Effects of pre-exercise listening to slow and fast rhythm music on supramaximal cycle performance and selected metabolic variables.

We examined the effect of listening to two different types of music (with slow and fast rhythm), prior to supramaximal cycle exercise, on performance, heart rate, the concentration of lactate and ammonia in blood, and the concentration of catecholamines in plasma. Six male students participated in this study. After listening to slow rhythm or fast rhythm music for 20 min, the subjects performed supramaximal exercise for 45 s using a cycle ergometer. Listening to slow and fast rhythm music prior to supramaximal exercise did not significantly affect the mean power output. The plasma norepinephrine concentration immediately before the end of listening to slow rhythm music was significantly lower than before listening (p < 0.05). The plasma epinephrine concentration immediately before the end of listening to fast rhythm music was significantly higher than before listening (p < 0.05). The type of music had no effect on blood lactate and ammonia levels or on plasma catecholamine levels following exercise. In conclusion, listening to slow rhythm music decreases the plasma norepinephrine level, and listening to fast rhythm music increases the plasma epinephrine level. The type of music has no impact on power output during exercise.

Larva currens: the usefulness of the agar plate method.

Creeping eruption caused by Strongyloides stercoralis is called larva currens, while those caused by other parasites are called larva migrans. To detect the parasite, the direct smear method was used. However, this method is not very sensitive in detecting S. stercoralis. A 62-year-old male with polyarteritis nodosa cutanea had an erythematous creeping eruption in various regions, such as his lumbar region, abdomen and thigh. The diagnosis of larva currens was made by the clinical appearance and the detection of S. stercoralis in his feces using the direct smear method and the agar plate method. This is the first reported case of larva currens in Japan. The agar plate method was useful and more sensitive than the usual direct smear method for detecting the S. stercoralis in feces and for elucidating the effects of treatment with antiparasitic drugs.

Traumatic aneurysm of the temporal artery: a report of five cases.

Five cases of traumatic aneurysm of the superficial temporal artery are reported. Four patients clearly stated that they had sustained traumatic injuries within 3 months prior to the appearance of the pulsating aneurysms; in the remaining case, more than two years had passed before the appearance of the nodule, and it was without pulsation. Color Doppler echography was very useful for observing the circulation in these aneurysms. Histologically, these cases were pseudoaneurysms composed of small vessels with internal elastic lamina and adjacent connective tissue proliferation, suggesting a break in the arterial wall.

[The effects of high-dose ifosfamide in the treatment of bone and soft tissue sarcomas].

From January 1990 to December 1995 we treated 35 patients (pts) with bone and soft tissue sarcoma with ifosfamide (IFM). Sixteen patients had measurable metastatic pulmonary lesion and 9 had primary lesion. Histology of the tumor included osteosarcoma in 13 pts, Ewing sarcoma in 5 pts, malignant fibrous histiocytoma of bone in 2 pts, synovial sarcoma in 7pts, primitive neuroectodermal tumor in 2 pts, and other in 7 pts. The IFM at the dose of 12-18g/m2 (mean 15. 4g/m2) for 5 to 8 days continuous infusion was administered to patients in each treatment course. The uroprotector, mesna, was also given concomitantly in 60-100% dose of IFM. Eighteen pts received one course of IFM treatment. Other pts received 2 to 8 courses of IFM treatment at three to four week intervals. The overall response rate was 40% (PR in 14 pts, NC in 18 pts, and PD in 3 pts). The response rate of 13 pts with osteosarcoma was 54% (PR in 7 pts) and 30% in 15 pts with soft tissue sarcoma (PR in 5 pts).

Infantile perianal pyramidal protrusion.

BACKGROUND: The number of reported cases of infantile perianal eruption has been increasing. However, infantile pyramidal protrusion located in the midline anterior to the anus has apparently been reported mainly as acrochordons or skinfolds. OBSERVATIONS: Fifteen infants (14 girls and 1 boy) had a pyramidal protrusion located only in the midline anterior to the anus. Histological examination revealed acanthosis in the epidermis, marked edema in the upper dermis, and mild infiltrates in the dermis. The patients had been brought to the hospital because of swelling of the protrusion. However, all protrusions showed reduction without any treatment. CONCLUSION: Infantile pyramidal protrusion located only in the midline anterior to the anus should be distinguished from acrochordons or skinfolds based on the characteristic clinical features.

[Toxic effects of ifosfamide in the treatment of bone and soft tissue sarcomas].

A total of 64 courses of ifosfamide (IFM) treatments for sarcoma patients were evaluated for toxic effects. A dose of 100 ml of 1/2 diluted Maalox by normal saline was instilled into urinary bladder with clump of catheter for 15 min for uroprotection instead of Mesna that was not available in Japan at that time. Forty-six courses of ifosfamide treatment in 12-16 g/m2 doses with daily Maalox instillation resulted only one hemorrhagic cystitis, while 18 courses at a dose of 6-10 g/m2 of IFM without Maalox eventuated in 5 cases of hemorrhagic cystitis. Forty-six courses of ifosfamide treatment at a dose of 12-16 g/m2 (mean dose of 14.4 g/m2) for 5 to 6 day continuous infusion were evaluated also for myelosuppression, nephrotoxicity, neurotoxicity, and other toxicities. Myelosuppression was acceptable, although the absolute neutrophil count of the 80% course was below 500. No patient showed neurotoxicity characterized by confusion or somnolence. No course resulted in abnormal serum creatinine elevation, although two of 46 courses caused an abnormal decrease of creatinine clearance. One patient had arrhythmia that required medical treatment after 5 courses of IFM treatment.

Late onset dermal melanocytosis: an upper back variant.

Seven elderly Japanese men with blue macules on their upper backs are reported. Histological examination showed this pigmentation to be melanosis due to the presence of spindle-shaped or oval pigment-laden cells in the dermis. Electron microscopy revealed the cells to be compatible with dermal melanocytes. We assume that this condition belongs to the category of late onset dermal melanocytosis and is an upper back variant. This concept has not been proposed before.

An unusual mechanical hyperkeratosis of the soles--health sandals keratosis.

A series of 13 patients with hyperkeratosis of both soles of the feet are presented. All patients developed hyperkeratosis after wearing the so-called "health sandals", which have multiple projections on their upper surfaces to stimulate the soles while walking. There was no evidence of hereditary or systemic factors producing the hyperkeratosis and it was concluded to be due to chronic mechanical stimulation of the soles. This side effect of the wearing of "health sandals" (health sandals keratosis) should be noted by dermatologists.

Two-dimensional FEM analysis of hydroxyapatite implants: diameter effects on stress distribution.

The effect of the diameter of hydroxyapatite (HAP) implants on stress distribution in alveolar bone was analyzed by two-dimensional finite element methods (FEM), with use of a 100-N vertical and lateral load. Since HAP is considered a bioactive material, the alveolar bone was assumed to be bonded directly to the HAP implant in the FEM model. The analysis showed that the stresses in cortical bone were generally higher than those in cancellous bone, and that the stress concentration occurred mainly in cortical bone around the neck of the implant with both vertical and lateral load. The stresses in cortical bone with lateral load were especially high-twice those in vertical load. Stresses in cortical bone decrease in inverse proportion to the increase in implant diameter with both vertical and lateral load. The results suggested that an implant with a large diameter is favorable from the standpoint of stress distribution.

Immobilization of xanthine oxidase to controlled pore-glass. Application to high-performance liquid chromatography.

Immobilized xanthine oxidase (XO) was prepared by intermolecular cross-linking to controlled pore-glass using glutaraldehyde. The performance of enzyme reactors packed with the immobilized XO were considered theoretically and investigated experimentally. Using a post-column reactor packed with the immobilized XO, simultaneous determinations of hypoxanthine and xanthine by reversed-phase high-performance liquid chromatography were achieved.

Ultrastructural studies of necrolytic migratory erythema.

Necrolytic migratory erythema is a disorder highly suggestive of glucagonoma syndrome. We carried out histologic and electron microscopic studies of the skin lesions in a 57-year-old woman with the glucagonoma syndrome. Light microscopic studies revealed hyperkeratosis, parakeratosis, formation of clefts in the upper epidermis, and scattered dyskeratotic cells. Ultrastructurally, the intercellular spaces were widened in the upper epidermis and desmosomes were reduced in number. The cytoplasm of affected cells showed vacuolar degeneration; the organelles were lysed or absent. There were scattered dark cells and dyskeratotic cells. These changes appear to represent a degenerative process of the keratinocytes.

A postmortem study of malignant glucagonoma with heart muscle hypertrophy, including chemical, histochemical, immunohistological and ultrastructural observations.

The autopsy findings in a 58 year old female with the typical glucagonoma syndrome with heart muscle hypertrophy are described. Despite remission of a skin rash and hyperglucagonemia following removal of metastatic liver tumors, the patient died of massive cerebral hemorrhage caused by uncontrolled hypertension and septic cerebral angiitis. On the basis of surgical and autopsy materials, the tumors in the pancreas and liver were defined as malignant glucagonoma by histological, histochemical, immunohistochemical, and ultrastructural studies. The pathohistological changes in several organs that might be related to the hypersecretion of pancreatic glucagon are discussed.

A case with glucagonoma syndrome--endocrine and metabolic studies.

The results of clinical endocrine and metabolic studies on a 57-year-old female with surgically and autopsy verified glucagonoma syndrome were presented. All of the clinical manifestations of glucagonoma syndrome so far reported in the literature were noted but there was no evidence indicating the presence of multiple endocrine adenomatosis. The plasma IRG level was always more than 20 times above the normal, and the IRG response to insulin and tolbutamide injection was abnormal and the results of the other endocrinological studies revealed less remarkable features, if any. The surgically removed metastatic tumor of the liver contained an enormous amount of IRG and an appreciable amount of IRI, indicating that the elevated plasma IRG was mainly of tumor origin. These results clearly indicate that in glucagonoma there is some abnormality in glucagon release from the tumor. In addition to these findings, hypocalcemia, cardiac left ventricular hypertrophy and gastrointestinal dysfunction reportedly due to hyperglucagonemia were also seen in this patient.
