Human chorionic gonadotropin in the rectosigmoid colon. Immunohistochemical study on unbalanced distribution of subunits.

For investigation of cellular localization of subunits of human chorionic gonadotropin (hCG) in normal and neoplastic rectosigmoid colon, immunohistochemical studies were performed on nonneoplastic colons (10 fetuses, 3 infants, and 23 adults, including 4 cases of ulcerative colitis) and 7 carcinoid tumors, 19 adenomas, and 50 carcinomas of the rectosigmoid colon. The alpha-hCG immunoreactive cells were present in endocrinelike cells of non-neoplastic glands (6 fetuses older than 14th gestational week, 3 infants and 19 of 23 adults). Many of the positive cells were argyrophilic, and all were nonimmunoreactive for beta subunits of glycoprotein hormones. alpha-hCG immunoreactivity was also present in many argyrophilic cells of all carcinoid tumors and in some of the endocrine cell micronests. The immunoreactive cells for isolated beta-hCG were found in 14 infiltrating carcinomas. The distribution of hCG subunits was unbalanced, and both subunits may be expressed through an independent mechanism, commonly in normal and neoplastic rectosigmoid colon.

So-called "papillary and cystic neoplasm of the pancreas." An immunohistochemical and ultrastructural study.

A case of so-called "papillary and cystic neoplasm of the pancreas" (PCNP) was reported and investigated immunohistochemically and ultrastructurally. A tumor of the pancreatic head in a 21-year-old female was curatively resected. The tumor was cystic and histologically consisted of uniform cells in papillary and solid structure. Although there was no immunoreactivity for pancreaticogut hormones or secretory products of the pancreas in the tumor cells, most of the tumor cells were diffusely immunoreactive for neuron-specific enolase (NSE). Some neurosecretory granules were detected in the tumor cells ultrastructurally. Both facts suggested endocrine cell character of the tumor. Certain cases of PCNP might show a differentiation to endocrine cells.

[The subrenal capsule assay modified with three-dimensional measurement (volume method) and its higher predictivity].

Subrenal Capsule Assay (SRCA) developed by Bogden et al. is a new in vivo sensitivity test for anticancer agents. However, the presence of host reaction on about day 6 is largest problem for this method, and now many researchers are scrutinizing or introducing several modifications for this assay chiefly by immunosuppressing procedures. As one of the means to measure the actual change of the tumor volume and minimize the misreading of host reactions, we introduced three dimensional measurements of the grafts (volume method) instead of original mean diameter measurement. Using this method, prospective quantitative study was made. The results of 16 assays with 15 cases of miscellaneous malignancies showed 94% evaluability and in 22 treatments predictive accuracy was 77%. Assay/clinical correlation coefficient was 0.38 and coincidence of ranking order of the effectiveness was 6/6, with good contrast to those of 78%, 56%, 0.00 and 1/3, by original method respectively. Clinical responses more than PR were obtained in 9 of 15 cases. Two cases are still in CR for two years by assay directed single agent treatments. Many other minor modifications were also adopted and evaluated by quantitative observation of assay/clinical correlations. SRCA with our modifications seemed to have utilities than original.

Malignant peritoneal mesothelioma as a pelvic mass. Application of athymic mice in clinical pathology.

We reported herein an unusual presentation of malignant peritoneal mesothelioma which appeared as a solid pelvic tumor in a 73-year-old man. The solid tumor was confined in the pelvic cavity, except for one tiny disseminated nodule on the mesocolon. The pelvic tumor was resected with whole pelvic organs en bloc. Histologically, the tumor consisted of tubular and sarcomatous structures, suggesting the biphasic malignant mesothelioma. One of the transplanted tumors in athymic mice was cystic and its histology had striking similarities to cystic mesothelioma. Transplantation of human neoplasms to athymic mice may be able to reproduce histological diversity of the tumor, even if it is not fully expressed in the original tumor.

A case of malignant cystic endocrine tumor of the pancreas.

A malignant pancreatic endocrine tumor with a cystic appearance in a 66-yr-old woman was reported. Total pancreatectomy was performed under the diagnosis of cystadenocarcinoma. The tumor was large and cystic, but the solid portion of the tumor showed histologically a ribbon-like array of small uniform cells typical for endocrine tumors. Immunohistochemically, most tumor cells were immunoreactive for neuron-specific enolase, chromogranin, and Leu 7-epitope. Ultrastructurally, innumerable neurosecretory granules were demonstrated in the cytoplasm of most tumor cells. The patient survived for 5.5 yr after operation despite liver metastases, some of which also showed cystic appearances.

Complete amino acid sequence of the large subunit of the low-Ca2+-requiring form of human Ca2+-activated neutral protease (muCANP) deduced from its cDNA sequence.

The complete amino acid sequence of the large subunit (catalytic subunit) of human low-Ca2+-requiring-calcium-activated neutral protease (muCANP) was deduced from its cDNA base sequence. It is composed of 714 amino acid residues and its sequence is highly homologous to the chicken CANP sequence determined previously. Human muCANP, like chicken CANP, has a clear 4-domain structure, and their fundamental structures are essentially the same, although their Ca2+ sensitivities are significantly different. The role of each domain in the Ca2+ sensitivity and protease activity of CANP is discussed on the basis of sequence comparison.

Amylase in human lungs and the female genital tract. Histochemical and immunohistochemical localization.

We investigated the localization of amylase in normal human lungs and the female genital tract using immunohistochemical and histochemical methods. Immunohistochemical procedures were applied to formaldehyde-fixed, paraffin-embedded specimens as well as to cryostat sections of periodate/lysine/paraformaldehyde (PLP)-fixed tissues. The starch-substrate-film method was used for the histochemical investigation of unfixed frozen sections. Amylase immunoreactivity was observed in ciliated epithelial cells of the bronchus and in serous cells of the bronchial glands but not in the alveolar epithelium. Immunoreactive amylase was also found in the cytoplasm of the ciliated epithelium of the fallopian tubes, especially in the apical part of the cytoplasm and in ciliary vesicles. Immunoreactive amylase was also found to be present in the surface epithelial cells and glands of the uterine cervix, as well as in the superficial part of the endometrial glands. The distribution of amylase activity revealed using histochemistry was similar to that observed in cryostat sections of PLP-fixed tissues after immunohistochemical staining. Amylase antigenicity was better preserved in cryostat sections of PLP-fixed materials than in formaldehyde-fixed, paraffin-embedded specimens. The results are discussed in relation to pulmonary and female-genital-tract diseases.

Nude mice bearing human primary hepatocellular carcinoma that produces hepatitis B surface, core, and e antigens, as well as deoxyribonucleic acid polymerase.

A primary hepatocellular carcinoma, from a patient carrying hepatitis B virus, was transplanted to athymic nude mice, and maintained through eight passages involving 39 mice. Hepatitis B surface and e antigens were detected in the circulation of tumor-bearing mice. Hepatitis B surface, core, and e antigens were demonstrated in tumor cells. Hepatitis B core particles were visualized in the tumor extract by immune electron microscopy, and they exhibited an activity of deoxyribonucleic acid polymerase. In the tumor tissue, deoxyribonucleic acid of hepatitis B virus was present both in integrated and extrachromosomal forms. Nude mice carrying the tumor would provide opportunities for studying the replication of hepatitis B virus and the expression of its various proteins, and also for evaluating the efficacy of virustatic drugs in interrupting the persistent infection.

Primary hepatocellular carcinoma with hepatitis B virus-DNA integration in a 4-year-old boy.

The autopsy findings in a case of hepatocellular carcinoma with hepatitis B virus (HBV)-DNA integration in a Japanese boy, 4 years and 10 month of age, are reported. The boy was an HBV carrier born in a typical familial cluster of HBV infection in Japan. He had been asymptomatic until the sudden manifestation of the liver tumor. Histopathologic examination revealed a well-differentiated, adult-type hepatocellular carcinoma without hepatic cirrhosis. HBV-DNA sequences were detected in the tumor cell DNA by the Southern blot hybridization method. This is the youngest patient with hepatocellular carcinoma with HBV-DNA integration reported to date, suggesting that HBV may manifest its oncogenic properties after a shorter incubation period than generally believed.

Glomerular lesions associated with liver cirrhosis: an immunohistochemical and clinicopathologic analysis.

Clinicopathologic and immunohistochemical analyses of the kidneys in 30 autopsy cases of liver cirrhosis of different types revealed glomerular lesions in 27 cases. Clinical nephritis was present in three cases. Characteristic diffuse mesangial proliferation with focal mesangial interposition and/or subendothelial deposits were present. Glomerular immunoglobulin deposition was observed in several cases, IgM in 23 cases, IgG in five, and IgA in 17; the latter were predominantly IgA1. Secretory component binding was found in 11 of the 17 cases with IgA deposits (65 per cent). Liver weight was related to IgA deposition (P approximately 0.05), and the volume of ascitic fluid was significantly correlated (P less than 0.05).

Neuron-specific enolase and Leu-7 immunoreactive small round-cell neoplasm. The relationship to Ewing's sarcoma in bone and soft tissue.

The authors present an immunohistochemical analysis of tissue from five cases of morphologically distinctive Ewing's sarcoma in bone and soft tissue. The mean age of the five patients was 16.6 years, with a range of 6-28 years. The tumors existed in chest wall, pelvis, and lower extremities. Two siblings with tumor in bone were clinically diagnosed as having Ewing's sarcoma. All cases had immunoreactivity for neuron-specific enolase (NSE), and four cases revealed positive staining for Leu-7. Neuron-specific enolase is highly specific for neurons and neuroendocrine cells. In addition, immunoreactivity for Leu-7, expressing a natural killer activity, has been demonstrated in peripheral nerve fibers and neuroendocrine cells. The authors suggest that NSE and Leu-7 immunoreactive small round-cell neoplasm is probably a primitive neuroectodermal tumor and should be categorized as Ewing's sarcoma in bone and soft tissue.

Development of human pancreas. Immunohistochemical study of fetal pancreatic secretory proteins.

The developmental sequence of human pancreatic secretory proteins has not previously been studied in detail. We applied immunohistochemistry to study 20 fetal and neonatal pancreas' (8th to 39th gestational weeks) using antisera against the following pancreatic secretory proteins: pancreatic secretory trypsin inhibitor (PSTI), serine proteinases (trypsin, chymotrypsin, and elastase I), and amylase. PSTI was first detected in developing buds of the pancreas during the 8th gestational week, and proteinases were observed in acinar cells during the 14th week of gestation. Immunoreactivity for both PSTI and proteinases was found in most acinar cells soon after their appearance. Immunoreactivity for amylase could not be detected in fetal or neonatal pancreas tissue. PSTI was also found in developing islets during the 14th gestational week, but the number of immunoreactive cells had decreased by term. Cells positive for serine proteinases were occasionally in contact with islets in second-trimester fetuses. In discussing these results, we give particular attention to the nonparallel appearance of secretory products in the fetal pancreas, and the significance of cells immunoreactive for secretory proteins in endocrine islets.
