Doppler umbilical artery velocimetry in fetuses with polyhydramnios.

Polyhydramnios is a condition of multiple etiologies, many of a benign nature, but some of which are incompatible with life. To evaluate Doppler velocimetry results as a prognostic parameter in these fetuses, we reviewed all of our cases of polyhydramnios that underwent Doppler analysis in the third trimester. Fifty-four fetuses were studied. Eleven (20.4%) had abnormal waveforms and 43 (79.6%) had normal waveforms. An abnormal waveform was associated with a significantly higher incidence of congenital anomalies, perinatal mortality and intrauterine growth retardation. Six of the 11 fetuses had abnormal karyotypes. Macrosomia was present in 37.2% of fetuses with normal waveforms and in no fetus with an abnormal waveform. Doppler analysis may aid in the counseling and management of patients with polyhydramnios. In cases with an abnormal ratio, the physician and patients should be prepared for a poor outcome and third trimester genetic analysis should be strongly considered.

Bactericidal activity of antimicrobial drugs in simulated urine specimens at various temperatures of incubation.

A total of 23 antimicrobial drugs were examined for bactericidal activity in midstream-urine against Escherichia coli ATCC 25922, Pseudomonas aeruginosa ATCC 27853, Staphylococcus aureus ATCC 25923, or Streptococcus faecalis ATCC 29212 at incubation temperatures of 35, 22, and 7 degrees C, respectively. The majority of chemotherapeutic agents significantly reduced bacterial inocula within 2 h at 35 degrees C. At room temperature, most drugs required 4 h for significant killing activity to ensue. At 7 degrees C only nitrofurantoin, polymyxin B, and rifampin were significantly active within 2 to 4 h following exposure. Following prolonged (overnight) incubation at 7 degrees C, however, 10 of 23 drugs (amikacin, cefotaxime, chloramphenicol, gentamicin, nalidixic acid, netilmicin, nitrofurantoin, polymyxin B, rifampin, and vancomycin) effected reduction of bacterial colony counts of greater than or equal to 50%. On the basis of these data it is recommended that urine specimens from patients with known antibacterial chemotherapy should not be refrigerated for longer than 2 to 4 h following collection.

["Functional relaxation" as a method in anthropologic medicine].

"Funktionelle Entspannung" (F.E.)--functional relaxation--according to M. Fuchs resumes Weizsäcker's "Gestaltkreis". Its medium is the body (comprising body, soul and mind) and its "rhythm", which penetrates and supports the individual human sphere. Somebody who is in agreement with his own body is also in agreement with the world around him; somebody who sets up tensions between the world and himself loses his personal rhythm. This becomes obvious in the "body-language" in the form of false tensions. In the F. E. these are not approached in a verbal way or by way of set exercises, but in each case individually--beginning at the spine and the joints and then continuing with the inner cavities and spaces. Approach and method of the F. E. are exemplified (e. g. hypertonie, asthma bronchiale, where other psychotherapeutic methods--psychoanalysis, autogenes training etc.--did not help).
