RBEs of thermal neutron capture therapy and 10B(n, alpha)7 Li reaction on melanoma-bearing hamsters.

Using Greene's melanoma transplanted into Syrian (golden) hamsters, we determined the relative biological effectiveness (RBE) of thermal neutron capture therapy (TNCT) using 10B-paraboronophenylalanine (10B-BPA) in comparison with a 9-MeV electron beam. We also obtained the RBE of the 10B(n, alpha)7 Li reaction by calculation based on summed dose data from TNCT. Throughout this study, the Kyoto University Research Reactor was used as the source for thermal neutrons; the reactor was specially altered to attain a low contamination level both for gamma-rays and fast neutrons. 10B-BPA was administered 8 hours before thermal neutron irradiation to the hamsters with melanoma. The tumor was then irradiated at 5 MW for 90 minutes. The absorbed dose from this TNCT was calculated by the method of Fairchild and Goodman (Phys. Med. Biol. 1966; 2:15-30). The RBEs of the TNCT and the 10B(n, alpha)7 Li reaction obtained by the tumor growth delay time (TGDT) method were 2.22 and 2.51, respectively, at 10.5 days of TGDT. These RBE values varied with TGDT and the absorbed dose. The RBE value of TNCT had a peak at 7.0 days of TGDT; that of the 10B(n, alpha)7Li reaction was higher at a low absorbed dose level and lower at a high absorbed dose level.

Expression of pulmonary mRNA encoding ICAM-1, VCAM-1, and P-selectin following thoracic irradiation in mice.

PURPOSE: Recent studies have revealed that ionizing radiation induces the expression of intercellular adhesion molecule-1 (ICAM-1), vascular cell adhesion molecule-1 (VCAM-1), and P-selectin in vitro. The purpose of this study was to investigate the expression of these adhesion molecules in mouse lung following whole thoracic irradiation. MATERIALS AND METHODS: C57BL/6J mice were irradiated with a single dose of 12 Gy to the thoraces and sacrificed at 4, 12, 24, and 48 hours and 1, 2, 4, and 8 weeks after irradiation. Expression of total lung mRNA for ICAM-1, VCAM-1, and P-selectin was quantified by the Northern blot method and normalized to beta-actin. RESULTS: There were increases in mRNA for ICAM-1 of 42% at 4 hours (p < 0.05), 76% at 24 hours (p < 0.01), and 51% at 48 hours (p < 0.05) compared with the controls. These returned to the control level at 1 week. The expression of VCAM-1 mRNA was also increased by 49% (p < 0.01) at 12 hours and was still increased by 25% at 1 week. P-selectin mRNA was transiently increased by 59% at 12 hours. CONCLUSIONS: These early inductions of mRNA for ICAM-1, VCAM-1, and P-selectin in mouse lung following thoracic irradiation were transient but significant, and are one of the most immediate changes reported in vivo.

Clinical usefulness of oral itraconazole, an antimycotic drug, for refractory atopic dermatitis.

We investigated the clinical usefulness of oral itraconazole for refractory atopic dermatitis in a crossover study. Patients with refractory atopic dermatitis were divided into two groups: Group A; a combination of itraconazole plus a conventional lactobacillus preparation was administered for 8 weeks, followed by lactobacillus preparation alone for 8 weeks, Group B; lactobacillus preparation alone was administered for 8 weeks, followed by itraconazole plus lactobacillus for 8 weeks. In both groups, a decrease in dose or strength of concomitant topical steroids was observed at the end of the treatment course of itraconazole, and improvement of parameters such as eosinophil count, serum IgE level and specific IgE antibody titers to fungi was also observed after the administration of itraconazole. These results suggest that oral itraconazole is useful for the treatment of intractable atopic dermatitis patients who do not respond to conventional therapeutic approaches.

Radiology of lung cancer.

It is of vital importance not only to prevent lung cancer but also to detect and treat it as early as possible. Plain chest X-ray examination often provides preliminary data useful for the early detection of lung cancer, and multiple diagnostic modalities such as computed tomography (CT) and magnetic resonance imaging (MRI) are used to obtain more accurate information. The authors describe the clinical utility of imaging diagnosis by computed radiography (CR), advanced multiple beam equalization radiography (AMBER), CT, and MRI in term of the imaging-pathologic correlation. We would like to emphasize that these modalities are not competitive but supplementary. It should be anticipated that, as technological improvements continue to be made, each modality will assume an increasingly important role in the imaging of lung cancer.

Pulmonary arteriovenous fistula: long-term results of percutaneous transcatheter embolization with spring coils.

PURPOSE: The aim of this study was to assess the effectiveness of percutaneous treatment of PAVFs through the results of long-term follow-up. PATIENTS AND METHODS: Transcatheter embolization of PAVFs using spring coils was performed in seven patients (six women and one man) with nine PAVFs, between 1989 and 1995, who were followed up at least one year. The size of the nine PAVFs ranged from 4 mm to 70 mm in diameter (mean 26 mm). Two fistulas were huge, 70 x 45 mm and 60 x 65 mm in diameter. Embolization was performed twice in three patients and once in four patients. In the two patients with huge PAVFs, after anchoring metallic "spiders" into the feeding artery, the largest coils were positioned and entangled with the "spider" followed by the placement of smaller coils to fill in the center of the feeding artery. RESULTS: No severe complications were seen in the short-term follow-up. Arterial oxygen pressure improved in all patients with low oxygen pressure. Five patients with seven fistulas, six of which disappeared on chest X-ray film, are still alive after follow-up ranging from one year and 10 months to seven years and nine months. The two patients with huge fistulas died more than one year after treatment, one of hemoptysis of the contralateral lung and the other of pulmonary fibrosis. CONCLUSION: Complete embolization of a medium-sized feeding artery can cure this anomaly.

Inferior vena cava filter placement for prevention of pulmonary tumor emboli of renal cancer with renal vein or vena caval tumor thrombus: prophylactic usage prior to transcatheter arterial embolization.

PURPOSE: To prevent pulmonary embolization of necrotic intravenous tumor thrombus after transcatheter embolization of renal cell carcinoma, we placed suprarenal inferior vena cava (IVC) filters. MATERIALS AND METHODS: Suprarenal IVC filters were placed prior to transcatheter arterial embolization (TAE) in four patients, two women and two men, with renal cancer accompanied by renal vein in one and vena caval tumor thrombus in three patients. We used one bird's nest filter for the patient with renal vein tumor thrombus and a titanium Greenfield filter for each of the three patients with vena caval tumor thrombus. TAE was performed with pure ethanol under balloon occlusion of the renal artery. RESULTS: IVC filters were successfully placed at the suprarenal position. The patients tolerated the procedure well and had extensive tumor infarction, including the tumor thrombus, on follow-up computed tomography (CT). No pulmonary infarction or adverse effects were seen during the follow-up period. One patient died from diabetic renal failure, two patients have been alive for 8.5 and 7.5 months respectively, the other one is lost to follow-up. CONCLUSIONS: Suprarenal IVC filter placement prior to TAE for advanced renal cell carcinoma invading the renal vein or the IVC is an effective procedure to prevent pulmonary thromboemboli and may contribute to longer survival.
