Heterogeneity of myocardial wall motion and thickening in the left ventricle evaluated with quantitative gated SPECT.

BACKGROUND: Global and regional ventricular function may be evaluated by using gated myocardial perfusion single photon emission computed tomography (SPECT). This study investigated two parameters of regional contraction of the left ventricle, segmental wall motion (WM) and wall thickening (WT), to determine their similarity and disparity in each myocardial segment in patients with normal myocardial perfusion. METHODS AND RESULTS: Thirty-five patients with normal myocardial perfusion and cardiac function (mean left ventricular ejection fraction, 62.6%+/-8.8%) were included in this study. A 1-day stress/rest protocol was used as a means of acquiring technetium 99m (Tc-99m) sestamibi gated SPECT protocol for each patient. A commercially available software package for quantitative gated SPECT (QGS) was used to generate cine loop three-dimensional surface display and SPECT images. The left ventricle was divided into 9 segments to score WM and WT (on a scale of 0 to 4, with 0 being normal and 4 being severely reduced) by 6 independent observers. The WM score was significantly higher than the WT score in the septum, whereas the WM score was lower than the WT score in the inferior segment. Similar WM and WT scores were observed in the remaining segments. CONCLUSIONS: Heterogeneous myocardial WM and WT were observed by using QGS software. These findings suggest that different criteria are required in each segment to evaluate segmental WM and WT by means of gated myocardial perfusion SPECT.

Distribution of gamma-glutamyl transpeptidase in male reproductive system of rats and its age-related changes.

The present study is designed to investigate localization of gamma-glutamyl transpeptidase (gamma-GTP) in male reproductive organs and its age-related changes using Wistar rats aged 2 to 15 weeks. Histochemically, gamma-GTP activity was detected intensively in epithelial cells of epididymides and seminal vesicles and weakly in those of anterior prostates, but not in testes under the present conditions. Biochemically, the highest gamma-GTP activity was found in epididymal head portions. The order of the activity was epididymides (head greater than body greater than tail), seminal vesicles, prostates and testes. The activity increases with sexual maturation in epididymides and seminal vesicles, but not in prostates. Since gamma-GTP is an enzyme involved in the incorporation of amino acids, the present findings suggest that not only epididymides but also seminal vesicles possess uptake mechanisms for amino acids.

The administration regimen of isepamicin in patients with chronic respiratory tract infection.

A total of 34 patients with intractable chronic respiratory tract infections were treated with isepamicin and/or piperacillin in different dosage regimens. A comparison of the bacteriological effect using a cross over method showed a reduction in the count of Pseudomonas aeruginosa in sputum in the group receiving once-a-day isepamicin combined with piperacillin, compared with the twice-a-day combined administration. A comparison of the clinical and bacteriological efficacy between the different regimen groups revealed no noticeable difference. The clinical effect of this regimen is comparable to the conventional regimen, but has the advantages of a safer dosage and ease of administration.
