Adult T-cell leukemia-lymphoma with mediastinal involvement and an asymptomatic chronic phase.

A black, West Indian woman with adult T-cell leukemia-lymphoma (ATLL), hypercalcemia, peripheral and retroperitoneal lymphadenopathy, and serum antibodies to human T-lymphotropic virus (HTLV) was found to have massive mediastinal adenopathy, a feature not previously reported in patients with ATLL. In addition, she had had asymptomatic leukocytosis with marked lymphocytosis for at least 6 years before presenting with full-blown ATLL. These findings broaden the clinical picture of ATLL. Cell surface-marker studies and close follow-up are recommended for patients with apparent chronic lymphocytic leukemia, especially if they have pleomorphic lymphocytosis, are younger than usual or are from the Caribbean or Japan.

Possible role for peptide-oligosaccharide interactions in differential oligosaccharide processing at asparagine-107 of the light chain and asparagine-297 of the heavy chain in a monoclonal IgG1 kappa.

The carbohydrate attached at Asn-107 of the light chain of a human myeloma IgG1 kappa (Hom) was isolated and the structure determined by 1H NMR. Two oligosaccharides were found corresponding to mono- and disialylated forms of the bisected biantennary class of glycopeptides. Both structures had Fuc alpha 1-6 linked to the GlcNAc residue attached to Asn and NeuNAc residues linked alpha 2-6. Because of the unusual nature of these structures, the Asn-297 oligosaccharides of the same IgG were prepared from Fc fragments and heavy chains. Comparison of the structures of the latter glycopeptides with structures from the same site on a second human myeloma IgG1 kappa (Tem) showed them to be quite similar in that the majority of the structures were biantennary but not bisected. We suggest that the completely bisected nature of the light-chain oligosaccharides comes from a high level of activity of GlcNAc-T-III (the enzyme responsible for the attachment of the bisecting GlcNAc) in the cells producing the IgG. We suggest a mechanism for differential glycosylation between the Asn-107 and Asn-297 sites based on the stabilization of the Asn-297 oligosaccharide in a conformation with the torsional angle omega about the C5-C6 bond of the Man alpha 1-6 linkage equal to -60 degrees. Protective effects of diazepam and valproate on beta-vinyllactic acid-induced seizures.

GABA level and the activity of L-glutamate-1-decarboxylase (GAD) (EC 4.1.1.15) were studied in brains of mice treated with beta-vinyllactic acid, a new, selective and pyridoxal phosphate-independent GAD inhibitor. Valproate and diazepam protected mice against convulsions caused by beta-vinyllactic acid although both anti-epileptic drugs antagonized neither the decrease in GABA concentrations nor the inhibition of GAD observed after treatment with beta-vinyllactic acid alone. Assuming that the anticonvulsant effect measured with both antiepileptics is GABA mediated, these results support the hypothesis of a postsynaptic enhancement of GABAergic transmission by diazepam and valproate.

Characterization of a long-term T-helper-cell line which produces IL-2 and induces immunoglobulin secretion.

Six OKT4+ human T-cell lines that require continuing PHA stimulation and TCGF for continuous growth were established. The cells from all six of these T-cell lines became smaller in size and lost the cell surface Ia antigen when they were grown in phytohemagglutinin (PHA)-depleted growth factor. These cells were unable to survive in the absence of PHA even if exogenous factor was present in great abundance. One of the cell lines (FL) was capable of providing helper functions. In the presence of PHA and phorbol myristate acetate, FL cells produced a growth factor, tentatively identified as Interleukin 2 (IL-2) by its ability to promote the proliferation of an IL-2-dependent murine T-cell line. Moreover, when FL cells were cocultured with B cells, pokeweed mitogen-induced immunoglobulin production was enhanced.

Serotonin increases intracellular Ca2+ transients in voltage-clamped sensory neurons of Aplysia californica.

Noxious stimulation of the tail of Aplysia californica produces behavioral sensitization; it enhances several related defensive reflexes. This reflex enhancement involves heterosynaptic facilitation of transmitter release from sensory neurons of the reflex. The facilitation is stimulated by serotonin (5-HT) and involves suppression of a 5-HT-sensitive K+ current (the S current). Suppression of the S current broadens the action potential of the sensory neurons and is thought to enhance transmitter release by prolonging entry of Ca2+ in the presynaptic terminals. We now report a component of enhanced Ca2+ accumulation that is independent of changes in spike shape. We have measured intracellular free Ca2+ transients during long depolarizing steps in voltage-clamped sensory neuron cell bodies injected with the Ca2+-sensitive dye arsenazo III. The free Ca2+ transients elicited by a range of depolarizing voltage-clamp steps increase in amplitude by 75% following application of 5-HT. Since it is observed under voltage-clamp conditions, this increase in the free Ca2+ transients is not merely secondary to the changes in K+ current but must reflect an additional mechanism, an intrinsic change in the handling of Ca2+ by the cell. We have not yet determined whether this change in Ca2+ handling reflects an increase in Ca2+ influx, a reduction in intracellular Ca2+ uptake, or a release of Ca2+ from intracellular stores. Regardless of the underlying mechanism, however, it seems possible that the enhancement of Ca2+ accumulation and the reduction in K+ current act synergistically in producing short-term presynaptic facilitation. Alternatively, this additional modulation of Ca2+ by 5-HT might contribute to processes such as classical conditioning or long-term sensitization that may depend on Ca2+.

The shared field: a precursor stage in the development of a psychotherapeutic alliance with the hospitalized chronic schizophrenic patient.

Psychotherapeutic work with chronic schizophrenic patients is difficult for both patient and therapist. Such patients lack many of the capacities necessary to participate in psychotherapy: motivation, observing ego, the ability to communicate verbally about thoughts and feelings, the capacity to sustain a relationship. Treatment of such patients may require phases in which such capacities are nurtured until the patient is able to form a therapeutic alliance of the ordinary kind. This paper discusses the ways in which hospitalization can be helpful during pre-alliance phases of psychotherapeutic work, through the provision of a protected setting and a team approach, in conjunction with individual psychotherapy. A perspective is offered for conceptualizing the early relationship between patient and hospital staff as a stage in the development of an alliance.

Effect of extracorporeal enzymatic deheparinization on formed blood components.

A stirred blood filter containing an immobilized enzyme, heparinase, has been used to neutralize heparin's anticoagulant activity at the outflow of an extracorporeal circuit in dogs. The hematocrit and red blood cell count remained unchanged throughout the 90-min perfusion period. Platelet and white blood cell counts decreased early in the procedure to approximately 20% of the initial levels, but then returned to 30 and 70%, respectively, of their initial values by the end of the procedure. After 24 h normal levels were reestablished. In vitro experiments with human blood were conducted to determine the principal cause of the observed decrease of formed blood components. An unstirred heparinase filter preserved platelets and white blood cells better than stirred filters possessing higher, the same, or no heparin-degrading capacity, suggesting that most of the loss of formed blood components is due to stirring and not to the heparinase or the Sepharose support on which the enzyme is immobilized.

Geriatric psychiatry in the emergency department, II: Evaluation and treatment of geriatric and nongeriatric admissions.

The records of a hospital psychiatric emergency department were reviewed for elderly and middle-aged patients who were diagnosed with organic brain syndrome (OBS) or psychiatric disorder. These records were then compared with those of elderly patients from the medical emergency department. While elderly medical patients received routine physical evaluations and laboratory testing, elderly psychiatric patients with behavioral symptoms were often referred to psychiatry before they received tests necessary to differentiate physical disorders from functional psychiatric disorders. When tests were administered to psychiatric patients, many abnormal results were apparent, indicating the possible presence of physical disease. It is recommended that all elderly patients with psychiatric symptoms undergo adequate medical evaluations to screen for physical causes of the psychiatric symptoms.

An in vitro gallium-67 lung index for the evaluation of sarcoidosis.

In the evaluation of the active alveolitis of pulmonary sarcoidosis, both the proportional lymphocyte count obtained by bronchoalveolar lavage and state of activation of the alveolar macrophage by gallium scanning are required. We injected 6 mCi (200 MBq) of 67Ga intravenously on 24 occasions in 13 patients with biopsy-proved sarcoidosis. Forty-eight hours later, patients were scanned with a rectilinear scanner and the generated scintigrams were evaluated using the NIH index. Seventy-two hours after injection, bronchoalveolar lavage was performed, and venous blood was sampled. The harvested lavage fluid was analyzed for absolute and proportional cell counts, and radioactivity was measured in blood and BAL fluid. An in vitro 67Ga index was generated and expressed as counts/100,000 alveolar macrophages/ml blood (mean, 0.0146 +/- 0.0087 SD). There was a significant relationship between the in vitro index and proportional lymphocyte BAL counts (r = 0.79; p less than 0.002) that was comparable to that obtained using the NIH index (r = 0.74; p less than 0.005). These data suggest that the in vitro index might offer a more objective assessment of 67Ga uptake by the lung, but this would require validation against clinical parameters in a prospective study.

Wilms' tumor and glomerular disease. Occurrence with features of membranoproliferative glomerulonephritis and secondary focal, segmental glomerulosclerosis.

A case of bilateral Wilms' tumor occurred in which a renal biopsy performed for proteinuria showed features of membranoproliferative glomerulonephritis (MPGN) type 1 and focal segmental glomerulosclerosis (FSGS). A review of the literature for similar cases revealed 24 patients with Wilms' tumor associated with glomerular disease; MPGN was present in one of these cases and FSGS in six. In our patient, circulating immune complexes were detected in the serum and likely played a role in the pathogenesis of the MPGN; the FSGS may have resulted from hyperperfusion of the remaining glomeruli in a kidney that was largely replaced by Wilms' tumor.

Intramuscular myxoma. Case report and review of the literature.

Intramuscular myxomas are relatively rare, benign, soft tissue neoplasms. They must be differentiated from other, more malignant lesions, such as myxoid liposarcoma, before definitive treatment is undertaken. The treatment of choice is wide local excision.

Adult T-cell lymphoma. A case report with lymph node studies by monoclonal antibodies and ultrastructure.

A case of retrovirus-associated adult T-cell lymphoma studied by monoclonal markers and ultrastructure is reported. The patient, who was born in Jamaica and emigrated to Canada 9 years previously, had a 5-year history of asymptomatic lymphocytosis preceding the onset of rapidly progressive disease. The peripheral blood lymphocytes were predominantly pleomorphic small lymphocytes with a smaller population of transformed lymphocytes. The peripheral blood lymphocytes were typed immunologically as T-cells of helper/inducer phenotype. Immunological typing of the lymph node lymphocytes, performed on tissue sections, demonstrated a similar phenotype. Morphological study of the lymph node showed a diffuse lymphoma, encompassing a wide range of cell sizes, including giant cells. In view of the morphological heterogeneity of this condition, immunological and viral studies are recommended to confirm the diagnosis.

[Delivery of black African women in France].

The African population is a specific entity. It is younger, the women have more babies and the pelvis is frequently narrow. Caesarean section for a narrow pelvis should be considered very carefully in this population, while the idea of feto-pelvic disproportion has to be a clinical one. Radiological pelvimetry and the use of Magnin's diagram play no part in deciding which way to deliver the women.

[Symptomatology of intracranial vascular ectasias].

The ectasia of intracranial arteries shows a versatile neurological picture containing all symptoms from paraphrased deficiency of brain nerves and vegetative imbalances until the suspicion of a tumor cerebri. Two cases of our own are represented and compared with data in literature also referring on their etiology.
