[Estimation of bleomycin-induced chromosome aberrations in lymphocytes of laryngeal cancer subjects. Preliminary report].

Chromosome instability is associated with an increased risk of malignancy. However, the quantitative analysis of chromosome breaks provided by the bleomycin test requires additional analysis aimed for the localisation of chromosome aberrations. For this reason, the metaphasis slides prepared for bleomycin test were stained with fluorochrome DAPI to estimate chromosome breaks in particular chromosomes. The additional staining of chromosomes can be recognised as an extension of the classical bleomycin test addressed for identification of structural aberrations. Preliminary results indicate that the most frequent chromosome breaks were found in chromosomes 1, 2, 3, 7 and 13.

[Amphetamine poisoning].

We describe five patients with complications following amphetamine overdose. Two of the patients died: one with disseminated intravascular coagulation and circulatory collapse, one with severe rhabdomyolysis and ischemic colitis. Among the other three cases, one developed acute psychosis, hyperthermia and rhabdomyolysis, one developed acute respiratory distress syndrome and one pericarditis. The effects of amphetamine are discussed along with the diagnosis and treatment of patients with such poisoning.

Insulin impairs the anticonvulsive activity of carbamazepine against maximal electroshock-induced seizures in mice.

In view of the data indicating that insulin can modify penetration of some drugs across cell membranes and tissue barriers, particularly the blood-brain barrier, the aim of the present study was to evaluate the effect of insulin on both the anticonvulsant activity and the brain concentration of carbamazepine in mice suffering from seizures induced by maximal electroshock. The antiepileptic drug was administered per os in single doses either alone or in combination with insulin given as single intraperitoneal injections. To assess the anticonvulsant activity of carbamazepine the ED50 values were calculated. The results indicate that insulin given in doses up to 2 units/kg did not affect the convulsive threshold, whereas insulin applied at 2 units/kg led to a significant reduction in the anticonvulsant activity of carbamazepine, as judged by an increase in the ED50 value from 16.2 to 41.3 mg/kg. This effect was accompanied by the marked reduction in both the brain and blood concentrations of the drug. It is likely, therefore, that the inhibitory activity of insulin on the anticonvulsive function of carbamazepine is related not only to the effect of the former on the blood-brain transport of the latter, but also to insulin-induced modulation of the serum concentration of the drug.

Severe organophosphate (demeton-S-methyl) poisoning in a two-year-old child.

A previously healthy 2-y-old boy was admitted to the hospital 30 min after the ingestion of 10 ml of demeton-S-methyl (META-SYSTOX). Treatment consisted of gastric decontamination, atropine, reactivator (obidoxime) and supportive therapy. Atropine was given to control the muscarinic features. Assisted ventilation was required for 6 h; however, this treatment was able to be discontinued following the second injection of obidoxime 11.5 h after the ingestion. Excess salivation and slight bradycardia were easily controlled with small doses of atropine for 5 d following admission to Ullevaal Hospital. Further course was uneventful, and the patient was discharged on the 8th d without any sequelae. Plasma cholinesterase levels were initially low (<400 U/l), but returned to reference values upon discharge. In this case, adequate supportive therapy and the rapid administration of both atropine and obidoxime were clearly associated with a favorable outcomes

Ethanol treatment in ethylene glycol poisoned patients.

Two otherwise healthy 16-y-old female patients were treated with sodium bicarbonate and ethanol after the ingestion of unknown quantities of ethylene glycol. Patient 2 was admitted twice for ethylene glycol poisoning in unrelated events. In patient 1, the maximum levels of ethylene glycol and glycolate in plasma were 14 mmol/L (0.9 g/L) and 8.2 mmol/L (0.5 g/L), respectively. In patient 2, the maximum levels of ethylene glycol in plasma during the 2 admissions were 18 mmol/L (1.1 g/L) and 45 mmol (2.8 g/L), respectively. In patient 1, a blood ethanol concentration between 130-140 mg/dL (28-30 mmol/L) was reached 3 h after the start of ethanol administration and maintained for 22 h. During this period, ethylene glycol metabolism was effectively inhibited as indicated by S-glycolate levels and that 88% of the eliminated ethylene glycol was accounted for in the urine. This suggests that ethanol therapy alone may be sufficient for patients admitted early with low serum ethylene glycol concentrations. During the admissions of patient 2, the blood ethanol concentrations were presumed to effectively inhibit ethylene glycol metabolism as judged from normal acid/base parameters. However, during the second admission the bolus infusion of ethanol was associated with respiratory arrest. During both admissions for patient 2, hemodialysis constituted the major route of ethylene glycol elimination.

Structural and immunochemical studies on the O-specific polysaccharide of Proteus penneri strain 15.

On the basis of sugar analysis and 1H- and 13C-NMR spectroscopy, it was shown that the O-specific polysaccharide of Proteus penneri strain 15 has a trisaccharide repeating unit, including an acetal-linked pyruvic acid residue, and is structurally identical to the capsular polysaccharide of Proteus vulgaris strain ATCC 49990. Serological studies supported this conclusion and demonstrated the presence in the homological antiserum of both anti-core and anti-O chain antibodies reacting with a lipopolysaccharide (LPS) epitope containing N-acetylglucosamine and galactose residues.

[Comparisons of laparoscopic and classical adrenalectomy from personal material].

Postoperative course in 12 patients which had laparoscopic adrenalectomy performed was compared with 8 underwent classic one in terms of operating time, blood loss, blood pressure recordings, C-reactive protein blood concentration, postoperative pain and hospital stay. The advantages of the laparoscopic approach have been stressed.

[Surgery for inguinal hernia from the extraperitoneal approach using laparoscopy--two years of personal studies].

From May 1995 to January 1997, 237 laparoscopic extraperitoneal inguinal hernia repairs were performed in Department of General Surgery of District Hospital in Szczecin. 197 patient had uni- and 20 bilateral inguinal hernia. The mean age of patients was 51 years. The average time of procedure and hospitalization was 38 min and 1.7 days respectively. Seven (3.2%) recurrences were observed in postoperative observation. As a conclusion, according to authors' achieved experience, one can say that the laparoscopic extraperitoneal inguinal hernia repair is in benefit both for patient and for surgeon.

Influence of CCK-8 and yohimbine on supraspinal modulation of nociceptive process.

We studied the effect of cholecystokinin (CCK-8) and yohimbine-alpha-2-adrenoreceptor antagonist administered locally into lateral reticular formation (LRF) on the bioelectrical response to the pain stimulation. The experiments were carried out on 8 conscious rabbits with bilaterally implanted electrodes into: motor-sensory cortex, ventro-postero-lateral thalamic nuclei, hippocampus and LRF. Nociceptive stimulation was performed by means of electrical pulses applied to the front paw. Bioelectrical activity (BA) of the chosen brain structures was analysed by means of spectral analysis (FFT) and directed transfer function (DTF). The results of our study may suggest that supraspinal administration of CCK-8 together with yohimbine have inhibitory effect on nociceptive transmission.

[Scintigraphy examination in differential diagnosis and evaluation during the course of Perthes disease].

Forty-four bone scintigraphies were done in 37 children with diagnosis or suspicion of Perthes disease. In 5 children with transient coxitis 3 hips were normal and 2 presented slightly increased uptake. In 13 cases of epiphyseal dysplasia 12 hips were normal; one presented decreased uptake. Nineteen Perthes cases showed typical uptake deficits; 3 joints showed slight increase of uptake, 3 joints were normal. Scintigraphy was related to different stages of Perthes disease. The value of bone scintigraphy in Perthes differentiation and monitoring was discussed.

Increased urinary C-peptide and albumin excretion in juvenile borderline hypertensives.

The study was designed to evaluate the urinary excretion of C-peptide and albumin, and urinary N-acetyl-beta-D-glucosaminidase (NAG) activity in juvenile borderline hypertensives. The second aim was to examine the relationship between these variables and ambulatory blood pressure level and variability. The study group consisted of 21 non-obese males consecutively chosen from patients with borderline hypertension, defined by sphygmanometer readings, examined in our outpatient clinic. All subjects collected separately their day-time and night-time urines during the period of ambulatory blood pressure monitoring. In 16 patients, who were considered to have "sustained" borderline hypertension, both 24-h urinary C-peptide excretion and 24-h UAE were significantly increased in comparison to those of the controls, while NAG activity did not differ significantly between the two groups. UAE was significantly lower at night than during the day in both borderline hypertensives and controls. Twenty-four-hour UAE in borderline hypertensives correlated significantly with the ambulatory blood pressure variability, but not with the average blood pressure level. These results suggest that the 24-h insulin secretion rate estimated by means of urinary C-peptide excretion is significantly increased in "sustained" borderline hypertensives. Elevated UAE in juvenile borderline hypertensives can be explained by a possible direct effect of systemic blood pressure variability on albuminuria.

The migration of granulocytes from allergic and nonallergic infiltrations in patients with atopic asthma.

The granulocyte migration inhibition tests both in vivo and in vitro from allergic skin infiltrations were carried out in 30 patients with atopic asthma and in 20 healthy people used as a control. It was found that the migration of leukocytes from allergic skin infiltrations was defective with the lowest ability to migrate. Neutrophils comprised about 80% cells found in the cutaneous allergic infiltrations.

[Incidence of goiter in children of the Tarnobrzeg district and environmental factors].

A group of 1897 school children from Tarnobrzeg district were examined out of this number 1241 (65.4%) children displayed goiter. Goiter incidence was also established depending upon age and sex. The analysis also included goiter incidence in relation to its size according to the WHO classification. In certain patients, newborns and their mothers, determinations of iodine excretion were made; the values were found to be within the normal. This range suggests that other etiological factors than iodine deficiency might possibly contribute to goiter development in the studied children.

[Effect of platelet activating factor (PAF) on granulocyte migration in vivo in patients with atopic asthma].

The study was carried out in 40 patients with atopic asthma, age 16-31 years (mean 26 years) and 30 healthy volunteers, age 19-25 years (mean 20 years). In vivo granulocyte migration tests were carried out simultaneously using two methods: Southam's and the authors' original (Matusiewicz, Brzezińska). The above mentioned tests were carried out twice: the first time in December/January, the second time February/March after subcutaneous administration of PAF. The authors have demonstrated that in patients with atopic asthma a defect in migration of tissue granulocytes is present. They have also shown that PAF does not effect significantly granulocyte migration in atopic patients.

[Mean elimination time of the impulse radiotracer (131I-hippurate) as a marker of renal function].

A possibility of assessment of renal excretory function using mean time of the impulse tracer (131I-hippurate) dose elimination has been shown on the basis of theoretical analysis of hippurate kinetics. Such an index was calculated during classic hippurate renography from time/activity curve over precordium. Other traces and different regions of monitoring might also be employed. The mean time of tracer elimination obtained in this manner only indirectly reflects the hippurate clearance, yet can be easily achieved. Moreover, this entity is not influenced by errors related to quantitative measurements of radioactivity and to hypothetical assumptions, which are common for more sophisticated isotope clearance methods. The enlisted characteristics seem to predispose the proposed method of renal function evaluation for application in screening isotope tests.

[Possibility of quantitative evaluation of renal function based on the renography alone].

Exploring the possibilities of the renal function evaluation on the basis of the renographic curve alone, a theoretical analysis and clinical investigations were carried out. As a result, some indicators of effective renal plasma flow and an indicator of mean transit time were developed. The methods of calculations may serve for a simplified, screening evaluations of renal function, using a simple two-scintillation probe set, as well as computerized camera. The said calculations might constitute a starting point for developing more precise quantitative methods.
