The histories and medical courses were reviewed in 154 patients with congenital heart disease (CHD) ranged in age 40 to 77 years (mean 48.8 years) who had not yet undergone cardiac surgery. The CHD most commonly involved were atrial septal defect (ASD)(63.6%), ventricular septal defect (13.6%) and patent ductus arteriosus (8.4%). There were no cases of tetralogy of Fallot or of transposition of the great arteries in this study. Hemodynamic observations showed no significant correlation between the age and systolic pulmonary artery pressure in left-to-right shunt disease. Mortality due to cardiac failure was higher in patients whose systolic pulmonary artery pressure was over 50 mmHg. 91 patients had cardiac surgery and surgical mortality was 7% (6 patients). There was one late ASD death, and 46% are in good condition (NYHA I). In patients without surgery, 27% (17 patients) died of cardiac failure except one, and 11% (6 patients) are in good condition. Among factors affecting prognosis, pulmonary vascular changes are the most important, with arrhythmia and myocardial damage occurring with age, and complications like infective endocarditis and coronary sclerosis affecting prognosis adversely.

Deoxycorticosterone pivalate (2.5 mg/kg) given intramuscularly on four occasions 10-15 days apart over a period of 45 days to unilaterally nephrectomized adult male mongrel dogs, receiving as drinking solution 0.9% NaCl in 5% dextrose, resulted in an average sustained rise in the mean arterial blood pressure of 30 mm Hg (1 mm Hg - 133 N/m2) in 60% of the animals. Hypertensive dogs had in their arterial tissues generally more sodium, potassium, magnesium, and calcium than the similarly treated but non-hypertensive dogs, but compared to the tissues of operated untreated or unoperated normotensive dogs, only sodium and calcium were significantly higher. The dogs who were similarly treated but did not develop hypertension had in their arterial tissues less sodium, potassium, and magnesium than operated untreated or unoperated normotensive dogs. Norepinephrine content in the branches of mesenteric arteries of all deoxycorticosterone- and NaCl-treated animals, irrespective of their blood pressure, was significantly lower, and in the myocardium significantly higher, than either the unoperated normotensive or operated but not further treated dogs. It is concluded, therefore, that in deoxycorticosterone + NaCl treatment the dogs which developed hypertension had more arterial sodium, potassium, magnesium, and calcium than those who were similarly treated but remained within the limits of normal blood pressure, and that there was no difference between hypertensive and non-hypertensive dogs in regard to their cardiovascular norepinephrine content.
In dogs made hypertensive renal artery stenosis and contralateral nephrectomy, the arterial and myocardial tissue content of water, cations (sodium, potassium, magnesium, calcium), and norepinephrine was measured 20 and 60 days after the operation. Hypertensive animals autopsied at the earlier stage of hypertension had significantly lower (-25 to-50%, P smaller than 0.01) arterial norepinephrine than either the sham or nonhypertensive animals. The water and cation content of arteries was unchanged, but aortic tissue contained significantly more water, sodium, potassium, and magnesium than the nonhypertensives. Hypertensives autopsied at 60 days also had lower arterial norepinephrine content than sham and nonhypertensive animals, but this refuction was not significant. Their arterial and aortic tissue contained more water and cations than the nonhypertensive, sham, unoperated, and unilaterally nephrectomized dogs. It is suggested that in the initial stage of renovascular hypertension the arterial norepinephrine content is reduced significantly, whereas at a later stage the arterial water and cations may be involved in the maintenance of an elevated blood pressure.
