Multiple paraganglioneuromas.

We report multiple paraganglioneuromas which occurred in a 40-year-old-man. Thirty-two tumours with similar histological appearance have been reported previously and most of them showed a striking predilection to occur in the second portion of the duodenum. In this case, three masses were detected; one was located in the periampullary region of duodenum with a polypoid appearance, the others were well defined masses in peri-pancreatic tissue adjacent to large vessels. Histology revealed two cellular components, epithelioid cells with NSE immunoreactivity and S-100 protein containing spindle-shaped cells. Moreover, on electron microscopical examination, three different epithelioid cell types were seen. Type I was a figure differentiating to ganglion cells, type II to paraganglion cells, type III was a hybrid form of ganglion and paraganglion cells. Paraganglioneuroma revealed the histopathology of ganglioneuroma, paraganglioma and also a mixed appearance. In this respect the tumour may be considered to originate in undifferentiated neural crest cells and develop organoid differentiation.

Ultrastructural study on a severe infantile sialidosis (beta-galactosidase-alpha-neuraminidase deficiency).

This study was undertaken to elucidate ultrastructural changes in a severe infantile sialidosis. The materials examined in this study consisted of biopsied rectal mucosa and autopsied small intestine, liver and kidney. In the biopsy sample, axons contained a number of pleomorphic electron dense bodies, and numerous membrane-bound vacuoles were found in Schwann's cells, fibroblasts, endothelial cells, lymphocytes and plasma cells. In autopsy samples, neurons in Auerbach's myenteric plexus of the small intestine were filled with a number of membranous cytoplasmic bodies, pleomorphic dense bodies and vesicles containing dense materials. Hepatocytes in the liver, and glomerular and tubular epithelial cells in the kidney were also extended by a number of membrane-bound vacuoles. These ultrastructural changes in severe infantile sialidosis closely resemble those in GM1-gangliosidosis type 1.

Familial occurrence of oligomeganephronia.

Oligomeganephronia (OMN) is a rare, renal hypoplasia, consisting of a reduced number of hypertrophied nephrons. This disorder has been considered to be a congenital but not a genetic disease. We describe the first report, to the best of our knowledge, of familial cases of OMN; two male siblings ran rapidly downhill courses and died 11 and 8 days after birth, respectively. In addition, the two patients had similar multiple anomalies; microcephaly, prominent glabella, hypertelorism, antimongoloid slant, epicanthal folds, broad nose, cleft lip and palate, down-turned mouth, short philtrum, micrognathia, low set ears, hypospadias, and cryptorchism. Although the patients and the parents had normal G-banded karyotypes, 4p monosomy syndrome is suggested from clinical features. The implications of this are discussed briefly.

Fatal case of pyruvate dehydrogenase deficiency.

A Japanese neonate with fatal pyruvate dehydrogenase deficiency is described. The patient lapsed into a coma shortly after birth with severe metabolic acidosis caused by accumulation of lactate and pyruvate. Hyperammonemia was also present and found to be the cause of the coma. Despite intensive treatment, the patient died at 93 hours of age. Enzyme study showed that the activity of pyruvate dehydrogenase was not detected in either the liver or the kidneys. This is the third reported case of fatal pyruvate dehydrogenase deficiency and hyperammonemia is described for the first time in this condition.

Plasma vasopressin response to peripheral administration of angiotensin in conscious rats.

To assess a role for peripherally administered angiotensin II (ANG II) in regulating vasopressin (antidiuretic hormone, ADH) release, the effects on plasma ANG II and ADH of intraperitoneal injections of ANG II dissolved in various solutions were examined in conscious rats. Plasma ANG II and ADH were determined by radioimmunoassay using the trunk blood collected after decapitation. Injections of 150 mM NaCl containing ANG II (6, 12, or 24 micrograms X 2 ml-1 X 100 g body wt-1) caused dose-related increases in plasma ANG II 15 and 30 min after, but plasma ADH remained unchanged. The lack of effect on plasma ADH of the ANG II dissolved in isotonic saline was also confirmed in another series of experiments in which the solution with a higher ANG II concentration was loaded by much smaller injection volume (14.3 micrograms X 0.1 ml-1 X 100 g-1). However, when given together with 600 mM NaCl, ANG II (8 micrograms X 2 ml-1 X 100 g-1) significantly potentiated the plasma ADH response to the vehicle at 15, 30, and 60 min, without affecting those of plasma osmolality, sodium, and hematocrit. The elevations of plasma ANG II and osmolality brought about by the treatment were comparable with those previously observed in rats deprived of water for 46 h. ANG II was without effect on the plasma ADH responses to the intraperitoneal injections of hypertonic sucrose or mannitol solution that did not alter plasma sodium, although these solutions were equipotent to 600 mM NaCl in augmenting plasma ADH and osmolality.(ABSTRACT TRUNCATED AT 250 WORDS)

In vivo clastogenicity of lead chromate in mice.

Cytogenetical effects of lead chromate were evaluated in vivo. When 0.5, 1, 2 and 4 g/kg body weight of lead chromate suspensions in 0.5% gum arabic were administered i.p. to C57BL/6N male mice twice, the frequencies of micronucleated polychromatic erythrocytes in the femoral bone marrow were significantly (p less than 0.01) different from that in the vehicle control. Moreover, as the dose increased, the ratio of the polychromatic to normochromatic erythrocytes decreased in a dose-response fashion (p less than 0.05). Thus, it is concluded that lead chromate in vivo gives rise to chromosomal damage as well as disturbance in hematopoiesis.

[Multitherapy of advanced esophageal carcinoma with special reference to pre- and post-operative administration of cis-platinum].

We discussed the results of multitherapy after pre- and post-operative administration of cis-platinum (CDDP) in resected cases of advanced esophageal carcinoma. The patients were, pre-operatively, dosed in the following ways; 1) CDDP (50 mg, 1/week) X 2-3, 2) CDDP (50 mg, 1-week) +BLM (15 mg, 1/week) X 2-3 or +MTX (30mg, 1/week) X 2-3. The patients under went surgery 1 to 2 weeks later. Post-operative administration of CDDP + MTX, or CDDP + BLM was performed 3-4 times about 3 weeks after surgery. Intra-operative and/or post-operative irradiation was also performed. CDDP was administered pre-operatively in 11 cases who ranged in age 45 to 71. Regarding stage, 2 were stIII and 9 were stIV; and curabilities where cIII in 2 cases, c I in 5 and c 0 in 4. With regard to surgery, the right thoracic and abdominal approach was adopted in 7 cases and the left thoracic and abdominal approach adopted in 3. Blunt dissection was performed in 1 case. We have never experienced post-operative complication of histological effects noticed in resected specimens, Ef1 was evaluated in 3/11 and Ef2 was noticed in 2/11. Patients who achieved an Ef2-1 effect due to the administration of CDDP as part of the multitherapy made satisfactory progress. On the other hand, patients who did not react to CDDP and who had marked vascular invasion died after a short period.

Antitumor activity of Sphaerotilus natans and its slime fraction in mice.

Antitumor activity of the whole cell and slime of an aquatic sheathed bacterium, Sphaerotilus natans IAM 12068, against ascites form of Ehrlich carcinoma in ddY mice was investigated. Intraperitoneal injection of whole cells and the slime fraction showed remarkable antitumor activity against mice inoculated with 10(4) to 10(5) tumor cells, the slime fraction being more effective. To examine the chemical nature of the active principle in the slime fraction, separation by Sepharose 4B gel filtration was carried out and two fractions designated as GF-P-1 and GF-P-2, which are mainly composed of protein, carbohydrate, and lipid, were obtained. GF-P-1 fraction, which contains large amounts of fucose and unidentified sugar as neutral sugar, showed marked antitumor activity at half the dose of the slime fraction, whereas the antitumor activity of GF-P-2, which is composed mainly of protein, was weak. This finding indicates that GF-P-1 fraction of S. natans slime may be a main active principle. The consistently demonstrable antitumor activity of GF-P-1 was abrogated by treatment of mice with silica, an anti-macrophage agent, suggesting that the antitumor activity of GF-P-1 depends on the activation of macrophages.

[Metastatic intracerebral malignant fibrous histiocytoma from the lung].

Malignant fibrous histiocytoma usually develops in the extremities, peritoneal or retroperitoneal space and the thigh. The occurrence in the lung and brain is quite rare. We report two cases of metastatic intracerebral malignant fibrous histiocytoma from the lung. The first patient developed cerebral metastasis six years after treatment of the primary site, and the second one died two months after the diagnosis of generalized metastases. Both were quite resistant to radiation therapy.

Amplification and enhanced expression of cellular oncogene c-Ki-ras-2 in a human epidermoid carcinoma of the lung.

The level of c-Ki-ras-2-specific mRNA was found to be markedly enhanced (10- to 20-fold) in a human epidermoid lung carcinoma transplanted into nude mice, compared with that in other lung carcinomas. Analysis of DNA revealed that c-Ki-ras-2 gene was amplified approximately 10-fold in this carcinoma, while c-Ha-ras, c-myc and c-sis were not amplified. Chromosome abnormalities were also observed in this carcinoma.

[Clinical application of local hyperthermia combined with antineoplastic agents].

At present hyperthermia shows great promise when combined with other modalities. Local thermo-chemotherapy may also be a very attractive research field, but the combination of hyperthermia and anti-cancer drugs is still poorly understood. In this review, the background and the clinical reports on heat and chemotherapy which have been reported during the last year were analyzed. Finally, our clinical experiences with local hyperthermia combined with anti-cancer drugs for gastrointestinal cancer were also reported.
