Elevated serum CA19-9 levels in poorly controlled diabetic patients.

Serum CA19-9 levels were measured in 60 diabetic patients and 40 healthy volunteers. Serum CA19-9 concentration was correlated with hemoglobin A1 (HbA1) (r = 0.4368 P less than 0.005) and fasting plasma glucose levels (r = 0.3410 P less than 0.01). None of the 40 healthy subjects showed elevated CA19-9 concentrations over 37 units/ml as the upper normal value. The percentage of positive serum CA19-9 levels in poorly controlled patients (fasting plasma glucose greater than 200 mg/dl or HbA1 greater than 13%) and moderately to well controlled patients was 50% and 10%, respectively. No correlation was found between the level of CA19-9 and those total cholesterol, and triglycerides, or the duration of diabetes. In patients who had diabetic retinopathy or persistent proteinuria, the CA19-9 concentration was significantly elevated when compared with those without these complications. It has been shown that patients with adenocarcinoma of the gastrointestinal tract have high plasma CA19-9 levels and those who have benign disease have normal CA19-9 levels. Even though diabetes mellitus is not a malignant disease, serum CA19-9 levels were increased in diabetic patients. These results indicate that HbA1 and fasting plasma glucose should be examined in patients with high CA19-9 levels.

Defect of the first-phase insulin secretion to glucose stimulation in the perfused pancreas of the nonobese diabetic (NOD) mouse.

To investigate the development of impaired insulin secretion in type I diabetes mellitus, the pancreata of ICR and NOD mice (10-50 wk of age) were perfused. According to insulin responses to 30 mM glucose and to 19 mM arginine, we classified the NOD mice into four groups: those having normal insulin secretion to glucose and to arginine similar to that of control ICR mice (group 1); those with a defect in the first-phase insulin secretion to glucose stimulation but with almost normal insulin secretion to arginine, total insulin release to glucose being significantly smaller than that of group 1 (group 2); those having only a small insulin response to either stimulus, but a fasting plasma glucose level still within the normal range (group 3); and those being overtly diabetic, showing no insulin response to either stimulus (group 4). The severity of insulitis and insulin concentration of the pancreas in each group of NOD mice was well correlated with the insulin release from the perfused pancreas. These results indicate that the initial sign of B-cell damage in NOD mice is a defect of the first phase of glucose-induced insulin secretion, which is followed by a total loss of ability to respond to glucose or arginine stimulation.

[Fusional vergence hysteresis and sensorimotor connection of the two eyes].

Three experiments were conducted to replicate and extend Helmholtz's observation indicating vergence hysteresis. The two stimuli were presented dichoptically to the eyes in the mirror stereoscope and moved temporalward or nasalward. Helmholtz's finding was confirmed in Experiment 1, in which the critical separations were measured for the stimuli divergently moving temporalward when fusion of their images was abruptly broken (breakaway point) and when fusion was re-established (refusion point) for the stimuli moving nasalward. In Experiment 2, in which only the stimulus for one eye moved temporalward while the stimulus for the other eye was stationary, the breakaway point of the moving stimulus was found to be linearly dependent on the position of the stationary stimulus. Experiment 3 showed that, in the asymmetric divergent tracking as in Experiment 2, the change of the perceived visual direction of the fused image was smaller than the angular magnitude of the movement of the tracking eye. These results were discussed as evidence indicating the sensory and motor connection of the two eyes.

[Immunochemical approach to clinical cytology by demonstration of tumor markers using enzyme-labeled antibody technic].

Demonstration of tumor markers using the enzyme-labeled antibody technique was evaluated as an immunochemical approach to cytodiagnosis. AFP, CEA, CA 19-9, or CA 125 were stained in specimens of pleural effusion, ascites, cerebrospinal fluid, hepatopancreatic tumor, bone marrow and spuata. Malignant cells of the cases with elevated marker levels in the body fluid were mostly positively stained. And, interestingly enough, some of the malignant cases showed positively, while diagnosis by Papanicolaou's staining was not established for malignancy. It is suggested that immunochemical cytology by tumor marker staining would be useful for analyzing malignant cells and helpful in clinical cytology.

Cystamine-enkephalin dimer. Syntheses and biological activities of enkephalin analogs containing cystamine and cysteamine.

A cystamine-enkephalin dimer, containing two molecules of [D-Ala2, Leu5] enkephalin cross-linked at the COOH-terminal leucine residue with cystamine, (NH2-CH2-CH2-S-)2, has been synthesized in order to examine directly the dimerization effect of an enkephalin molecule on the opiate receptor interactions. In a comparison of potencies against [3H]-[D-Ala2,D-Leu5] enkephalin (3H-DADLE) and [3H]-[D-Ala2,MePhe4,Gly-ol5] enkephalin (3H-DAGO) as delta and mu tracers, respectively, enkephalin dimer showed a very high affinity, especially for the delta opiate receptors. Dimer was almost threefold more potent than DADLE, which is one of the most utilized delta ligand to date. When the binding affinity of cystamine-dimer was compared with that of its reduced thiol-monomer, namely [D-Ala2,Leu5,cysteamine6] enkephalin, the increment in affinity was four to fivefold for both delta and mu receptors. The results strongly indicate that the dimeric enkephalin is more potent presumably due to the simultaneous interaction with the two binding sites of the opiate receptors.

[Breast conservation surgery with irradiation in breast cancer].

For the treatment of patients with early breast cancer, breast conservation surgery with irradiation is gaining world wide acceptance as opposed to more mutilating operations. In Japan, however, most surgeons and radiation oncologists are still unfamiliar with this technique. To achieve optimum functional and cosmetic results, understanding the philosophy and details of the technique is of primary importance. We propose a method of executing the procedures. Tumor excision with a tumor-free margin and breast irradiation with tangential fields are the essential part of the treatment. The entire breast should not receive more than 50 Gy. Limited axillary dissection is used for deciding the indications for chemotherapy, and it is necessary to obtain more than five nodes. Irradiation to the lymph nodes is optional.

[Assay of serum carbohydrate antigen (CA) 19-9 in the diagnosis of gastric cancer].

The levels of the serum carbohydrate antigen (CA) 19-9 were assayed in 284 cases including 77 patients with gastric cancer. The CA 19-9 levels were positive in 38% of gastric cancer cases, and they were significantly higher than those of benign gastric diseases. Extremely increased levels were found in advanced gastric cancer, but not in early gastric cancer. There was no significant correlation between CA 19-9 and CEA, and CA 19-9 seemed to be superior to CEA in specificity for gastric cancer, but inferior in sensitivity. Thus, the assay of CA 19-9 in combination with CEA could be helpful in the differentiation between benign and malignant gastric diseases.
