Long-term followup of young patients with stage A adenocarcinoma of the prostate.

A total of 23 men less than 60 years old with stage A adenocarcinoma of the prostate who were managed expectantly (that is untreated) and were at risk for 10 to 25 years form the basis of this study. The original amount of tissue obtained at transurethral resection, number of chips involved and examined, and tumor grade (Mayo grades 1 to 4) were recorded and compared in an in-depth analysis whereby the entire tissue removed was examined without knowledge of previous grading attempts. On the basis of volume estimation of the amount of cancer present 8 patients were reclassified as having stage A2 disease. Of these 8 patients 2 had disease progression and 1 died of metastatic adenocarcinoma of the prostate. At review 15 patients remained with stage A1 disease and 4 had disease progression (3 systemically and 1 locally) an average of 10.2 years after diagnosis. Because of longer life expectancy the young patient with stage A1 disease is at increased risk for local and/or systemic disease progression. Therefore, when incidental adenocarcinoma of the prostate is found in young patients consideration should be given to examination of all tissue resected, and to repeat transurethral resection and biopsy to ensure accurate staging. Lifelong careful followup is mandatory not only to detect local recurrence owing to heterogeneous adenocarcinoma of the prostate but also to detect a possible secondary clinical lesion.

Renal vein and inferior vena cava tumor thrombus in renal cell carcinoma: CT, US, MRI and venacavography.

Renal cell carcinoma has a propensity to extend as tumor thrombus into the renal vein and inferior vena cava (IVC). The preoperative assessment for the presence and extent of renal vein and IVC tumor thrombus is important for planning appropriate surgical resection. Imaging procedures [CT, ultrasound (US), MR, venacavography] were correlated with surgical findings and pathology in 431 consecutive patients who had a radical nephrectomy for renal cell carcinoma. Ninety-nine (23%) patients had tumor thrombus extending at least into the main renal vein. Of these, 29 had tumor thrombus extending within the IVC. Patients were classified into two groups based on the surgical extent of tumor thrombus. Group A patients had no tumor thrombus or had tumor thrombus only in the renal vein proximal to the site of surgical ligation. Group B patients had tumor thrombus that extended to or beyond the distal renal vein at the site of surgical ligation. Forty-one patients had Group B tumor thrombus. Group B tumor thrombus was not seen in a renal cell carcinoma that was smaller than 4.5 cm. The sensitivity of CT for detecting Group B tumor thrombus was 79% and that of US was 68%. However, a much higher percentage of US examinations were technically indeterminate. In the patients who had either MR or venacavography, both imaging procedures were 100% sensitive for detecting group B tumor thrombus. Magnetic resonance imaging and venacavography appear to be the most sensitive means of identifying tumor thrombus.

Local microwave hyperthermia as a treatment alternative for benign prostatic hyperplasia.

It appears that the technology for local microwave application of heat to the prostate for the management of benign prostatic hyperplasia has arrived. There are a number of issues to be resolved in the coming years that will determine the role this modality will play in the overall management of men with benign prostatic hyperplasia. These issues include: transurethral versus transrectal route, hyperthermia (42 degrees C to 44 degrees C) versus thermotherapy (greater than 45 degrees C), and a proper assessment as to whether the technique is really efficacious, given the known placebo response in all studies currently available. The results with the transrectal route appear to improve patients' symptoms objectively and subjectively, without causing irreversible tissue effects. Thus, its action has been likened to alpha blockade. But, it appears that the transrectal approach is relatively inefficient because of a significant loss in microwave power with rectal cooling. A probe placed transurethrally can accurately and easily deliver the intended power to the center of the prostate, where theoretically it has its greatest effect on both the dynamic and static components of outlet obstruction. Currently, the transurethral devices described by Sapozink and Devonec will produce histologic necrosis. The theoretical value of combining urethral heating with cooling is that it will allow treatments of greater power deeper in the prostate adenoma, but the greatest advantage over transurethral heating without cooling may be in the ability to effect a response in a single session. Finally, the placebo response is a well known phenomenon seen in all drug trials conducted for the management of benign prostatic hyperplasia.(ABSTRACT TRUNCATED AT 250 WORDS)

A giant left renal cyst presenting as obesity: a unique presentation.

OBJECTIVE: Benign renal cysts are commonly found in adult patients. However, giant renal cysts measuring more than 15 cm in greatest diameter or containing more than 1500 cc of fluid are rarely seen. A giant renal cyst presenting as obesity, a unique form of presentation, is described herein. METHODS/RESULTS: We report the unique case of 78-year-old male who was diagnosed with a 25 cm left renal cyst after presenting solely with diffuse progressive abdominal distention. He had no other signs or symptoms and had been followed at our institution for seven years with a diagnosis of obesity. The patient was effectively treated by open renal cyst decortication. CONCLUSIONS: Giant renal cyst measuring > or = 15 cm or containing more than 1500 cc of fluid rarely occur. This case is unique in that unlike other adult patients, this patient presented with no sign or symptom other than diffuse, nonlateralizing, abdominal distension.
