[Chemoembolization and regional hyperthermia with degradable starch microspheres in the treatment of malignant hepatic tumors].

Antitumor effects of chemoembolization with degradable starch microspheres (DSM) combined with regional hyperthermia (HT) were investigated in patients with hepatocellular carcinoma and metastatic liver cancer. Chemoembolization with DSM was performed in 39 cases of hepatocellular carcinoma (HCC) and in 25 cases of metastatic liver cancer. Thirteen cases of HCC and 3 cases of metastatic liver cancer were treated with the combination of HT. A catheter was placed in the proper hepatic artery via the trans-femoral approach. Adriamycin or Mitomycin C mixed with DSM was injected every 2 or 3 weeks through the catheter. Thermotron RF-8, the heating device used in this study, is operated at 8 MHz radiofrequency. Hyperthermia treatment was applied twice a week. The therapeutic effect of this treatment was evaluated by the change in tumor size measured by angiography or computed tomography. Tumor regression over 50% was observed in 42% of the patients with HCC treated with chemoembolization alone and in 54% of those with chemoembolization and HT. In the patients with metastatic liver cancer, tumor regression over 50% was observed in 65% of the patients treated with chemoembolization alone and in 33% of those with chemoembolization and HT. One-year survival rate after the initial treatment in patients with HCC was 66% and 89% in the patients treated with chemoembolization alone and with chemoembolization and HT, respectively. One and two-year survival rates in the patients with metastatic liver cancer was 55% and 41% in the treatment with chemoembolization alone. These results suggest that chemoembolization using DSM was markedly effective in the patients with malignant hepatic tumors, particularly in metastatic liver cancer.

[A case report of acute renal failure induced by melphalan in a patient with ovarian cancer].

Melphalan is widely used in the chemotherapeutic treatment of an ovarian cancer. Bone marrow depression, nail change, hypersensitivity, chronic pulmonary fibrosis, and nausea and vomiting, are the side effects reported as a result of the toxicity of Melphalan. So far as we know, renal toxicity caused by Melphalan has not been reported in the past literature. A case of acute renal failure induced by Melphalan in a patient with 3rd stage ovarian cancer is presented in this paper. This therefore is the first report of renal toxicity caused by Melphalan.

[Chemoembolization with degradable starch microspheres in metastatic liver cancer].

Transcatheter chemoembolization using Degradable Starch Microspheres (DSM) was performed in 22 patients with metastatic liver cancer. DSM, 45 micron in diameter, which are degraded by serum amylase, temporarily obstruct arterial blood flow at the arteriolar capillary bed. Mitomycin C mixed with DSM was injected into the proper hepatic artery. The therapeutic effect of this chemoembolization was evaluated by the change in tumor size measured by angiography or computed tomography. Tumor regression of over 50% was observed in 15 of 22 cases (68%). Elevated serum CEA levels (greater than 10 ng/ml) decreased in 13 of 18 cases (72%). The overall one-year survival rate was 55%. The one-year survival rate was 83% in 8 patients with H2 type liver metastasis. Although half of the patients had transient pain within 2 hours, no major side effects such as bone marrow suppression or hepatotoxicity were observed. Our results suggest that chemoembolization using DSM is effective in the treatment of metastatic liver cancer.

[Acute promyelocytic leukemia with central nervous system leukemia--a report of two cases].

Two cases of promyelocytic leukemia (APL) with central nervous system leukemia (CNS-L) are reported. The first case displayed some symptoms similar to meningitis at onset, and the second case, during induction therapy suddenly developed left hemiplegia and was found to have CNS-L. There have been only a few case reports of APL associated with CNS-L and each has said that APL was rarely accompanied by CNS-L. Yet, of the reports registered with the Joint Committee for Hematologic neoplasm in Japan, the incidence of APL with CNS-L is not much lower than any of the other types of acute non-lymphocytic leukemia that can accompany a CNS-L. There fore, we feel that CNS-L should not be overlooked as an important prognostic consideration in APL cases.

[Utility of digital perfusion images of the lung in mitral stenosis: the pathophysiological significance of the distribution of pulmonary perfusion].

The relationship between pulmonary hemodynamics (cardiac index; CI and the mean pulmonary artery wedge pressure; mPw) and the redistribution of pulmonary perfusion were studied in 300 patients with mitral stenosis using computerized Tc99m-MAA perfusion images (digital perfusion images: DPI), which consisted of isocount areas. Various types of DPI were obtained and were classified in six grades according to patterns of 100-70% of the isocount area in the right anterior DPI, i.e. grade-0 (normal), grade-1 (increased perfusion in the upper zone), grade-2 (uniform distribution), grade-3 (disappearance of the basal hyperperfusion area), grade-4 (apical hyperperfusion area) and grade-5 (decrease of perfusion at the lower zone). The mPw was thought to be one of the factors determining distribution. Thus, it was not adequate to estimate the mPw from DPI. In patients with the mPw less than 20 mmHg, an increase in perfusion at the upper zone and disappearance of the basal hyperperfusion might be noted, however, the mPw more than 20 mmHg was necessary to cause an apical hyperperfusion area. The predictive values of grades 0 and 1 for the mPw less than 20 mmHg were 70% (54/77) and those of grades 4 and 5 for the mPw less than 20 mmHg and the mPw less than 25 mmHg were 86% (110/128) and 56% (71/128), respectively.(ABSTRACT TRUNCATED AT 250 WORDS)

[A primary malignant lymphoma of the vulva--a case report].

Malignant lymphoma is a rare entity in the neoplasms of the female genital tract, and thus a case of malignant lymphoma of the vulva is reported in this paper. On admission, the patient, a 37-year-old, Japanese female, had complained of vulvar swelling and pain that had persisted for one month. Stage IV tumors were noted in the vulva and bladder and a histological diagnosis indicated non-Hodgkin's lymphoma with diffuse, large cell and B cell types, according to the classification of the Lymphoma Study Group. The patient thus was treated with CH* OP therapy, which consisted of Cyclophosphamide, Aclarubicin Vincristine (Oncovin), and Prednine , and given brain irradiation. Unfortunately her condition has rapidly deteriorated during the 5 months since her admission.

[Long-term follow-up after tumorectomy and irradiation for breast cancer].

From 1968 to 1976, 31 patients underwent breast conservation surgery (tumorectomy) and irradiation. A review of the available pathologic slides has revealed that 11 of that number had definite breast cancer. Of these 11 patients, three experienced a relapse, one with local recurrence only, and two with distant metastasis, though one of these two also had a local recurrence. The patient manifesting a local recurrence only was salvaged by a mastectomy, while the other two with distant metastases have died. Survival and disease-free survival in these 11 patients were at least equal to the results of other Japanese patients of the same ages and stages who had undergone a halsted operation during the same period. As for the other 20 patients, either their pathologic slides were not available, or their diagnosis was equivocal or benign. None suffered a relapse. Cosmetic results were excellent in most patients, and there were no second malignancies in the treatment volume.
