[Laboratory and clinical studies on biapenem (L-627) in the field of pediatrics].

Laboratory and clinical studies on biapenem (L-627), a new carbapenem antibiotic, were carried out in the field of pediatrics. 1. Antibacterial activities of L-627 against clinically isolated organisms in our department were generally high. 2. After 30 minutes intravenous infusion of L-627 at a dose of 10 mg/kg in 3 and of 12 mg/kg in 2 children, peak plasma levels of L-627 ranged from 25.6 to 44.6 micrograms/ml at the end of the infusion. The half-lives were from 0.68 to 0.94 hours. The cumulative urinary recovery rates in the first 6 hours after the start of drip infusion ranged from 11.4 to 47.5%. 3. 14 patients with various bacterial infections were treated with L-627. The clinical efficacy rate was also 92.9% and the bacteriological efficacy rate was also 92.9%. 4. No side effects were observed. A few abnormal laboratory test results were obtained, but they were mild with slight elevation of GOT/GPT and platelets in 1 each.

[A case of herpes zoster meningoencephalitis followed by involvement of cranial nerves IX, X, XI].

The patient is a 64-year-old woman with herpes zoster meningo-encephalitis followed by involvement of cranial nerves IX, X, XI. On admission, she had severe neck pain on the left side and mild nuchal rigidity. Three days later, herpetic vesicles on the left side of her neck (C2, C3, area). Herpes zoster meningoencephalitis was diagnosed based on CSF pleocytosis, high serum and CSF titers of herpes zoster antibody, and EEG abnormality. During hospitalization, paralysis of the left vocal cord, rightward deviation of the uvula, and gradual paralysis of the left sternocleidomastoideus and trapezius muscles developed. On cranial magnetic resonance imaging (MRI), T2-weighted image clearly revealed a high-signal lesion in left dorsal part of the medulla oblongata. This area appeared to correspond to the nucleus ambiguous and vagal nucleus. In this case, we believe that the inflammation originated in the C2, C3 posterior ganglion cells, extended to the IX, X, XI cranial nerves and to the part of the medulla oblongata. It is likely that the number of patients in whom a lesion of the cranial nucleus is revealed by MRI will increase in the future.

[Recombinant human erythropoietin (KRN5702) therapy for autologous blood transfusion in patients with rheumatoid arthritis undergoing joint replacement surgery--a multicenter phase II clinical trial].

In order to evaluate the effect of recombinant human erythropoietin (rHuEPO) on autologous blood transfusion in patients with rheumatoid arthritis (RA), we performed a phase II clinical trial in 65 RA patients undergoing elective surgery. rHuEPO was administered subcutaneously once a week and after observing erythropoiesis, autologous blood was collected. Fifty-seven of the 58 patients who completed treatment responded to rHuEPO and could donate more than 400 ml of autologous blood. Among them, 23 out of 28 patients undergoing total hip arthroplasty, 27 out of 28 undergoing total knee arthroplasty and 1 out of 1 undergoing spinal surgery did not need homologous blood transfusion perioperatively. During rHuEPO treatment, no significant changes of clinical parameters of RA activity were observed. Two patients discontinued the treatment because of mild and transient side effects. These results indicate that subcutaneous rHuEPO is safe and effective in eliminate the need for homologous blood transfusion, even in anemic RA patients undergoing elective orthopedic surgery.

Relationship between basal metabolic rate, thermogenic response to caffeine, and body weight loss following combined low calorie and exercise treatment in obese women.

To clarify whether there were any differences in basal metabolic rate (BMR) and thermogenic response to caffeine in individual obese women, and if so, whether such differences affected weight loss, the basal and resting metabolic rates at 30 min after a caffeine loading test (4 mg/kg ideal body weight, per os) were measured in 136 obese women and ten lean age-matched controls. The obese subjects were then asked to follow a combined low calorie diet and exercise regimen. There were no differences in the BMR and thermogenic responses to caffeine between the obese and lean groups. However, the BMR and the thermogenic responses to caffeine varied widely in obese subjects. After two months of treatment, body weight and percentage body fat in obese women were significantly (P < 0.001) reduced. There were significant correlations between the BMR and body weight loss (r = 0.3621, P < 0.001), between BMR/lean body mass and body weight loss (r = 0.3196, P < 0.001) and between the thermogenic response to caffeine and body weight loss (r = 0.6943, P < 0.001). When the criterion of a BMR less than 3.10 kJ/min (less than two standard deviations below the mean of the age-matched lean control) was used to define an obese group with reduced BMR, there were 30 obese subjects in this group, and their body weight was significantly decreased by treatment.(ABSTRACT TRUNCATED AT 250 WORDS)

Recurrence of cystic adventitial disease in an interposed vein graft.

A case of cystic adventitial disease (CAD) of the popliteal artery with intermittent claudication in the left calf is reported. This patient was first treated by total excision of the cyst and the involved artery followed by graft interposition with an autogenous saphenous vein. Recurrence of CAD in the interposed vein graft was noted after 6 months, and excision of the cyst with the involved graft and graft interposition with an autogenous saphenous vein was again required. We consider that the definitive evidence from early recurrence in the interposed vein graft shown in this case will close the discussion of the cause of CAD. The cystic lesion is thought by some to originate from the adventitia; traumatic, embryologic, or systemic abnormality theories were proposed. We assume that the mucin-secreting synovial cells originating from the neighboring joint capsule, tendon sheath, or, in some cases, from the ganglion itself, directly invade the adventitia through any rough or injured crack caused by trauma to the adventitia or simply attach to and encircle the adventitia. Total resection of the lesion with grafting is the recommended treatment for CAD because of the excellent results. However, as shown in our case, complete removal of the synovial cells even by resection technique may be difficult; therefore intensive follow-up is fundamentally necessary in this disease.

[Changes in interpersonal values in student nurses: a longitudinal study].

Our cross-sectional studies of interpersonal values in female student nurses showed that (1) third graders attached more importance to the values of Support and Independence and less to those of Benevolence and Leadership than did first graders, and (2) the discrepancy between the ratings of what they were and those of what their ideal nurses were was greater in the third than in the first graders. We interpreted these differences between the two graders as indicating a developmental change brought about during the three years. This study examined the internal validity of this interpretation through the use of a longitudinal method. The KG-SIV (Kikuchi-Gordon Survey of Interpersonal Values) was administered twice to 85 female student nurses: immediately after entering their nursing schools and immediately before finishing them, with a testing interval of approximately three years. For each testing, subjects were asked to rate what they were and then what their ideal nurses were. Analyses produced almost the same results as obtained in cross-sectional studies: perceived values of Benevolence and Leadership decrease while those of Support and Independence increase over the three years, with the discrepancy increasing from the first to the second testing. Findings thus support the internal validity of our prior interpretation. The changes in interpersonal values found for the student nurses suggest a socialization process they undertake in nurse training and imply an increase of stress they experience in that process.

Scotopic threshold response in complete and incomplete types of congenital stationary night blindness.

PURPOSE: To study the function of the rod visual pathway in the complete and incomplete types of congenital stationary night blindness (CSNB), with special reference to the scotopic threshold response (STR) of electroretinograms (ERGs) METHODS: Using full-field stimuli with light intensities ranging from near absolute threshold to bright, ERG intensity series from two patients with complete CSNB, four patients with incomplete CSNB, and four normal subjects were recorded. RESULTS: Neither the rod b-wave nor the STR was recordable from the patients with complete CSNB. In the patients with incomplete CSNB, the STR was clearly recorded, although the absolute threshold was elevated in accordance with elevation of the psychophysical absolute threshold. The b-wave stimulus threshold was not elevated, and the b-wave amplitude near the threshold was normal. The peak time of the STR was delayed by approximately 80 msec, whereas that of the b-wave was normal. CONCLUSIONS: These STR results indicate that the rod system abnormality in complete CSNB differs from that in incomplete CSNB. Furthermore, the greatly delayed peak time of STR in the patients with incomplete CSNB made the interaction between b-wave and STR different from that in normal subjects.

Changes in tissue antioxidant enzyme activities and lipid peroxides in endotoxin-induced multiple organ failure.

Intravenous administration of bacterial endotoxin (lipopolysaccharide: LPS) induces shock and disseminated intravascular coagulation in rats. Our report here shows that LPS-administered rats (10 mg/100 g) develop tissue injuries and functional disorders in multiple vital organs. In the present study, we investigated changes in tissue antioxidant enzyme activities, neutrophil sequestration, and lipid peroxides in multiple organs (lung, stomach, small intestine for antioxidant enzyme activities and neutrophil sequestration; lung, stomach, small intestine, liver, abdominal aorta for lipid peroxides) of LPS-treated rats. LPS-treated animals morphologically revealed pulmonary interstitial edema, alveolar hemorrhage, and mucosal hemorrhage in the small intestine 45 min after LPS administration. Blood samples withdrawn from LPS-treated animals exhibited increases in serum amylase, blood urea nitrogen, creatinine, and transaminase levels up to 180 min post-LPS infusion. LPS-treated animals showed a significant increase in tissue myeloperoxidase (MPO) activities of the lung, but not of the small intestine and stomach 45 min after LPS infusion. Thiobarbituric acid reactive substances (TBARS) in the lung, small intestine, stomach, liver, and abdominal aorta significantly increased at 45 min post-LPS-infusion. Tissue superoxide dismutase (SOD) activities of the LPS-treated animals demonstrated a significant decrease in the lung, which suffered from severe insults and neutrophil sequestration; no significant change in the small intestine, which suffered from morphological insults without neutrophil sequestration, and a significant increase in the stomach, which showed no histological impairment, at 180 min post-LPS administration. Glutathione peroxidase (GSH-PX) activities of the lung and small intestine showed no significant change in LPS-treated rats, while those of the stomach revealed a marked increase.(ABSTRACT TRUNCATED AT 250 WORDS)

Sharing of the interleukin-2 (IL-2) receptor gamma chain between receptors for IL-2 and IL-4.

The gamma chain of the interleukin-2 (IL-2) receptor is an indispensable subunit for IL-2 binding and intracellular signal transduction. A monoclonal antibody to the gamma chain, TUGm2, inhibited IL-2 binding to the functional IL-2 receptors and also inhibited IL-4-induced cell growth and the high-affinity binding of IL-4 to the CTLL-2 mouse T cell line. Another monoclonal antibody, TUGm3, which reacted with the gamma chain cross-linked with IL-2, also immunoprecipitated the gamma chain when cross-linked with IL-4. These results suggest that the IL-2 receptor gamma chain is functionally involved in the IL-4 receptor complex.

Cloning and sequence analysis of cDNA encoding the bovine testis-derived male-enhanced antigen (Mea).

A full-length cDNA encoding the bovine male enhanced antigen (Mea) has been cloned from a bovine testicular cDNA library and sequenced. The primary structure of the bovine Mea peptide deduced from this nucleotide sequence has 174 amino acid residues and is highly homologous to human (95.9%, 165/172) and mouse (92.5%, 161/174) Mea gene products. It is located on an autosome, and is expressed highly in the testes.

Amino acid sequence surrounding the retinal-binding site in retinochrome of the squid, Todarodes pacificus.

Squid (Todarodes pacificus) retinochrome was reduced to N-retinyl protein with borane dimethylamine and cleaved by CNBr. The retinyl peptide was then isolated by chromatography while being monitored for absorbances at 215 and 330 nm, and the N-terminal amino acid sequence was determined to be Ser-Lys-Thr-Gly-X-Ala-Leu-Phe-Pro. This sequence was the same that we had observed at the 7th transmembrane domain of retinochrome whose structure was reported previously. During Edman degradation of the retinyl peptide, the yield of the PTH-lysine at the second cycle was lower than those of the other PTH-amino acids, proving that the lysine residue forms a Schiff's base with retinal (Lys-275 in retinochrome). The amino acid sequence surrounding the retinal-binding lysine in retinochrome greatly differed from those in a variety of known visual pigments. This fact would be associated with the difference in the photoisomerization of chromophore between retinochrome and rhodopsin. The protein structure of retinochrome is also compared with that of rhodopsin in Todarodes.

[Treatment results of breast cancer patients who hoped for breast preservation].

From 1983 to August 1990, 143 patients (148 breasts) were treated with breast conservation therapy in our department. Age ranged from 27 to 75 years (median: 45 years). Forty-one were stage I, 91 were stage II, 6 were stage III, and 5 were stage IV. Surgery consisted of lumpectomy and axillary dissection up to level II. When axillary lymph node(s) were pathologically involved (n(+)), chemotherapy (CMF) was performed. Radiation dose was 50 Gy to the entire breast, and booster irradiation was not performed. After October 1989, patients without palpable axillary node(s) underwent lumpectomy without axillary dissection. Instead, almost all patients started to receive chemotherapy, and not only the breast but also lymph node areas were also included in the radiation field. In 143 patients received conservation therapy, 5-year survival rate was 94%, and 5-year freedom from relapse rate was 81%, respectively. Microscopic surgical margin was positive in 39%. Excluding 5 stage IV patients, 14 relapsed out of 138 patients. Initial relapse sites were as follows: the breast in 3, the axilla 2, the distant organ in 10 patients. One patient relapsed in the axilla and the distant site simultaneously. Distant sites were the lung (4), the bone (4), the bone and the other organs (2). Three stage II patients showed in-breast recurrences. Microscopic surgical margins at initial lumpectomy were positive in one and negative in other 2 patients. One patient was salvaged with re-lumpectomy, and no relapse was seen after that.

Cloning of the mouse interleukin 2 receptor gamma chain: demonstration of functional differences between the mouse and human receptors.

We isolated a cDNA clone for the gamma chain of the mouse interleukin 2 receptor. Introduction of the mouse gamma chain cDNA clone into a mouse fibroblast cell line, L929, expressing the mouse alpha beta heterodimer IL-2 receptor converted pseudo-high affinity of the IL-2 receptor into functional high, resulting in internalization of IL-2 and induction of the c-myc, c-fos and c-jun genes. The mouse beta gamma heterodimer, however, failed to bind IL-2 unlike the human beta gamma heterodimer intermediate-affinity receptor. These results indicate that the mouse functional IL-2 receptor is a complex comprising three distinct subunits, alpha, beta and gamma chains, but the beta gamma heterodimer is not functional and different from the human heterodimer.

The role of active oxygen species and lipid peroxidation in the antitumor effect of hyperthermia.

The role of active oxygen species and lipid peroxidation in the antitumor effect of hyperthermia was studied in an experimental rabbit model. VX2 tumors were transplanted into rabbit hind legs, and the effect of hyperthermia on tumor growth was measured at 7 and 14 days after heating. As an index of lipid peroxidation, thiobarbituric acid-reactive substances in the tumor tissue were measured prior to hyperthermia and 3, 6, 12, and 24 h after hyperthermia. Tumor growth in rabbits treated with hyperthermia was significantly reduced, and thiobarbituric acid-reactive substances in the tumor tissue treated with hyperthermia were significantly increased until 6 h after hyperthermia. In addition, alpha-tocopherol in the tumor tissue was significantly decreased after hyperthermia. The antitumor effect of hyperthermia and the increase of thiobarbituric acid-reactive substances in the tumor tissue treated with hyperthermia were significantly inhibited by the administration of superoxide dismutase and catalase or dimethyl sulfoxide. These results suggest that lipid peroxidation mediated by active oxygen species plays an important role in the antitumor effect of hyperthermia.

Cloning and nucleotide sequence of cDNA for rhodopsin of the squid Todarodes pacificus.

A cDNA for rhodopsin was isolated from a library constructed from poly(A)+RNA of the squid (Todarodes pacificus) retina. One positive clone with the longest insert of cDNA (3.1 kb) was selected by employing a PCR-amplified cDNA fragment as a probe. The nucleotide sequence of the cDNA revealed a single open reading frame of 1,344 bp encoding a polypeptide (M(r)49,833), which covered a complete sequence for the squid opsin. This clone had a very long 3'-non-coding region (1.7 kb) including multiple polyadenylation signals, AATAAA, resembling the clones for Todarodes retinochrome and retinal-binding protein (RALBP). The analysis of hydropathicity demonstrated the presence of seven transmembrane spanning domains, and a possible retinal-binding site, Lys-305, was found in the 7th domain. Todarodes rhodopsin contained characteristic sequences of PPQGY repeated in the C-terminal region, as reported in Loligo and octopus rhodopsins. Structural comparison of those cephalopod rhodopsins is also discussed.

Carbonic anhydrase II deficiency in three unrelated Japanese patients.

Three Japanese patients with carbonic anhydrase II (CAII) deficiency from three families were described. The parents of one patient were unrelated, the parents of each of the other two patients were first cousins. All the patients had renal tubular acidosis, osteopetrosis, symmetrical cerebral calcification and mental retardation. They exhibited poor activity and poor appetite in the neonatal period, and then developed psychomotor retardation. Two of them were diagnosed as having osteopetrosis at 10 months and 36 years of age, respectively, and the other as having osteomalacia at 28 years of age. All patients had recurrent episodes of muscle weakness. The CAII enzyme activity and protein levels in red blood cells in each of the three patients were deficient. Their parents exhibited approximately 50% normal levels of CAII activity and protein. This is the first report of patients with CAII deficiency in the Japanese population.
