Mild exercise decreases plasma endogenous digitalislike substance in hypertensive individuals.

Changes in a plasma endogenous digitalislike substance were investigated in relation to the antihypertensive mechanism of mild exercise. Fifteen women with mild essential hypertension and seven normotensive female volunteers were divided into exercised hypertensive (n = 10), nonexercised hypertensive (n = 5), and nonexercised normotensive (n = 7) groups. A 4-week general clinical observation period preceded the study period of 10 weeks. The exercised hypertensive individuals were treated with a lactate threshold exercise that corresponded to approximately 50% of the maximum oxygen consumption three times a week, whereas the nonexercised groups were observed at the outpatient clinic as control groups. In the exercised group, systolic blood pressure fell by 7 mm Hg (p = 0.05), diastolic by 6 mm Hg (p less than 0.01), and mean blood pressure by 7 mm Hg (p less than 0.01) after 10 weeks. The reduction in the plasma endogenous digitalislike substance was significant after 7 (-1.02 ng/ml, p less than 0.05) and 10 (-1.04 ng/ml, p less than 0.05) weeks in this group. It positively correlated with the reduction in diastolic (r = 0.70, p less than 0.05) or mean (r = 0.66, p less than 0.05) blood pressure and with changes in plasma norepinephrine (r = 0.76, p less than 0.05). The mean corpuscular volume of erythrocytes decreased (-1.7 fl, p less than 0.01) after 10 weeks of exercise, and the plasma volume index tended to decrease (-108 ml/m2, p = 0.28). In the control groups, significant changes in blood pressure and plasma endogenous digitalislike substance were not observed.(ABSTRACT TRUNCATED AT 250 WORDS)

Identification and characterization of high molecular weight forms of basic fibroblast growth factor in human pituitary adenomas.

Basic fibroblast growth factor (bFGF) extracted from 23 human pituitary adenomas and 2 nontumorous anterior pituitary glands was examined by Western blot analysis to elucidate their molecular forms. Four bands that specifically interacted with anti-bFGF antibody were identified at the regions of 16K, 18K, 22K, and 24K mol wt. These immunodetectable bFGFs were bound to a heparin-Sepharose affinity column and eluted with 1-2 M NaCl. These partially purified tumor extracts could stimulate dose-dependently the growth of FGF-responsive cells. Some tumors contained bFGF with a mol wt of 24 K as a dominant form. In contrast, a major form of bFGF at the region of 18K was identified in the nontumorous anterior pituitary glands. We also addressed the possible presence of FGF receptor mRNA using Northern blot analysis. Many, but not all, human pituitary adenomas were found to contain detectable levels of FGF receptor mRNA with a size of 3.5 kilobases. The present results suggest that the role of bFGF in growth and hormone secretion in human pituitary adenomas should be examined using high mol wt forms of bFGF.

Penetration in vitro of newly excysted juvenile flukes of Japanese Fasciola sp. through ligated intestines of rabbits, mice, rats and chickens.

Ligated intestines of rabbits, mice, rats and chickens were used to examine the penetration of newly excysted juvenile flukes of Japanese Fasciola sp. in vitro. In rabbit intestines, the penetration rate was relatively high in the rectum and duodenum. Penetration rates in the jejunum, ileum, cecum and colon were comparable to those in the rectum and duodenum, although it was lower in the appendix. In the case of mouse, juvenile flukes penetrated the duodenum, jejunum, cecum, and rectum at considerably high rates. In rat intestine, penetration by flukes was less in the duodenum and rectum, although flukes were detected in the jejunum. In chicken intestine, flukes barely penetrated the duodenum, jejunum and rectum. Consequently, newly excysted flukes of Fasciola sp. seem to penetrate any region of the intestine in rabbits and mice. In rats, the middle small intestine may be the site suitable for flukes to penetrate. In chickens, the difficulty in penetration of the intestinal wall may be one of the reasons why chickens are scarcely infected with Fasciola sp.

Factors contributing to the resistance of chickens to infection with Japanese Fasciola sp.

Attempts were made to clarify the factors contributing to the resistance of chickens to infection with Japanese Fasciola sp. Infection was not successfully established in chickens by oral inoculation of metacercariae, nor by inoculation of excysted juvenile flukes into the body cavity or to the liver surface. Many metacercarial cysts were detected within two days in the feces of orally inoculated chickens. In the in vitro excystation test with chicken bile at 42 degrees C, metacercariae emerged successfully. These results indicate that the major resistant factors may not act during the migration from the mouth to the liver. Histopathological examination of the liver of experimental chickens could not prove the effect of a resistant factor. Excysted flukes were cultivated at 37-42 degrees C in RPMI1640 supplemented with calf serum, with the result that the survival rate of flukes fell with higher temperatures. When chicken serum was used instead of calf serum, flukes survived for a long period of time at 37 degrees C, while all died within four days at 42 degrees C. The higher body temperature of chickens than that of other mammalian hosts is considered to be the major factor contributing to the resistance of chickens to infection with Fasciola sp.

Intracranial and intraspinal dissemination of an ACTH-secreting pituitary tumor.

A case of a 29-year-old man with an ACTH-producing pituitary tumor disseminated into the subarachnoid space is described. After total adrenalectomy for Cushing's disease at the age of 15, Nelson's syndrome developed. Transsphenoidal adenomectomy at 17 and 21 years of age, pituitary irradiation and medical therapies with sodium valproate, baclofen and bromocriptine failed to lower his plasma ACTH level. Multiple intracranial and intraspinal tumors associated with the symptoms of left hemiparesis developed. The removal of a tumor grown at the level of C1-3 was performed with successful palliation of his symptoms. Histologically, the tumor cells showed sinusoidal, papillary and diffuse patterns with a preponderance of the former over the latter two, although the papillary pattern predominated in the primary pituitary tumor. Immunohistochemical analysis demonstrated most cells to be positive for ACTH in the metastatic tumor as well as the primary adenoma. The clinical significance of his course is discussed with a review of 11 reported cases with metastatic ACTH-producing pituitary tumors.

The changes of gasoline compounds in blood in a case of gasoline intoxication.

A 44 year-old man, found in his gasoline vapor filled car, died of multiple organ failure on d 9. Blood samples from d 2-7 were analyzed by gas chromatography/mass spectrometry. The estimated half-lives of gasoline, toluene, ethylbenzene, m,p-xylene, and o-xylene were 16.9, 21.4, 27.5, 32.8, and 25.8 h respectively. The estimated initial gasoline concentration in blood was 247 micrograms/ml.

[Determination of bromide ion concentration in urine using a head-space gas chromatography and an ion chromatography--biological monitoring for methyl bromide exposure].

A head-space gas chromatography (GC) and an ion chromatography coupled with a conductive detector (IC) were used to evaluate levels of bromide ion in urine. The GC method followed by methylation with dimethyl sulfate, showed higher accuracy (2.7% of relative standard deviation) and lower minimum detection limit (0.4 mg/l) compared to the IC procedure of those values which were 8.7% and 1.0 mg/l. The correlation coefficient between the analytical results of 15 urinary samples by the two methods was 0.793. The bromide ion concentrations detected in 36 urine samples of workers exposed to methyl bromide were averaged at 13.3 +/- 7.7 mg/l. The average bromide ion of the non-exposed group was 7.1 +/- 2.1 mg/l (n = 6). Exposed methyl bromide concentrations of workers were monitored with passive samplers during their work shifts (8 hr). No significant correlation between exposed methyl bromide and bromide ion concentrations in urine was observed.

Tuberculous liver abscess with bronchobiliary and gastrobiliary fistulae--a case report.

A rare case of a primary tuberculous liver abscess with bronchobiliary and gastrobiliary fistula formation is presented herein, emphasizing the difficulty in establishing its diagnosis. The clinical course was complicated and a wide range of antibiotics administered repeatedly, based on the initial diagnosis of a liver abscess, until the final diagnosis of a tuberculous liver abscess was established by histopathological reinvestigation of the resected liver specimen. Thereafter, antituberculous chemotherapy proved extremely effective for the alleviation of symptoms, reduction of the abscess and closure of the fistulae.

Induction of hepatocyte mitosis in intact adult rat by interleukin-1 alpha and interleukin-6.

In the adult rat the liver is normally quiescent, but it proliferates rapidly in response to partial hepatectomy (PH). A hepatectomized rat is subjected to stress by the operation. We have examined the effects of acute phase cytokines. To investigate the mediation of hepatocyte growth, recombinant human interleukin-1 alpha (IL-1) and interleukin-6 (IL-6) were injected into male rats. Administration of IL-1 or IL-6 followed by NH4Cl and glucagon could induce hepatocyte mitosis 30 h after the first injection. This activity was lost when interleukins were exposed to 90 degrees C for 30 minutes. NH4Cl augmented the effects of IL-1 and IL-6. These results suggest that the IL-1 and IL-6 are important mediators of liver regeneration after PH. We present a hypothesis for the triggering mechanism of hepatocyte proliferation.

A single nucleotide substitution in the D domain of estrogen receptor cDNA causes amino acid alteration from Glu-279 to Lys-279 in a murine transformed Leydig cell line (B-1 F).

A polymerase chain reaction method was carried out to address the possible presence of estrogen receptor (ER) mutations in murine transformed cell lines. The segment of ER cDNA coding the C and D domains was amplified and cloned into pUC 19. Mammary carcinoma cell line (SC-3) did not show any mutation in this segment. However, the sequence analysis of ER in the Leydig cell line (B-1 F) revealed a single base change at acidic Glu-279 (GAA) to basic Lys-279 (AAA) compared with murine uterus ER cDNA. The biological significance of this mutation is discussed.

Retinoic acid acts synergistically with 1,25-dihydroxyvitamin D3 or antioestrogen to inhibit T-47D human breast cancer cell proliferation.

Although retinoic acid has been shown to inhibit proliferation in human breast cancer cells, the mechanisms by which these effects are mediated are not known. Since several steroid hormones and their synthetic antagonists also inhibit proliferation of human breast cancer cells, we investigated the interactions between retinoic acid, 1,25-dihydroxyvitamin D3 [1,25-(OH)2D3] and antioestrogens in the control of human breast cancer cell proliferation in vitro. When T-47D cells, the most sensitive of six human breast cancer cell lines to the growth inhibitory effects of retinoic acid, were treated with retinoic acid and 1,25-(OH)2D3, a synergistic inhibitory effect on cell growth was observed. Retinoic acid also enhanced the growth inhibitory effect of various antioestrogens (4-hydroxytamoxifen, 4-hydroxyclomiphene or LY117018). However, retinoic acid did not affect oestradiol-induced growth stimulation. Measurement of the cellular receptors for 1,25-(OH)2D3 and oestrogen revealed no significant change in receptor levels following treatment with concentrations of retinoic acid which modulated growth. These results indicate that retinoic acid not only has direct growth inhibitory effects on breast cancer cell proliferation but also augments the effects of some other known regulators of breast cancer cell replication including 1,25-(OH)2D3 and antioestrogens. Synergism appears to involve interactions with steroid hormone action distinct from changes in steroid hormone receptor levels.

Functional abnormality of glucocorticoid receptor in Shionogi carcinoma 115 cells as evidenced by gene transfer experiments.

The assay systems for steroid receptor functions in steroid-sensitive cells (SC-3 cells) were developed in which hormone-responsive element linked to a reporter gene [chloramphenicol acetyl transferase (CAT) gene] was transfected by the electroporation technique. Stimulation with androgen of SC-3 cells transfected with mouse mammary tumor virus promoter-CAT gene (MMTV-CAT) resulted in clear enhancement of CAT activity, whereas glucocorticoid required abnormally high concentrations to obtain significant stimulation. The simultaneous addition of glucocorticoid surprisingly inhibited androgen-induced CAT activity in SC-3 cells, whereas glucocorticoid and androgen acted together synergistically to activate CAT activity in T 47D cells. When SC-3 cells were cotransfected with the expression vector of human glucocorticoid receptor (GR) gene, inhibition with glucocorticoid of androgen-enhanced CAT activity was abolished. These results would suggest that SC-3 cells contain functionally abnormal GR.

Amyloid enhancing factor (AEF). Isolation and biochemical and pathological characteristics.

Neutrophils and spleens were prepared from mice after treatment to induce amyloid deposition. The deposition of amyloid was accelerated in normal recipients by intravenous injection of more than 2 x 10(4) neutrophils, and intraperitoneal injection of supernatants obtained by homogenization and centrifugation of the neutrophils and spleens. The supernatants were subjected individually to DEAE ion-exchange chromatography. Amyloid enhancing factor (AEF) activity was present in peaks eluted at an NaCl concentration of 0.17 M. The fractions containing AEF were subjected to high-performance liquid chromatography (HPLC), and AEF was eluted at a position corresponding to about 15 KDa. Purified AEF was analyzed by amino acid sequencing and gas chromatography. The N-terminal amino acid was blocked, and AEF contained some saccharides including glucose, mannose, galactosamine and sialic acid, and an undefined substance (probably derived from certain proteins). Immunoelectron microscopy by the pre-embedding method using an anti-AEF antiserum demonstrated that the cytosol, but not primary and specific granules in neutrophils from the spleen of amyloid-laden mice, reacted with the antiserum. These findings suggest that AEF is a glycoprotein associated with neutrophils.

Changes of dopamine and atrial natriuretic factor by mild exercise for hypertensives.

Changes of humoral factors related to the regulation of fluid volume were investigated in exercise training for hypertensives. Twelve patients with essential hypertension were treated with an aerobic exercise for 10 weeks. A significant reduction in blood pressure from 161 +/- 3/100 +/- 2 mmHg at week 0 to 142 +/- 5/94 +/- 3 mmHg at week 4 was observed which continued until week 10. Urine dopamine was increased significantly at the 4th week from 386 +/- 29.4 micrograms/day at week 0 to 524 +/- 46.3 micrograms/day and plasma atrial natriuretic factor (ANF) was significantly reduced at the 4th week, from 41.5 +/- 2.7 pg/ml at week 0 to 32.6 +/- 3.7 pg/ml. Plasma volume was found reduced significantly from 2,531 +/- 166 ml/m2 at week 0 to 2,221 +/- 165 ml/m2 at week 10. These results suggest that the increase of dopamine and reduction of plasma ANF which took place at the early stage might be related to, at least in part, the depletion of plasma volume and the reduction of blood pressure in mild exercise for hypertensives.

[Coronary artery bypass grafting combined with operation for non-cardiac disease].

During the last two years and six months, ten patients underwent operation for both coronary artery disease and non-cardiac surgery. There were six patients with vascular disease, three with gastric cancer and one with cholelithiasis. Six patients had simultaneous operation of both coronary artery bypass grafting (CABG) and non-cardiac surgery, and four underwent non-cardiac surgery mean of 105 days after CABG. The number of significant lesions of coronary artery in patients with simultaneous or two-staged operation was 2.2 +/- 0.8 or 2.8 +/- 0.5 per patient, respectively. The operative time in patients with simultaneous operation was 361 +/- 49 minutes. However, in patients with staged operation the time for cardiac surgery was 380 +/- 36 minutes and the time for non-cardiac operation was 388 +/- 83 minutes. There were no patients with complications after either cardiac or non-cardiac operation. In patients having surgical indications for both coronary artery and non-cardiac diseases, CABG should be performed prior to the non-cardiac surgery. In patients with normal cardiac function and sufficient operative time after CABG, concomitant operation seemed to be recommended. In case with advanced malignant disease or incompetent cardiac function, staged operation would be selected for good surgical result.

[A case of brain infarction associated with viral encephalitis: MRI and pathological findings].

A 10-year-old girl had an infarction in the left brain during an acute viral meningoencephalitis. She initially showed seizure, unconsciousness and fever, and right hemiplegia gradually developed. She died at the 48th day of disease from respiratory disturbance and renal failure. Cranial MRI during the acute phase of the disease, when there was no clinical sign of right hemiplegia, showed a high intensity lesion in the left parietal and occipital areas on T-2 weight image. Autopsy disclosed the findings suggesting viral encephalitis, including multiple focal necrosis, perivascular round cell infiltration, proliferation of glial cells and spongy degeneration with some intranuclear inclusion bodies, and infarction in the left hemisphere. These findings suggest that T-2 high intensity lesion on MRI reflected infarction.
