic criteria, physiological and psychological changes incurred through hormonal therapy are introduced as well as surgical techniques. By assisting individuals with gender dysphoria syndrome to enhance the integration of their new gender role into their self-identity, nurses are in an excellent position to facilitate improvements in their overall health status as well.
