[Differential diagnostic considerations in the assessment of habitual abortion].

The nonviability of fertilized eggs or the death of the embryo is a frequent event (34 to 60%). They are called recurrent or habitual in the case of more than 2 consecutive abortions. Maternal etiologic factors are discussed: Endocrine disorders (sexual hormones, diabetes, thyroid function), uterine congenital anomalies (septale uterus), uterine tumors (fibromyoma) and uterine synechia formation, cervical incompetence, infections (i.e. mycoplasma infections, chlamydia infection, cytomegalovirus, herpes simplex virus) and autoimmune-disease with antinuclear antibodies. According to the etiology the diagnostic procedures in cases of recurrent abortions consist of hormone analysis, basal body temperature, oral glucose tolerance test, thyroid function and endometrial biopsy. Sonographic examination of the uterus, hysterosalpingography, search for local and systemic infections and cytologic examination for genetic disorders of both parents are obligatory. At the end of the diagnostic scheme HLA-analysis of the parents must be included.

["Munchausen syndrome" with variable, largely neurologic symptoms].

The paper gives an account of a man, now 32 years old, who has exhibited the typical characteristics of a "Münchhausen Syndrome", increasing in intensity, since later childhood. The increasingly neurological-neurosurgical range of his predominantly simulated and manipulated complaints covers in the main seizures, attempted suicide, craniocerebral injuries, hemiplegia, and paraplegia. The patient has had four operations for a "Cauda Syndrome". The complex conditioning the complaint, and appropriate medical reaction, are discussed with critical reference to previous literature on the subject.

Indium-111-labeled leukocyte and technetium-99m-sulfur colloid uptake by a malignant fibrous histiocytoma: phagocytosis by tumor cells?

Indium-111-labeled leukocyte imaging, performed on a patient with a calcified mass in the right thigh, demonstrated labeled leukocyte accumulation in this mass. Technetium-99m-sulfur colloid imaging was performed to differentiate labeled leukocyte uptake in heterotopic bone marrow from uptake in a focus of infection. Leukocyte and sulfur colloid images were virtually identical, and the study was interpreted as without evidence of infection. Excision of the mass revealed an angiomatoid malignant fibrous histiocytoma with metaplastic bone formation. While no marrow elements were present in either the tumor or the metaplastic bone, phagocytosis of leukocytes by tumor cells was identified. Phagocytosis of leukocytes by tumor cells may be another cause of white cell accumulation in uninfected neoplasms.

[New pregnancy following previous HELLP syndrome. A case report].

The HELLP-Syndrome (H = hemolysis, EL = elevated liver enzymes, LP = low platelets) is a well known and severe form of gestosis. In this case report we analyse the positive outcome of a pregnancy after HELLP-Syndrome has happened. The obstetric management requires regular control of the complete blood count, SMAC and coagulation.

[Follow-up in HELLP syndrome].

The so-called HELLP-syndrome, a special form of severe preeclampsia, is characterized by (H-) hemolysis, (EL-) elevated liver enzymes and (LP-) low platelet count. The data of three HELLP-cases are shown, the development and outcome of the pathologic laboratory parameters are presented. After a several months control it was evident that after delivery the initial improvement of all laboratory findings proceeds and lasts. A special medical treatment is not necessary.

[High CA-125 values in miliary tuberculosis and ascites].

High levels of CA-125, the specific tumour marker for epithelial ovarian cancers are also found in inflammatory diseases of the pelvis. This case report of a 28 year-old woman with pathological CA-125 levels demonstrates the association between this finding and ascites caused by miliary tuberculosis. Tuberculostatic chemotherapy led to normalisation of the CA-125 levels within 8 weeks.
