Laparoscopic suturing technique for enteral access procedures.

We have developed a technique to precisely place transabdominal-wall sutures to anchor the desired viscera to the abdominal wall during laparoscopic enteral-feeding access procedures. The use of an intravenous catheter and wire snare to achieve this is described.

An analysis of interpersonal manipulation.

The term 'manipulation' is frequently employed but rarely discussed or defined in psychiatric circles. This paper reviews previous conceptual analyses of the term by philosophers and psychiatrists, and examines its use in ordinary discourse. A series of characteristics which comprise the conceptual core of the term when it is unambiguously applied in interpersonal settings are proposed. Manipulation is contrasted with other behavior control methods such as rational persuasion and coercion, with emphasis on the role played by deception and the communicative context in which the manipulative transaction occurs. It is argued that manipulative behavior is fundamentally intentional, and the usefulness of the concept of 'unconscious manipulation' is questioned. Though the proposal that Manipulative Personality Disorder be formally recognized as a new diagnostic category is rejected, it is urged that the concept of manipulation receive wider attention and discussion within the mental health community.

Does simian virus 40 T antigen unwind DNA?

Recently developed techniques allow rapid and accurate determination of protein-induced DNA unwinding. We have used such techniques to determine whether SV40 T antigen unwinds DNA. Assays were performed with purified wild type T antigen, D2 protein and the SV80 T protein over a wide range of protein concentrations. Additionally, several DNA templates were used, including SV40 DNA, pBR322 DNA, and two plasmid DNAs containing one and three copies of the SV40 origin of replication. None of the purified proteins showed unwinding activity either in the presence or absence of ATP and Mg2+. As a positive control, we found that unwinding of pBR322 DNA by Escherichia coli RNA polymerase occurred under identical assay conditions. Protein-DNA interactions that involve unwinding of the helix are sensitive to the superhelical density of the DNA. Using a filter binding assay, we found that the efficiency of the binding of T antigen to the viral origin of replication does not depend upon the topological state of the DNA. This result further supports our conclusion that SV40 T antigen is not an unwinding protein.

The technique for growth plate sparing in soft-tissue sarcoma around the knee.

Amputation in a growing child should be performed through a joint and not above or below a joint, as is commonly the case in an adult. We describe a technique for performing a through-knee amputation in a situation in which a soft-tissue sarcoma involved a leg, reaching up to the knee joint. Soft-tissue dissection was performed above the knee, leaving a safe zone from the tumor. The distal femur was dissected free of all attachments to the tibia and leg, leaving it intact but protruding from the soft-tissue sleeve of the thigh. To be able to close the stump over the protruding distal femur, the femur was shortened in the metaphysodiaphyseal area. Follow-up over 3 years shows a good through-knee stump, tumor free, and a normal distal growth plate.
