Further simplification of ultrasensitive enzyme immunoassay (immune complex transfer enzyme immunoassay) for anti-HTLV-I IgG using microplates and fluororeader.

In a previously reported ultrasensitive enzyme immunoassay (immune complex transfer enzyme immunoassay) for anti-HTLV-I IgG, polystyrene beads were handled with tweezers, and bound beta-D-galactosidase activity was measured with a fluorometer. The use of tweezers was causative of false-positivity by carryover, and testing many samples was difficult. Recently, these drawbacks have been minimized using microplates and a fluororeader. However, tweezers were still required in the initial and final steps. In the present study, the immune complex, comprising 2,4-dinitrophenyl-antigen, anti-HTLV-I IgG, and antigen-beta-D-galactosidase conjugate, was formed in and trapped onto microplate wells coated with (anti-2,4-dinitrophenyl group) IgG. Subsequently, the microplate wells were incubated with epsilon N-2,4-dinitrophenyl-L-lysine and polystyrene beads, modified by attaching to plates through cylindrical bars and coated with (antihuman IgG gamma-chain) IgG, to transfer the immune complex from the microplate wells to the modified polystyrene beads. Alternatively, modified polystyrene beads coated with (anti-2,4-dinitrophenyl group) IgG and microplate wells coated with (antihuman IgG gamma-chain) IgG were substituted for microplate wells coated with (anti-2,4-dinitrophenyl group) IgG and modified polystyrene beads coated with (antihuman IgG gamma-chain) IgG, respectively. The fluorescence intensity for bound beta-D-galactosidase activity was quickly measured with a fluororeader. Thus the modified polystyrene beads were transferred from wells to wells more quickly and easily without tweezers, eliminating false-positivity due to carryover, and it became easy to test many samples with high sensitivity and reliability, although the assay of bound beta-D-galactosidase activity became slightly more time-consuming.

PSA assay of dried blood samples from the ear lobe on a filter paper with special reference to prostatic mass screening.

To eliminate complicated procedures for taking blood samples in the field work for prostatic mass screening, we developed a method for PSA testing using blood samples taken from the ear lobe and dried on a filter paper. The stability, the diagnostic efficacy, and the cost-benefit effect of the method were confirmed in hospitalized patients, as well as in subjects from our mass screening program.

Biphasic pressor responses to norepinephrine in humans.

We investigated pressor responses to intravenous bolus infusion of norepinephrine in seven healthy volunteers. Norepinephrine (1, 2, and 4 micrograms/kg) elevated blood pressure in a concentration-dependent manner and decreased heart rate. The pressor response to norepinephrine was biphasic (early and late). Intravenous administration of phentolamine (10 mg) completely abolished the pressor response to norepinephrine, and prazosin (5 mg, given orally) inhibited the early and late responses to a same extent. Continuous intravenous infusion of nicardipine (2 micrograms/kg/min) inhibited the late pressor response but not the early one. These data suggest that bolus infusion of norepinephrine evoked the biphasic pressor response and that the late response depends on vasoconstriction by calcium influx through nicardipine-sensitive calcium channels.

Down-regulation of Th2 cell-mediated murine peritoneal eosinophilia by antiallergic agents.

Local eosinophilia has been linked to the pathogenesis of the inflammatory aspect of allergic diseases. The present study found that co-injection of D10G4.1 (D10) cells, a murine Th2 clone, with conalbumin (CA) into the peritoneal cavity of AKR/J mice increased the number of peritoneal eosinophils. The accumulation of eosinophils reached a maximum level at 24 to 48 hr and was accompanied by a marked increase in the number of neutrophils and a minor increase in the number of mononuclear cells. D10-induced peritoneal eosinophilia was suppressed by administration of either anti-IL-4 and anti-IL-5 monoclonal antibodies in an additive manner or by cyclosporin A (CsA). Interestingly, suplatast tosilate (IPD-1151T), known to be antiallergic agent capable of suppressing IgE synthesis and chemical mediator release, but not disodium cromoglycate, selectively suppressed eosinophil accumulation. Taken together with the observation that CsA and IPD-1151T suppressed IL-4 and IL-5 production by CA-stimulated D10 cells in vitro, the present results strongly suggest that agents capable of down-regulating Th2 cell cytokine production may attenuate allergic inflammation by impairing the recruitment of eosinophils that is mediated by Th2 cells.

Identification of rat urinary kinin as bradykinin.

Bradykinin (BK)-like activity, which was detected by BK-enzyme-immunoassay, was purified from 80 ml of ureter urine of Sprague-Dawley rats by Sephadex G 25 chromatography, FPLC, and reversed phase HPLC. The purified kinin fraction showed the same retention time as authentic BK on HPLC and produced contraction of isolated rat uterus, the contraction being suppressed by a B2-antagonist Hoe140. There was no other kinin detected on the HPLC at the corresponding retention time to kallidin, arginyl-BK or T-kinin. The peptide showed an amino acid sequence identical to that of BK by amino acid sequence analysis.

Use of prostate-specific antigen and tumor volume in predicting needle biopsy grading error.

OBJECTIVES: To study the discrepancy between histologic grades of needle biopsy and prostatectomy specimens and to define the potential role of prostate-specific antigen (PSA), PSA density, and tumor volume in predicting the tumor grade. METHODS: Histopathologic grades on needle biopsy and subsequent radical prostatectomy specimens were compared in 135 patients with clinically organ-confined prostate cancer. The frequency of undergrading (difference of one or more grades) by needle biopsy was then compared to PSA, PSA density, and other preoperative parameters. RESULTS: The frequency of undergrading by one level was 45% using the M. D. Anderson Hospital (MDAH) system and 47% using the Gleason system. When the tumor grades were classified into two categories, undergrading rates of 26% (low to high grade by the MDAH system) and 21% (moderately well to poorly differentiated by the Gleason system) were noted. Serum PSA and PSA density correlated with the grading error rate: the higher the PSA and PSA density values, the higher the biopsy undergrading rate. PSA showed a significant correlation with undergrading (P = 0.0019). Undergrading occurred in 32% of patients with a PSA more than 4 and 10 ng/mL or less and in 47% of those with a serum PSA more than 10 ng/mL. In all cases when the PSA was 4 ng/mL or less, the needle biopsy results correctly predicted the final tumor grade category. Tumor volume was not associated with frequency of undergrading. CONCLUSIONS: The ability to predict tumor grade can be enhanced by considering the serum PSA value.

Suppurative lesions without prominent epithelioid cell response in abscess-forming granulomatous lymphadenitis.

To clarify the clinicopathological significance of the suppurative lesions without an epithelioid granulomatous response (SLs without Ep) in lymph nodes and their relationship to abscess-forming granulomatous lymphadenitis (AGL) and cat scratch disease (CSD), 10 cases were assessed clinicopathologically and immunohistologically. SLs without Ep were located in the subcapsular sinus, paracortical area and medullary cords, but not in the germinal centers. The microabscesses were surrounded by collections of monocytoid B-lymphocytes (MBLs), histiocytes without epithelioid features, neutrophils, small lymphocytes and small numbers of plasma cells. The majority of the MBLs seen in the SLs without Ep were of the large cell type. The histological triad of toxoplasmic lymphadenitis, i.e., reactive follicular hyperplasia, small clusters of epithelioid cells and aggregates of MBLs, were also seen in all cases. Some of the clinical and pathological findings in our 10 cases were characteristic of CSD, i.e., (1) cat exposure before the lymphadenopathy was in four of the 10 cases, (2) occurrence in autumn and winter months in all cases, (3) typical suppurative granulomas surrounded by palisaded epithelioid cells were in four of the 10 cases, and (4) Warthin-Starry silver stain-positive bacteria were detected in seven of the 10 cases. The results of our study suggest that SLs without Ep are an early stage of CSD.

Effects of replacement of Lys25 with Gln on the conformation of ribonuclease T1: sequence-specific 1H NMR resonance assignments of Gln25 ribonuclease T1 by two-dimensional NMR spectroscopy.

Two isozymes of ribonuclease (RNase) T1 exist in nature, i.e. Gln25 RNase T1 and Lys25 RNase T1. Gln25 RNase T1 is less stable than Lys25 RNase T1, although the enzymatic activity is not distinguishable between these two isozymes. To elucidate the effects of the replacement of Lys25 with Gln on the conformation and microenvironments of RNase T1 in detail, two-dimensional NMR spectra were measured, sequence-specific 1H NMR resonance assignments of Gln25 RNase T1 were performed, and then the determined parameters and microenvironments of Gln25 RNase T1 were compared with those of Lys25 isozyme [Hoffmann, E. and Rüterjans, H. (1988) Eur. J. Biochem. 177, 539-560]. The main chain protons were assigned for 101 out of the total of 104 amino acid residues. Secondary structure elements were identified from analysis of characteristic NOE patterns, interstrand NOE connectivities, and hydrogen-deuterium exchange rates of main chain amide protons. The results indicated that Gln25 RNase T1 contains a single alpha-helix and seven beta-strands. The secondary structure of Gln25 RNase T1 is, thus, essentially the same as that of Lys25 RNase T1. On the other hand, comparison of the conformation-dependent shifts of Gln25 RNase T1 with these of Lys25 RNase T1 showed that the replacement of Lys25 with Gln has significant effects on the C-terminal part of the alpha-helix region and the base-binding site. These results may indicate that the base-binding site is relatively flexible in the RNase T1 molecule. Among the residues of the C-terminal part of the alpha-helix region, the protons of Asp29 were most affected in terms of their chemical shifts, which may indicate that the side chain carboxylate anion of Asp29 is the counterpart of the electrostatic interaction of Lys25 in Lys25 RNase T1. The Gln25 of Gln25 RNase T1 may have little or no interaction with Asp29, and this may be the reason why Gln25 RNase T1 is less stable than the Lys25 isozyme.

Effect of intravenous calcium antagonists on left ventricular diastolic function in hypertension: assessed by pulsed-Doppler echocardiogram.

OBJECTIVE: To investigate the acute effects of the calcium antagonists nicardipine and diltiazem on blood pressure and left ventricular diastolic filling in hypertensive patients. DESIGN: Transmitral flow velocities were measured by pulsed-Doppler echocardiography before and during infusion of nicardipine (1, 2 and 3 micrograms/kg per min) and diltiazem (5, 10 and 15 micrograms/kg per min) in seven patients with essential hypertension. Arterial pressure was measured directly, and other haemodynamic indices were obtained using a right-sided heart catheter. RESULTS: The calcium antagonists significantly decreased blood pressure and total peripheral resistance. Changes in mean blood pressure correlated with those in total peripheral resistance. Heart rate was significantly increased by nicardipine but decreased by diltiazem. Nicardipine and diltiazem both increased significantly the peak early filling velocity and the peak velocities of early: late filling ratio. The changes in the ratio correlated inversely with those in total peripheral resistance. CONCLUSION: Intravenous infusion of nicardipine or diltiazem causes a fall in blood pressure by decreasing total peripheral resistance, with beneficial effects on left ventricular diastolic filling that are independent of changes in heart rate in patients with hypertension.

Depressed sodium-calcium exchange in human umbilical arteries from women with preeclampsia.

Responses to low-sodium solution were studied in helical preparations of human umbilical arteries from women with preeclampsia and age-matched normotensive women. Reduction of extracellular sodium concentration (replaced by N-methyl-D-glucamine, NMG) and consequent restoration of extracellular sodium-evoked contractions and relaxations, respectively, in the arteries pretreated with phenoxybenzamine, caffeine, ryanodine, and verapamil. The rate and magnitude of the contraction induced by low-sodium solution and the rate of the relaxation induced by the restoration were less in preeclampsia than in normotension. Therefore, the activity of Na-Ca exchange is depressed in human umbilical arterial smooth muscle in preeclampsia, suggesting a possibility that the depressed activity leads to an abnormal regulation of arterial tone in preeclampsia.

Clinicopathologic study of primary gastric lymphoma of B cell phenotype with special reference to low-grade B cell lymphoma of mucosa-associated lymphoid tissue among the Japanese.

Resection specimens from 83 patients with primary gastric lymphoma (PGL) of B cell phenotype at stage IE and at stage IIE according to the Ann Arbor classification were investigated. Histologically, these lymphomas could be divided into four types: Type I lesions (n = 24) were entirely made up of MALT lymphoma; Type II lesions (n = 13) were predominantly MALT lymphoma containing one to a few foci of high-grade B cell lymphoma; Type III lesions (n = 22) consisted largely of high-grade lymphoma with small areas of low-grade MALT lymphoma; and Type IV lesions (n = 24) were pure high-grade B cell lymphoma, mostly of the large cell type. All patients had undergone primary gastric resection, and 14 received additional chemotherapy (n = 12), or both chemotherapy and radiotherapy (n = 2). The survival probability was significantly higher for Types I and II lymphomas than for Types III and IV tumors (P < 0.05 by the generalized Wilcoxon test). According to The General Rules for the Gastric Cancer Study by the Japanese Research Society for Gastric Cancer, the stage of disease showed a clear distinction between each of them (P < 0.01 by the generalized Wilcoxon test). This staging method seemed to serve well as a prognostic indicator. The histological typing of the PGL of the present series also seemed to correlate with the gross appearance, pathologic stage and prognosis. Furthermore, the expression of cyclin D1, bcl-2 and p53 protein, and PCNA was immunohistochemically investigated in 42 cases of the present series. Most of the low-grade PGL (Types I and II) had less than 60% PCNA-positive cells, whereas the high-grade PGL (Types III and IV) had more than 60% positive cells. In a study for cyclin D1 protein, no cases showed the nuclear staining pattern characteristic for mantle cell lymphoma, and the cytoplasmic staining frequently observed in the node-based large B cell lymphoma was seldom identified in the PGL. This discrepancy might suggest a lineage difference among the morphologically similar, but site-different, lymphomas. On the other hand, bcl-2 protein overexpression was almost equal in frequency between the gastric and node-based high-grade B cell lymphomas. This is in contrast to the reports from Western countries, in which the majority of high-grade gastric tumors were bcl-2 negative.

Primary malignant lymphoma of the intestine: clinicopathologic and immunohistochemical studies of 39 cases.

Clinicopathologic and immunohistochemical features in 39 cases of primary intestinal non-Hodgkin's lymphoma (NHL) in Japanese patients were studied. Only resection materials in state IE and IIE-1 were included in this study because of the certainty that the intestine was the primary site of the lymphoma. The updated Kiel classification was used to classify NHL. Histologically, only two cases (5.1%) were follicular lymphomas, and the others were diffuse lymphomas. Twenty-eight patients (71.8%) had high-grade NHL and 11 (28.2%) had low-grade NHL. Twenty (71.4%) of the 28 high-grade NHL were centroblastic lymphomas, and 14 (70.0%) of these 20 cases of centroblastic lymphoma were the polymorphic variant. Ten (90.9%) of the 11 low-grade NHL were low-grade mucosa-associated lymphoid tissue (MALT) lymphomas. Macroscopically, 18 patients had polypoid masses, 17 ulcerative tumors and four had diffusely infiltrating NHL. Seven of the 10 low-grade MALT lymphomas were polypoid masses. Immunohistochemically, 35 lesions (89.7%) were of the B cell phenotype and three (7.7%) were of the T cell phenotype. In the remaining case, the cell lineage could not be determined. No lesions were considered to be of histiocytic origin. The 5 year survival rate for high-grade B cell lymphomas was poorer than for low-grade B cell lymphomas, and the present study indicated that the histological grade of the intestinal B cell lymphomas was a prognostically significant factor.

Kinetic analysis of prostatic volume in patients with stage D prostatic cancer treated with LHRH analogues in relation to prognosis.

OBJECTIVE: To evaluate the clinical usefulness of the kinetic analysis of prostatic volume in the prognosis of patients with Stage D prostatic cancer treated using luteinizing hormone-releasing hormone (LHRH) analogues. PATIENTS AND METHODS: The reduction of prostatic volume was monitored in 12 patients with Stage D prostatic cancer using transrectal ultrasonography (TRUS) after treatment with LHRH analogues. Data obtained from the kinetic analysis of prostatic volume were compared with the prognosis. RESULTS: All the patients having a reduction time, tau (derived from the kinetic analysis of prostatic volume change with time), of less than 41 days had neither clinical progression within 15 months nor death caused by prostatic cancer during the 5-year follow-up, while the disease-specific 5-year survival rate in patients having a tau of greater than 42 days was as low as 17%. The difference in both the progression and disease-specific survival between these groups was statistically significant (P < 0.05) despite the limited number of patients. In contrast, conventional prognostic parameters showed no significant predictability for prognosis with the exception of prostatic acid phosphatase, which correlated strongly with the occurrence of progression within 15 months. CONCLUSION: The kinetic analysis of the change of prostatic volume using TRUS shows promise in the prognosis of the patients with Stage D prostatic cancer.

A simple and effective video keratometric system.

We tested a personal computer-based video keratometric system used to analyze projected placido disk rings. The system consists of a Maloney surgical keratometer that projects the rings and a video camera attached to an operating microscope. Images of the rings were transmitted to a video image processing board and analyzed on a personal computer using an analysis program we developed. System precision was evaluated in 18 eyes. The values were compared with those obtained using a commercial photokeratometer (control). In all eyes, the principal meridian values differed from the controls by < 0.04 mm. In 15 eyes (83%), the measurements were within 0.03 mm of the controls. The corneal astigmatism values and cylindrical axes were within 0.12 diopters in 17 eyes (94%) and within 5 degrees in all cases, respectively. Our system, which can analyze placido disc rings projected from other systems, is sufficiently precise to measure the corneal radius of curvature and astigmatism.

In vivo morphological changes in rat lenses induced by the administration of prednisolone after subliminal X-irradiation. A preliminary report.

In order to induce steroid cataracts in rat lenses, prednisolone acetate was administered together with a single subliminal dose of X-irradiation, which was applied unilaterally before steroid application started. The rats were divided into a control group (without prednisolone acetate administration), a group with a topically administered daily dose of 1 mg/kg prednisolone acetate suspension and a group with a systemically applied daily dose of 0.8-1.0 mg/kg prednisolone acetate suspension. Changes in the lens were objectively evaluated with in vivo Scheimpflug slit images during a 30-week period. Although body weight increase was significantly affected in both groups administered prednisolone, animals survived until the end of the observation period. The initial changes in the lens were the dissociation of the Y-suture and a slight increase in scattering light intensity at the posterior embryonic nucleus after both topical and systemic steroid administration.

Morphological and biochemical changes in lenses of guinea pigs after vitamin-C-deficient diet and UV-B radiation.

The effect of ultraviolet B (UV-B) radiation and a vitamin-C-deficient (VCD) diet on guinea pig lenses was investigated. The initial lens changes in the VCD group were observed by slit-lamp examination 6 weeks after the start of the VCD treatment; after 12 weeks the changes in the posterior subcapsular region became more prominent, and the dissociation around the posterior suture became wider and slightly deeper toward the posterior cortex. The high concentration of lens oxidized glutathione (GSSG), and the low ratio of reduced glutathione (GSH) to oxidized glutathione (GSH/GSSG) on the lens posterior region correlated with density changes in the corresponding layers as measured by Scheimpflug images with linear microdensitometry. It is suggested that the strong oxidative stress of the VCD diet caused the damage at the posterior part of the lens. UV-B radiation appeared to accelerate cataract progression in lenses that lack vitamin C.

Topographic distribution of prednisolone in the lens after organ culture.

Topographic distribution of prednisolone in the lens after organ culture was investigated. Freshly enucleated pig lenses were divided into two groups; the complete vitreous was removed from the lenses in group A, and the adherent parts of the vitreous were left in group B. Groups A and B were each divided into two subgroups which were separated according to drug application: two groups underwent drug application by a divided chamber (Iwata, 1982, DC) and two groups by a closed chamber. The incubation medium was TCM-199 with 2 micrograms/ml prednisolone, and the incubation was performed for 8 h. In order to know the participation of the vitreous in drug penetration into the lens, prednisolone was applied by a divided chamber from the anterior or posterior side, with or without vitreous, and incubated for 4 h as a first incubation. A secondary incubation was performed for another 4 h with a medium that did not contain prednisolone. After the lens incubation, the lenses were separated into single lens layers, and the drug concentration of each lens layer was measured by high performance liquid chromatography. The highest prednisolone concentration in the lens was detected by a closed chamber in the equatorial part followed by the posterior cortex, anterior cortex and nucleus. There were no significant differences in drug concentration due to the direction of drug application or the absence or presence of vitreous. The prednisolone distribution pattern in the lens by DC depended on the direction from which the drug was applied. The highest drug concentrations were detected in the lens cortex of the drug-applied side followed by the equator. No drug was detected in the side of the lens opposite the drug-applied side or in the nucleus. When a drug was applied from the posterior lens side, the presence of the vitreous made a big difference in the amount of drug penetration. The comparison of drug concentration between the absence or presence of the vitreous after secondary incubation in a medium that did not contain prednisolone indicated that the vitreous participates in the drug kinetics of the lens. The experiments suggest that while prednisolone penetration into the posterior lens part is small compared to that of the anterior part, when the steroid penetrated into the posterior part it remained there for a long period.
