[Basic and clinical evaluation of an immunoradiometric competitive inhibition assay for 2----6 sialyl Lewis a antigen--2. Evaluation of clinical significance].

The clinical significance of serum 2----6 sialyl Lewisa antigen was evaluated using "SLA 2----6 Otsuka" kits. Results indicated that the antigen was frequently elevated in sera obtained from patients with various cancers, including pancreas (73%), liver (67%), bilialy tract (66%), uterus (35%), and stomach (33%). Among other tumor markers examined, CA 19-9 (2----3 sialyl Lewisa) had a very similar cancer spectrum as 2----6 sialyl Lewisa. In the sera of patients with malignant disorders of digestive and respiratory organs, including stomach, intestine, pancreas, biliary tract and lung, the serum levels of CA19-9 tended to be higher than those of 2----6 sialyl Lewisa, usually exceeded those of CA19-9. This suggests that the 2----6 sialylation of Lewisa antigen is equally observed in malignant and non-malignant diseases, while the 2----3 sialylation is relatively specific to cancers. As a result, the ratio of the two antigens, CA 19-9: 2----6 sialyl Lewisa antigen ratio, exceeded 2.0 in most of the sera obtained from patients with malignancy, whereas the ratio was below 2.0 in most of patients with corresponding non-malignant diseases of those organs. The determination of the ratio may be clinically useful in the differential diagnosis of the malignant and non-malignant diseases in those organs.

[Clinical studies and sputum levels of ceftriaxone once daily administration in respiratory tract infection].

Ceftriaxone (CTRX), a new cephalosporin, was investigated by once daily administration for its clinical efficacy and safety on respiratory tract infections. The results obtained are summarized as follows: 1. Clinical responses to CTRX of a total of 39 cases with respiratory tract infections were excellent in 12 cases, good in 23, fair in 3, poor in 1 with an efficacy rate of 89.7%. Against acute bronchitis, lung abscess, bronchiectasis, chronic bronchitis and obstructive pneumonia, efficacy rates were 100%. 2. Serum levels and urinary excretion rates of CTRX were investigated in 2 cases after intravenous drip infusion of the drug at doses of 1 g and 2 g, respectively. Although urinary excretion rate tended to decrease with the deterioration of renal functions, prolongation of serum half-life was slight in those patients with normal liver function. In 1 case, it remained at 1.9 micrograms/ml at 12 hours and in another at 0.9 microgram/ml at 22 hours in sputum. According to the results, it appears that once daily administration of CTRX is effective and well tolerated in patients with acute respiratory infections.

[Right ventricular regional wall motion and function after tetralogy repair].

The right ventricular (RV) function is very important after correction of tetralogy of Fallot (TOF) similarly to left ventricular (LV) function. The influence to RV contraction caused by RV outflow tract patch after repair of TOF is serious. There are a few literatures about contraction mode of RV but we can't see any reports about regional RV wall motion before and after surgery of TOF. This paper is the first report about RV regional wall motion and function after 20 cases of tetralogy repair. We would emphasize the following points. 1) Regional wall motion of RV at antero-basal and anterolateral is getting worse but is improved at anteroapical of RV. 2) Appropriate rate of RV systolic pressure to LV systolic pressure just after weaning from cardiopulmonary bypass and rate of patch length to RV length is below 0.5 and 0.4 respectively. 3) There is positive correlation between RV ejection fraction and wall motion at anteroapical of RV.

[A case of mediastinal undifferentiated carcinoma].

A 33-year-old man showing the right anterior mediastinal mass on his chest x-ray film admitted to our hospital. At thoracotomy extirpation of the cystic tumor was performed. The tumor was 7 x 7 x 5 cm in size and contained coffee-coloured fluid. Histological examination confirmed undifferentiated carcinoma. Postoperatively he was treated with 60Co 50 Gy and chemotherapy. He has been well 4 years after operation.

Micronucleus test in mice fed on an irradiated diet.

A mutagenicity study was carried out in mice fed on a gamma-irradiated diet. As an indicator of mutagenic activity, we observed an incidence of micronuclei in erythrocytes. The average body weight of the mice fed on the diet irradiated to dose range of 400-1,000 kGy decreased, and the mice fed on the 800-1,000 kGy-irradiated diet died during the period from 8 to 14 days after the start of feeding. On the other hand, when the mutagenic activity of the irradiated diet was tested by observing occurrence of micronucleus in erythrocytes, no significant increase was recognized. These results indicated that the irradiated diet had no mutagenic activity, even though it possessed a toxic effect on the growth of mice.

[A case showing a free substance in the thoracic cavity detected during surgery in lung cancer considered to the precursor of the so-called "intrathoracic calculus"].

A 56-year-old male showed a pearl-like free substance in the thoracic cavity during operation for pulmonary cancer. In the thoracic cavity, apart from a small quantity of fluid, no particularly abnormal finding, such as calcification, pleural plaque, etc. was seen in the parietal pleura. Further, the pulmonary pleura showed no abnormal findings, except for pleural indentation at the site of the tumor. On histopathological examination of the free substance, it was spherical, with fat forming the nucleus, and covered by fibrous and hyaline tissue. The fat forming the nucleus of the free substance was assumed to have been derived from a lipoma or fat in the thoracic cavity or on the pulmonary pleura which had become detached. This free substance is believed to be the precursor of an "intrathoracic calculus", as designated by Takiguchi et al. This is very rare and is believed to be only the second such case in Japanese or foreign literature.

ESR and spin-trapping study of free radicals in gamma-irradiated solid lysozyme.

Free radicals produced by gamma-irradiation of solid lysozyme were investigated by a technique combining ESR, spin-trapping and enzymatic digestion. MNP and DMPO were used as spin-trapping reagents. The solid lysozyme was first gamma-irradiated and then dissolved in an aqueous solution containing the spin-trapping reagent to stabilize free radicals. The spin adducts of lysozyme were digested to oligopeptides to get ESR spectra having a well-resolved hyperfine structure. The ESR spectra obtained showed that carbon-centered radicals, -CH-, at the side chains of amino acids, and thiyl radicals, -CH2-S., at disulfide bridges were produced in gamma-irradiated solid lysozyme.

Metabolic effects of 3'-deoxyadenosine (cordycepin) and 2-halo-3'-deoxyadenosine on repair of X-ray-induced potentially lethal damage in Chinese hamster V79 cells.

Using cultured Chinese hamster V79 cells, an attempt has been made to examine the phosphorylation of cordycepin and its 2-halo derivatives (2-chloro-3'-deoxyadenosine, 2-bromo-3'-deoxyadenosine, and 2-iodo-3'-deoxyadenosine) by adenosine kinase, within the cells, and to correlate it with their cytotoxicities and abilities to inhibit the repair of X-ray-induced potentially lethal damage (PLDR). Of all compounds, only cordycepin was found to be phosphorylated and showed both potent cytotoxicity and ability to inhibit PLDR.

[Laboratory and clinical studies of norfloxacin in respiratory tract infections].

Norfloxacin (NFLX), a new quinolone antibacterial agent, was investigated for its antibacterial activity and clinical efficacy on respiratory tract infections. The results obtained are summarized as follows: 1. Antibacterial activities were evaluated against 127 strains of various bacteria isolated from clinical sources. MIC80's of this drug were: against Staphylococcus aureus and Streptococcus pyogenes 1.56 micrograms/ml; Haemophilus influenzae 0.05 microgram/ml or less; and Klebsiella sp. and Enterobacter sp. 0.10 microgram/ml. These antibacterial activities were superior to these of ampicillin and cephalexin, except against S. pyogenes. 2. Clinical responses to NFLX in a total of 32 cases with respiratory tract infections were excellent in 9 cases, good in 12, fair in 9, poor in 2, with an efficacy rate of 65.6%. Neither adverse reactions nor abnormalities of laboratory test results were observed.

Spin trapping of precursors of thymine damage in X-irradiated DNA.

A spin-trapping method combined with ESR spectroscopy was utilized to obtain evidence for the presence of precursor radicals leading to damage in X-irradiated DNA. Two technical improvements were introduced to the conventional spin-trapping method to make possible its application to large molecules such as DNA: prior to X irradiation, sonolysis of aqueous DNA solution by 19.5-kHz ultrasound was made to get a highly concentrated DNA solution and to lower the viscosity of the solution; after precursor radicals in X-irradiated DNA were trapped by a spin-trapping reagent, the DNA was digested to oligonucleotides by DNase I to get an ESR spectrum with a well-resolved hyperfine structure. Thus, it was recognized that the ESR spectrum obtained after X irradiation of the aqueous solution containing DNA and the nitroso spin-trapping reagent 2-methyl-2-nitrosopropane consisted of at least three sets of signals in the DNA. Identification of free radicals was made by comparing the spectrum with that of thymidine, which was precisely examined by a spin-trapping method combining two kinds of spin traps (nitroso and nitrone compounds) with liquid chromatography. As a result, all the signals were identified as the spin adducts of radicals produced at the thymine base moiety of DNA. The 5-hydroxy-5,6-dihydrothymin-6-yl radical was identified as a precursor of 5,6-dihydroxy-5,6-dihydrothymine (thymine glycol), the 6-hydroxy-5,6-dihydrothymin-5-yl radical as a precursor of 6-hydroxy-5,6-dihydrothymine, and the 5-methyleneuracil radical as a precursor of 5-(hydroxymethyl) uracil.
