Surgical treatment of double-outlet left ventricle in situs inversus [I,D,D].

A case of double-outlet left ventricle [I,D,D] with a subaortic ventricular septal defect and pulmonary stenosis is reported. The diagnosis was made at operation and was reconfirmed postoperatively by two-dimensional echocardiography. Repair was achieved by patch closure of the ventricular septal defect and enlargement of the functional right ventricular outflow tract using a valved patch. The postoperative course was uneventful except for a transient complete heart block. To our knowledge, this represents the second instance of surgical treatment of double-outlet left ventricle [I,D,D] reported in the literature.

A long-term follow-up study of valve replacements.

We have studied the long-term results of valve replacement operations in 591 patients, one year or more after surgery, and also analyzed rates of social return in 523 patients (response rate, 86.1 per cent) by questionnaire. Of the 591 patients, 81 patients suffered early death (mortality rate, 13.7 per cent) and 41 patients late death (mortality rate, 6.9 per cent). The causes of the latter were left atrial thrombus (18 cases), heart failure (10 cases), artificial valve failure (five cases), renal failure (two cases) and hepatic insufficiency, infection and accident (one case each). Second replacement was performed in 18 patients (3.0 per cent), 49 months on average from the first operation. The survival rate at the 12th postoperative year was 68.7 per cent. The overall rate of social return after valve replacement was 64.8 per cent, including 74.8 per cent for mitral valve replacement, 54.3 per cent for aortic and 51.5 for multivalvular. The rate of social return has been improved to 76.4 per cent in the last five years.

Phorbol ester-induced differentiation of a non-T/non-B human leukemic cell line (REH) to macrophage-like cells.

Treatment of non-T/non-B human leukemic cell line REH with 5 X 10(-9) M 12-O-tetradecanoylphorbol-13-acetate (TPA) at 37 degrees resulted in their adherence to culture flasks by 24 to 36 hr and, after 72 hr, the entire surface of the flask/coverslips became covered with macrophage-like cells containing pseudopodia. Wright-Giemsa-stained untreated cells had blast morphology, whereas TPA-treated cells (adherent or excess cells remaining in suspension) had characteristic morphology of macrophages and phagocytized large numbers of latex beads. Untreated REH cells were negative for nitroblue tetrazolium reduction, Sudan Black B, and peroxidase, and they were weakly positive for periodic acid-Schiff, acid phosphatase, chloroacetate esterase (pH 6.8), and nonspecific (naphthol AS-D acetate, pH 6.8) esterase, whereas TPA-treated cells (adherent or in suspension) gave strong reaction for these stains except for peroxidase and chloroacetate esterase which showed moderate reaction. Furthermore, the nonspecific esterase activity of TPA-treated cells and weak activity in 10% of untreated cells was strongly inhibited by NaF, a characteristic of monocytic series of cells. Lysozyme activity was not detected in culture supernatant from control or TPA-treated cells. No cytoplasmic immunoglobulin was detected in untreated or TPA-treated cells, and the monocyte/granulocyte antigen (detected by MCS-2 monoclonal antibody) which was absent from untreated REH cells was expressed in TPA-treated cells. TPA-treated cells lost common acute lymphoblastic leukemia antigen but showed significantly elevated expression of histocompatibility locus DR antigen. Terminal transferase estimated by immunofluorescence and biochemical assay was high in untreated REH cells, whereas TPA-treated cells were negative in terminal transferase immunofluorescence and had only negligible terminal transferase activity in biochemical assay. All these changes in REH cells observed on TPA treatment represent the differentiation of a human leukemic non-T/non-B-cell line to macrophage-like cells for the first time which indicates that some non-T/non-B acute lymphoblastic leukemia cells may have latent monocyte-like phenotype.

[Morphological studies on the pathogenesis and treatment of hepatolithiasis].

Review of 108 patients with hepatolithiasis showed a recent increase of primary intrahepatic gallstones. 55 per cent of cases with hepatolithiasis had their gallstones in the left intrahepatic bile ducts. Clinicopathological study on the resected hepatic specimens of 33 patients revealed numerous intrahepatic periductal glandular formations. Periductal glandular formations were classified into the intramural and extramural glands. The mucous substances which might had been released from the periductal glands seemed to play a role in the formation of stones in combination with bilirubin pigments, cholesterin, bacterial organisms, cellular debris and other bile component. Intrahepatic gallstones and extramural glands were seen in the intrahepatic segment and area ducts. The defunctionalized atrophic hepatic lobe or segment should be resected in order to remove the calculi completely and to prevent the recurrence.

[Valvuloplasty for congenital mitral valvular diseases].

Valvuloplasty is first applied as the surgical treatment for congenital mitral regurgitation (MR). Follow up study after the operation around the valvuloplasty was performed in the patients with congenital MR. In the patient under 12 years old, there were isolated MR in 3; MR associated with VSD in 14; MR and ASD in 3; MR with PDA in 10; and MR with ECD in 36. Operative methods were suturing of mitral cleft in 27; mitral annuloplasty in 17; mitral valve a, advancement in 7; cusp plasty in 4; no treatment for the MR in 14. Early death was noted in 6 cases (9.0% of mortality) and late death was noted in 3 cases (mortality rate of 9.0%). Conclusions were as follows: Surgical treatment was required in the patients with MR more severe than Sellers' Grade-II. Single suturing was recommended for the suturing of mitral cleft because shortening of the valve occurred in a few degree and the residual regurgitation reduced. Mitral valve advancement by elongation of the valvular apex was useful for the impediment of the regurgitation caused by incompetence of mitral coaptation. Mitral annuloplasty increased the effect for the impediment of the regurgitation. Ultrasonic cardiogram was useful for the follow up after the operation of the mitral valvuloplasty.

[A case of subependymoma of the septum pellucidum].

A 72-year-old-female was admitted because of slight disorientation and urinary incontinence. CT scan on admission revealed isodensity mass in the portion of the septum pellucidum and dilated lateral ventricle with patcy enhancement. A patient underwent the right frontal osteplastic craniotomy and subtotal removal of the tumor. Microscopical examination of the removed tumor tissue revealed a typical finding of subependymoma. In the previous literature subependymoma of the septum pellucidum has been reported only seven cases. We reported a rare case of subependymoma derived from the septum pellucidum, and histopathology and symptomatology of the tumor were discussed with the literature.

[2 autopsy cases of congenital muscular dystrophy of Fukuyama type--a typical and an atypical cases].

Two autopsy cases of congenital muscular dystrophy of Fukuyama type (F-CMD) were described. The first case was diagnosed clinically and pathologically as its typical case. Neither his family history nor the history of his prenatal period were contributory. He had suffered from muscle weakness and atrophy since his birth. Serum CPK was markedly elevated. EMG and muscle biopsy proved dystrophic changes of the skeletal muscles. In addition, he manifested mental retardation and attacks of convulsion. EEG failed to elicit remarkable changes, but PEG represented ventricular dilatation. He died of respiratory insufficiency at age 12. His postmortem examination showed variegated anomalies in the nervous system. Extensive micropolygyria was present in the cerebrum and cerebellum accompanied by adhesions between the bilateral cerebral hemispheres. Assymmetry of the longitudinal fibers was pointed out in the pontine base. Anterior horn cells were atrophic and moderately depopulated. On the other hand, the second patient was an atypical F-CMD case in symptoms, signs and pathology. His grand-mothers on both father's and mother's sides wee first cousins. His three siblings showed no similar disorders. His mother developed slight gestational toxicosis in the sixth and seventh months of pregnancy. His muscle weakness, contracture of the bilateral hip-joints and clubfoot had been observed since his birth. Physical and neurological examinations at age 6 showed deformity of the skull, myopathic face, macroglossia, high-arched palate, pigeon chest, scoliosis of the thoracic spine. In addition, generalized muscular atrophy, hypotonia and areflexia were recognized. Pseudohypertrophy of the muscles was absent. Sensation was intact to all modalities. Serum CPK and LDH were moderately increased.(ABSTRACT TRUNCATED AT 250 WORDS)

[Surgical treatment of infective endocarditis in childhood--report of 3 cases].

This is the case report of 3 surgical patients with infective endocarditis in childhood. Case 1: A 8 year-old boy was admitted with high fever and congestive heart failure. Aureus staphylococci were identified by blood culture. Echocardiogram showed a vegetating mass on the posterior mitral leaflet. This patient was cured by emergency mitral valve replacement during the active phase of infection. Case 2: A 3 year-old girl was admitted with infective endocarditis related to VSD. Viridans streptococci were identified by blood culture. Echocardiogram showed a vegetating mass on the septal tricuspid leaflet. After successful antibiotic therapy, this patient underwent the closure of VSD and tricuspid valvuloplasty. Case 3: A 7 year-old boy was admitted with infective endocarditis related to VSD. Aureus staphylococci were identified with blood culture. Echocardiogram showed a vegetating mass on the anterior tricuspid leaflet and moderate tricuspid regurgitation. After successful antibiotic therapy, this patient underwent the closure of VSD and tricuspid valvuloplasty. We believe that echocardiography plays an important role in the diagnosis and management of infective endocarditis and that tricuspid valvuloplasty is the method of first choice in treatment of the patient with regional tricuspid infective endocarditis.

Sea urchin (Anthocidaris crassispina) egg zinc-binding protein. Cellular localization, purification and characterization.

Gel-filtration analysis of cytosol fraction obtained from unfertilized sea-urchin (Anthocidaris crassispina) eggs on Sephadex G-75 revealed the presence of two Zn-binding-protein fractions. The major Zn-binding protein fraction had a low molecular weight and a low absorbance at 280 nm, properties similar to those of the metallothionein found in the regenerating rat liver. These fractions were further purified by DEAE-cellulose and Sephadex G-50 chromatography. Homogeneity of the Zn-binding protein was judged by polyacrylamide-disc-gel electrophoresis and gel-permeation chromatography in the presence of 6 M-guanidinium chloride. The molecular weight determined by gel-permeation chromatography was 3900. This value is in good agreement with the minimum molecular weight calculated from the amino acid composition, which was 3655. Zn-binding protein is composed of 36 amino acid residues and the distinctive features include an extremely high content of cysteine, which accounted for one-third of the total amino acid residues, and a complete absence of aromatic amino acids, as well as of methionine, histidine and arginine. Zn-binding protein contained 4.1 g-atoms of zinc per mol and a trace of cadmium, but no copper, iron or calcium. The molar ratio of reactive thiol groups to metal ion was calculated to be 2.73:1. Possible roles of this Zn-binding protein in the homoeostasis of zinc in unfertilized sea-urchin eggs are discussed.
