Comparative biomechanical evaluation of different external fixation sidebars: stainless-steel tubes versus carbon fiber rods.

Carbon fiber rods were developed to provide radiolucent sidebars for external fixation. In the present study, a single-plane, half-pin, double-bar external fixator construct with either stainless-steel tubes or carbon fiber rods was applied on the anteromedial surface of an osteotomized synthetic human tibia and evaluated for fixation rigidity. Testing was performed with the bone fragments in cortical contact and with a 5-mm midshaft gap between the fragments. The sidebars then were loaded to failure in bending. The results of this study show (a) that the carbon fiber rods were 15% stiffer than the stainless-steel tubes (p = 0.009) and (b) that the external fixator with carbon fiber rods achieved approximately 85% of the fixation stiffness of the external fixator with stainless-steel tubes. The loss of stiffness of the external fixator with carbon fiber rods is most likely due to the clamps being less effective in connecting the carbon fiber rods rigidly to the Schanz screws.

Our experience with Lyme arthritis.

Since 1975, when first described, Lyme disease becomes more and more interesting and complicated medical problem. One of the "classical features" of the disease is arthritis. Based upon our own experience we described and discuss the diagnosis, treatment and outcome of three patients with Lyme arthritis.

[Dysplasia epiphysealis capitis femoris].

A description of rare condition in children, dysplasia epiphysealis capitis femoris, known in the literature as Meyers dysplasia is presented. Radiographic appearance resembles well known Legg-Calve-Perthes disease. Clinical follow-up of 6 cases, documented radiographically proves incidence of this condition. The management and differential diagnosis is offered.

[Characteristics of hay fever during pollen season with regard to the influence of specific immunotherapy. I. Clinical course and biochemical changes in nasal lavage].

Nasal lavage is a useful tool for monitoring of inflammatory process in pollinosis. In 27 patients with pollen allergy the nasal lavage was performed before, during and after pollen season. The concentration of total protein, albumin and lysozyme were determined in obtained fluid. In one group Pollinex (Bencard) was applied before pollen season, and in the second one--placebo in similar injections. The concentrations of total protein in nasal lavage fluid was significantly lower after the pollen season in both analyzed groups. On the other hand, the concentrations of albumin and lysozyme were increased during pollen season relatively to values before and after the season. In patients treated with Pollinex observed values after the season were lower than in placebo group.

In vivo exposure to sodium fluoride does not modify the yield of viral tumour-induced periosteal bone nor of heterotopic bone induced by human tumour KB cells in mice.

In mice local or systemic administration of fluoride (5-25 mg NaF/kg body weight) during the proliferative phase of bone formation (up to 20 days) has no effect on the yield of bone formed either by local stimulation of periosteal membrane by Moloney sarcoma virus-induced tumour or on bone induced heterotopically by human KB cells. The lack of stimulatory activity of fluoride on rapidly induced osteogenesis in mice is in agreement with recent reports which show that fluoride is not a potent mitogen for human osteoblasts grown in vitro.

[Surgical treatment of lenses dislocated to the vitreous cavity].

Removal of the lens dislocated to the vitreous body is one of the most difficult problems in ophthalmic surgery. The authors present 8 consecutive cases in which the lenses luxated to the vitreous were removed. Pars plana vitrectomy was used to free the lenses from the surrounding adhesions and to carry them over to the pupillary region. Then the limbal incision was performed to remove them from the eye. Good anatomical and functional results achieved in the described cases point to the suitability of this operative method.

[Early and late observations of intraocular pressure in patients with implanted intraocular lens].

The aim of the paper was to evaluate intraocular pressure changes that follow the implantation of posterior intraocular lens. In two examined groups of patients (I group--sulcus ciliaris, II group--intracapsular implantation) the decrease of average intraocular pressure values was observed in the early days after surgery, then during 3 months, it gradually returned to the initial values. This decrease was less marked in the first group of patients. Long-term observation showed the stabilization of intraocular pressure at a level similar to the mean values before surgery.

[Left ventricular outflow obstruction as a result of external cardiac compression by a mediastinal tumor].

A case of left ventricular outflow tract stenosis resulted from compression of the heart is presented. This symptomatic cardiovascular lesion was produced by extensive growth of mediastinal tumour. Numerous noninvasive methods were necessary to establish this difficult diagnosis. The patient underwent surgery that disclosed a benign process (of neurofibroma). After tumour's resection marked hemodynamic signs subsided.

Production of mitogen-contamination free alginates with variable ratios of mannuronic acid to guluronic acid by free flow electrophoresis.

Commercial alginates consisting of variable homopolymeric regions of beta-D-mannuronic acid and alpha-L-guluronic acid, interspaced with regions of alternating blocks, are potent stimulators of macrophages and lymphocytes. Therefore, inflammatory reactions and fibrotic overgrowth of the beads result if Langerhans islets are encapsulated in raw alginate hydrogel beads (cross-linked with divalent cations). The result is random failure of the islets some time after transplantation. Analysis of raw alginates by using free flow electrophoresis demonstrated that commercial alginates contained at least 10-20 fractions (characterized by different electrophoretic mobilities) which showed mitogenic activity. These fractions could be quantitatively separated from the alginic acids by free flow electrophoresis on a preparative scale. The purified alginates cross-linked with Ca2+ ions exhibited no mitogenic reactions as proved by an in vitro assay. In addition, examination of purified Ba2+ alginate beads implanted intraperitoneally in rats or mice for three weeks showed no fibrotic overgrowth in contrast to implants made from unpurified alginate.

[Recurrent bacterial endocarditis with involvement of the tricuspid valve after surgical correction of congenital heart defect].

A case of recurrent tricuspid valve endocarditis after surgical closure of ventricular septal defect is presented. Intensive medical treatment lasting nearly ten years completely failed. There were still vegetations attached to the septal leaflet of the tricuspid valve with positive cultures (Ps. aeruginosa). Persistent sepsis without signs of heart failure required surgical intervention. Tricuspid valvuloplasty with excision of infected patch was successfully performed. Six months later the patient remained symptomless.

[Endemic occurrence of lyme disease in the forested areas of the Piła district].

Lyme disease (L.D.) caused by Borrelia burgdorferi and spread by Ixodes ticks arouses great interest with more and more clinicians and other scientists. It may be very difficult to diagnose a disease as L.D. because of its various clinical symptoms expressions. This is why it is often called "the great imitator". There are three phases in the natural history of the disease. Diagnostic problems come from the fact that early phases are often lacking. The disease may begin with any symptom of any stage. In our paper we present the endemicity of Lyme disease among a group of 28 people who spent their summer holidays on a forest camp near Piła (in the north-western part of Poland) in July 1991. We diagnosed 15 of them as having early stages of L.D.
